The author describes the case of a girl with bilateral persistent pupillary membranes and good visual acuity. An otherwise healthy 10-year-old girl presented with persistent pupillary membranes in both eyes since childhood. The patient had ametropic amblyopia, which was treated with spectacles and part-time occlusion therapy, with the patient responding conventionally. After therapy, her best-corrected visual acuity was 20/25 in both eyes. She had stereopsis of 50 seconds of arc, orthophoria, and normal extraocular movements. The remainder of the examination was normal. Patients with bilateral

A 65-year-old female patient with major depressive disorder suffered from clonus, shivering and impaired visual acuity after 20 mg/day of paroxetine administration. The symptoms were initially regarded as further manifestations of her somatic symptoms of depression, and paroxetine was increased to 30 mg/day resulting in frequent clonus, increased shivering, serious dysarthria, ongoing impairment in visual acuity and agitation. These symptoms subsided upon paroxetine discontinuation. Ten mg/day of paroxetine rechallenge provoked dysarthria, tremor and headache, but these symptoms improved again

PURPOSE: To report a case of immune recovery uveitis (IRU) in an iatrogenically immunosuppressed human immunodeficiency virus (HIV)-negative patient.
METHODS: Interventional case report. One patient was diagnosed with cytomegalovirus retinitis in the left eye while receiving immunosuppressive treatment following renal transplantation. The retinitis resolved completely with systemic ganciclovir. Further reduction of immunosuppressive treatment, causing a rapid increase in CD4-T lymphocyte count, was associated in the same eye with the occurrence of IRU consisting of anterior uveitis, vitritis,

We describe the use of long-term dilation and patching therapy in the management of three cases of pediatric monocular cataract. The cataracts were felt to be visually significant in the undilated state but consistent with good formed vision in the dilated state. One patient had a congenital cataract managed from 4 weeks of life, and two patients had late diagnosed cataracts of indeterminate onset presenting with dense deprivation amblyopia. The congenital cataract patient ultimately demonstrated recognition acuity of 20/40, and both the older children ultimately demonstrated an acuity of 20/3

We present a 62-year-old woman who had implantation of an intraocular lens (IOL) with a torus of 30.0 diopters (D) in an eye after penetrating keratoplasty. The patient presented with cataract and a best corrected visual acuity of 20/400. Penetrating keratoplasty was performed in 1975 because of decompensated keratoconus. With a clear but inferiorly decentered corneal graft, visual acuity was limited because of keratoplasty-related high corneal astigmatism of 22.5@156 that could not be successfully treated with spectacles or contact lenses. A custom toric IOL with -30.0 D of cylindrical power

Chest pain is a frequently encountered emergency room presentation, of which about 15% of cases are due to acute coronary syndromes. Cardiogenic shock is a relatively uncommon complication with associated high morbidity and mortality. Emergency medicine practitioners frequently encounter critically ill patients that require quick, definitive treatment to optimize patient outcomes. These high acuity presentations often are of relatively low occurrence which makes training residents and learners challenging. Simulation-based medical education has been shown to enhance patient outcomes by teachin

A case report of a 68-year-old male who developed acute retinal necrosis (ARN) after Boston type I keratoprosthesis is presented. The procedure was performed for multiple graft failure secondary to herpetic keratitis. Clinical data including visual acuity, color fundus photography, fluorescein angiography, laboratory tests findings, and management are presented. After exclusion of other causes by laboratory workup, the patient was diagnosed with ARN most likely secondary to herpetic infection. Intravenous acyclovir and oral prednisolone were administered to the patient resulting in marked impr

Thygeson's superficial punctate keratitis (TSPK) is a recurrent bilateral corneal epithelial disease. Typically, small, multiple discrete epithelial lesions occur in the central cornea. However, dendritic corneal lesions are rare. Herein, we report a rare case of TSPK in both eyes after a unilateral dendritic corneal lesion. A 42-year-old woman presented with decreased vision and foreign body sensation in her right eye that persisted for 1 month. Her uncorrected visual acuity and best-corrected visual acuity (BCVA) were 20/160 in the right eye. Slit-lamp microscopy revealed a dendritic lesion

BACKGROUND: Ophthalmic evaluation of patients with keratoconus (KC) often reveals highly myopic and irregular astigmatic refractive corrections. Irregular corneal astigmatism and central corneal scarring in patients with KC often result in a loss of best-corrected spectacle acuity. Rigid gaspermeable contact lenses generally optimize visual acuities for patients with KC.
CASE REPORTS: Two cases are discussed of patients who manifested clinically diagnosed KC but unusually good unaided Snellen visual acuities (20/25+ or better) in both eyes.
CONCLUSION: Good unaided visual acuities are not ne

We used a multivariate statistical analysis to identify factors predicting visual outcome in 281 eyes that underwent primary repair of a penetrating ocular injury. Visual acuity of 20/800 or better on initial examination was the most important factor, with these eyes 28 times more likely to have a final visual acuity of 20/800 or better than eyes with initial visual acuities worse than 20/800. The effect of other factors differed depending on the initial visual acuity. In patients with initial visual acuities of 20/800 or better, youth (less than or equal to 18 years) was a significant predict
