The transverse musculocutaneous gracilis flap for chest wall reconstruction in male patients with Poland's syndrome.

Poland's syndrome represents a congenital unilateral deformity of the breast, chest wall, and upper limb with extremely variable manifestations. In most cases, the problem is mainly cosmetic, and the reconstruction of the chest wall should use a method designed to be performed easily and to achieve minimal scarring and donor site morbidity. We describe using a transverse musculocutaneous gracilis (TMG) flap for chest wall and anterior maxillary fold reconstruction in three male patients. In two patients, only the pectoralis major muscle was missing. In the third case, the ipsilateral latissimu

Sonographic localisation of lymph nodes suspicious of metastatic breast cancer to surgical axillary levels.

The axillary lymph node (LN) burden of breast cancer patients guides multidisciplinary management and treatment regimes. Sonographic imaging is used to identify the presence, number and location of axillary LNs suspicious of malignancy and used to guide nodal fine needle aspirations and biopsies. Axillary LNs suspicious of harbouring breast cancer metastasis can be localised to three surgical axillary levels, numbered according to their location relative to the pectoralis minor muscle and lymph flow. To sonographically identify and localise suspicious axillary LNs, an understanding of the axil

Poland syndrome with dextrocardia: case report.

Poland syndrome is a rare congenital condition presentingwith typical features including an absent costosternal head of pectoralis major andipsilateral brachysyndactyly. There are many clinical variations of the syndrome including rib defects, absence of shoulder girdle muscle and breast hypoplasia or agenesis. Dextrocardia is rarely associated with Poland Syndrome with only 22 cases being previously reported in the worldwide literature. Whereas 'classical' Poland syndrome is predominantly right sided, all cases associated with dextrocardia have been left sided. We report a further case of lef

[An unusual case of carotid arrosion bleeding].

Carotid bleeding is one of the most severe complications in ear, nose and throat (ENT) surgery. It is a rare complication in patients with cancer of the head and neck after radiochemotherapy. We report the case of a 65-year-old man who suffered from cancer of the tonsils (pT2pN1M0) and was treated in 1987 with surgery and local chemotherapy. Since then he reported recurrent bleeding in the left cervical region especially during physical exertion. The patient was re-operated and during surgery the tip of a catheter was found in the external carotid artery, obviously a remnant from a catheter fo

Poland's syndrome and gastric cancer: report of a case.

Poland's syndrome, a rare congenital anomaly characterized by pectoralis muscle defect and ipsilateral hand abnormalities, has been reported in association with various malignancies. Gastric cancer associated with Poland's syndrome has not been described previously. To our knowledge, the case of the 21-year-old man we describe herein represents the first report of Poland's syndrome associated with gastric cancer. Although previously there was no certain evidence that linked Poland's syndrome and cancer, elucidating the molecular mechanisms that cause this syndrome may further clarify the relat

The Relationship of Ultrasound Measurements of Muscle Deformation With Torque and Electromyography During Isometric Contractions of the Cervical Extensor Muscles.

OBJECTIVES: Speckle tracking analysis (STA) of ultrasound (US) images quantifies the longitudinal deformation of a region of muscle tissue to provide a mechanical measure of muscle activity. As the validity and reliability of this method has not yet been adequately assessed, the aim of this study was to determine the validity and reliability of STA in the dorsal neck muscles during isometric neck extension contractions. METHODS: Twenty volunteers performed 3 repetitions of isometric neck extension in a dynamometer at 10%, 20%, 40%, 60%, and 80% of maximal voluntary torque while US and surface

Coordinated activities of trunk and upper extremity muscles during walker-assisted paraplegic gait: A synergy study.

Individuals with spinal cord injury (SCI) at lower thoracic levels might walk independently with the aid of mechanical orthoses and walker by using their unimpaired trunk and upper extremity muscles (TUEM). The required motor skills and the associated subtasks of the paraplegic locomotion, however, have not been well understood yet. The purpose of this study was to investigate the coordination of the TUEM activities throughout the paraplegic gait cycle using synergy analysis. For eight paraplegic individuals (30.6 ± 11.6 years; SCI level: T12), the kinematics data and the surface electromyogra

Fully Arthroscopic Anterior Latissimus Dorsi Transfer for Irreparable Subscapularis Tears.

Latissimus dorsi (LD) transfer can be used for the treatment of irreparable subscapularis tears. This article describes a fully arthroscopic LD transfer procedure to manage irreparable subscapularis tears. The LD tendon and muscle are dissected and released arthroscopically, and care must be taken to avoid injury to the axillary and radial nerves. With the use of a portal superior to the upper margin of the pectoralis major (PM) and a portal inferior to the lower margin of the PM, detachment of the proximal PM tendon is not necessary for access to the insertion of the LD tendon. The posterior

Evaluation of Dynamic Spinal Morphology and Core Muscle Activation in Cyclists-A Comparison between Standing Posture and on the Bicycle.

(1) Background: Cycling is characterized by a sustained sitting posture on the bicycle, where physiologic spinal curvatures are modified from standing to cycling. Therefore, the main objective was to evaluate and compare the morphology of the spine and the core muscle activity in standing posture and cycling at low intensity. (2) Methods: Twelve competitive cyclists participated in the study. Spinal morphology was evaluated using an infrared-camera system. Muscle activation was recorded using a surface electromyography device. (3) Conclusions: The lumbar spine changes its morphology from lordo

IgG4-Related Sclerosing Disease of the Breast in a Male Patient.

IgG4-related sclerosing disease of the breast is a rare entity with 10 reports in the literature. We report the first case in a male patient. A 48-year-old male presented with 4-week history of palpable right upper outer quadrant breast mass associated with skin puckering. He reported a family history of breast cancer in his aunt. Ultrasound and mammography showed a spiculated 2.5-cm mass associated with skin retraction with extension to the pectoralis muscle. Ultrasound-guided core biopsy was performed. The findings were interpreted as acute and chronic inflammatory process. The patient subse

Extended latissimus dorsi myocutaneous flap reconstruction of major head and neck defects.

The purpose of this article is to familiarize th surgeon with the latissimus dorsi flap and some particularly useful applications of it. This flap has proved extremely versatile, with many uses, among them immediate coverage for extensive head and neck wounds. It has been equally successful in repairing defects in both normal and irradiated tissue. The range of the arc of utilization has been exceptional, easily reaching the skull vertex from the chest wall. However, to be able to extend this flap so far requires careful dissection of the neurovascular pedicle and intimate anatomic knowledge o

Unusual Presentation of a Rib Osteochondroma as Hard Breast Lump in a Young Male: A Case Report.

Osteochondroma arising from the rib is rare. They arise as bony outgrowths from the rib and extend either extrathoracically into the subcutaneous plane or intrathoracically compressing the lung or mediastinal structures. A 23-year-old male patient presented with complaints of breast lump since last year. On clinical examination, a hard bony projection with lobulated contour was palpable. Chest radiograph and contrast-enhanced CT showed a bony outgrowth arising from the anterior aspect of costochondral junction of the right fourth rib with displacement of pectoralis major muscle anteriorly. Ost

Chest wall reconstruction with methacrylate prosthesis in Poland syndrome.

Poland syndrome is a rare congenital malformation. This syndrome was described in 1841 by Alfred Poland at Guy's Hospital in London. It is characterized by hypoplasia of the breast and nipple, subcutaneous tissue shortages, lack of the costosternal portion of the pectoralis major muscle and associated alterations of the fingers on the same side. Corrective treatment of the chest and soft tissue abnormalities in Poland syndrome varies according to different authors. We report the case of a 17-year-old adolescent who underwent chest wall reconstruction with a methyl methacrylate prosthesis. This

Sternoclavicular joint septic arthritis: an interesting lesson in clinical anatomy.

An otherwise healthy 45-year-old man presented to our emergency department holding his left arm in abduction and external rotation. He reported pain in his left shoulder and chest for the preceding 10 days that had become much worse over the last 48 h and was eased only by holding his arm in this position. The CT of the chest revealed soft tissue swelling around the left sternoclavicular joint consistent with sternoclavicular joint septic arthritis with significant inflammatory involvement of the medial component of the left pectoralis major muscle. The patient was taken to theatre for washout

Neuroendocrine carcinoma of the thymus gland with sternal invasion.

Large-cell neuroendocrine carcinoma of the thymus is a rare cancer that is more aggressive than other thymic epithelial tumors with poorer prognosis. We report one such case in a 44-year-old male patient with invasion into the upper 2/3rd of the sternum but no endocrinopathy. Complete enbloc resection of the tumor with adherent pericardium, upper 2/3rd of the sternum and mediastinal lymphadenectomy was performed. The chest wall was reconstructed with polypropylene mesh and bilateral pectoralis major muscle advancement flap cover. A search of PubMed database with user query words 'neuroendocrin

The management of anterior chest wall deformity in patients presenting for breast augmentation.

Anterior chest wall asymmetry is sometimes encountered in patients presenting for consideration of breast augmentation. The chest wall asymmetry or deficiency may be significant enough to consider reconstruction at the same time as breast augmentation in a small number of cases. Customized and prefabricated chest wall implants have been used in a variety of conditions including Poland syndrome, pectus excavatum, and sunken anterior chest. Careful moulage preparation and on-table implant modification are needed to "seat" these implants on the skeletal chest wall under the pectoralis major muscl

Spontaneous retraction of indwelling catheters: previously unreported complications.

The development of totally implantable reservoir central venous access systems has been an important advance in patients requiring long-term central venous access. With lower rates of infection and thrombosis than those with external catheters, they have potential for greater longevity and patient acceptance. Complications such as subcutaneous prosthetic infection, infusate extravasation, and difficulty in locating and puncturing the port have been reported. We present two cases of catheter migration and withdrawal from the subclavian vein. This presumably occurred from the action of the under

[Clinical and etiological data in 21 cases of Poland's syndrome].

Clinical and etiological data of 21 cases of Poland's syndrome are reported. The anomaly was seen especially on the right side and was significantly more frequent in males than in females (4:1). The variable expressivity of the morphology of the ipsilateral hand is analyzed. Height of the hand was obtained in 18 cases. Simian crease and Sydney line are significantly more frequent in patients than in controls. These observations confirmed that unilateral isolate absence of the pectoralis major muscle is a mild expression of the syndrome. Most of the cases were sporadic. No difference was found
