The efficacy and safety of extracorporeal shock wave lithotripsy in children.

Technological advances in the design of shock wave lithotriptors have precipitated important changes in the management of urolithiasis in children. New generation lithotriptors have reduced the need for anesthesia, lowered hospitalization duration, and resulted in better fluoroscopic targeting reducing radiation exposure. Currently, shock wave lithotripsy (SWL) has become standard first line treatment for most renal and ureteral calculi in children. Herein, the literature and assess success rates, re-treatment rates, preoperative stenting, anesthesia requirements, side effects, and complicatio

Consequences of Untreated Pediatric Trigger Thumb: Case Report.

Pediatric trigger thumbs present a conundrum for hand surgeons. Surgery for trigger thumbs has inherent risks, such as infection, nerve injury, and the risks of anesthesia, but will reliably solve the problem. But is surgical intervention necessary? Would these cases resolve spontaneously, eventually, without intervention? If not, what are the long-term consequences of the inability to fully extend the thumb interphalangeal joint? We present a pediatric trigger thumb that was symptomatic for 22 years, with complete resolution of symptoms after surgical intervention. This report illustrates at

Protocol implementation for in-office manual tympanostomy tube insertion.

Bilateral myringotomy/tympanostomy tube insertion (BTI) can decrease the frequency, severity, and quality of life burden of acute otitis media and chronic otitis media with effusion for pediatric patients and their caregivers. Implementing a practice protocol for in-office BTI (IO-BTI) can benefit patients by eliminating the risks associated with general anesthesia, pre-procedure fasting, and post-anesthetic effects such as emergence delirium. However, adaptation of IO-BTI has largely relied on proprietary devices that may limit cost savings and access to care. The goal of this study was to de

Ultrasound-guided unilateral thoracic paravertebral block for postoperative analgesia in pediatric patients undergoing thoracoscopic mediastinal tumor resection: a single-center randomized controlled trial.

BACKGROUND: Thoracic paravertebral block (TPVB) has rarely been reported for pediatric mediastinal tumor surgery. This study aimed to evaluate the efficacy of unilateral TPVB for postoperative analgesia in children undergoing thoracoscopic mediastinal tumor resection, providing a potential option for multimodal analgesia in pediatric thoracic oncology surgery. METHODS: Children aged 4 to 12 years scheduled for thoracoscopic mediastinal tumor resection were enrolled and randomly assigned to either the general anesthesia group (GA group) or the general anesthesia combined with paravertebral bloc

Pediatric intensive care treatment of uncontrolled status epilepticus.

The critically ill mechanically ventilated child with ongoing seizures that are refractory to any treatment presents a distinct challenge in pediatric neurocritical care. The evidence base from randomized controlled trials on which anti-epileptic drug (AED) strategy should be used is inadequate. This review of refractory and super-refractory status epilepticus summarizes recent pediatric case series regarding definitions, the second-tier AED therapies once initial anticonvulsants have failed, and the experience of high-dose midazolam, barbiturate anesthesia, and volatile anesthetics for uncont

Comparison of Different Neostigmine Doses for Reversal of Cisatracurium-Induced Neuromuscular Block in Children Under Total Intravenous Anesthesia: A Randomized Controlled Trial.

BACKGROUND: Neostigmine is widely used to reverse nondepolarizing neuromuscular blockade in children, but the optimal dose under total intravenous anesthesia is uncertain. AIMS: The primary aim was to compare the time to full neuromuscular recovery (TOF ratio of 1.0) following administration of neostigmine at doses of 0, 10, 20, and 30 μg/kg in children at a TOF count of 3. Secondary objectives were full reversal within 10 min and adverse events. METHODS: This prospective, randomized, double-blind, parallel-group, superiority trial enrolled 120 children (2-10 years; ASA I-II) undergoing tons

Retrospective comparison between MRI examinations during radiographer-administered intranasal sedation or general anesthesia.

INTRODUCTION: In order for young children to be able to undergo a Magnetic Resonance Imaging (MRI) examination, general anesthesia is often required. The aim of this study was to compare the image quality, times, and costs of the examinations of infant brains performed with MRI either during sedation with dexmedetomidine administered by radiographers or anesthesia with propofol administered by anesthesia staff. METHODS: This study was a quantitative retrospective study of 27 consecutive standard brain examinations performed under sedation or anesthesia, involving 15 children under sedation an

Refractory Bradycardia and Unreactive Mydriasis Revealing Severe Intraoperative Neonatal Hypothermia in a Resource-Limited Setting.

Perioperative hypothermia represents a major complication in pediatric anesthesia, particularly in neonates. We report the case of a newborn who underwent general anesthesia for esophageal fistula repair (following primary repair of esophageal atresia) and developed severe hypothermia manifested by the triad of refractory severe bradycardia, bilateral areflexic mydriasis, and chest wall rigidity. Management combining active rewarming, alkalinization, and cardiovascular support resulted in a complete recovery with a progressive normalization of clinical parameters. We discuss the clinical signs…

Intranasal Premedication With Dexmedetomidine in an Adult Patient With Intellectual Disabilities: A Case Report.

Recently, intranasal dexmedetomidine (DEX) has been reported to be effective as a preanesthetic medication, mostly in healthy pediatric patients. We attempted to administer intranasal DEX premedication in this case to an adult patient with intellectual disability who previously had difficulty tolerating premedication with oral midazolam. Using an intranasal atomization delivery device (MAD Nasal, Teleflex), we administered 1.5 µg/kg of DEX intranasally and were able to achieve adequate sedation, which facilitated a smooth mask induction of general anesthesia with sevoflurane. Premedication wit

Feasibility and Safety of Using Supraglottic Airway Devices for Pediatric Patients Undergoing Magnetic Resonance Imaging: A Case Series of High-Risk Patients.

BACKGROUND: General anesthesia or sedation is commonly required for pediatric patients undergoing magnetic resonance imaging (MRI) scans, and airway management during the procedure is the highest concern for anesthesiologists owing to the limited access to the patient in the MRI unit. The use of supraglottic airway devices (SADs) has recently become more popular than endotracheal tubes; however, the feasibility of using SADs for children in MRI suites was reported only in a few studies that involved healthy patients. METHODS: We present a successful case series of 30 pediatric patients, and t

[Anesthetic Management of Children with Upper Respiratory Tract Infections].

Respiratory adverse events, such as cough, hypoxia, laryngospasm, bronchospasm and stridor, are major causes of morbidity and mortality during pediatric anesthesia. Since several studies have shown that upper respiratory tract infection is an independent risk factor for perioperative respiratory adverse events, children presenting for elective surgery with upper respiratory tract infection require a careful evaluation to decide whether or not to proceed with anesthesia. However, there are no standard guidelines to proceed with or postpone anesthesia, and thus the decision pro- cess is often ba…

Techniques of common bile duct exploration in pediatric surgery.

With the rising of incidents in pediatric cholelithiasis, pediatric surgeons are increasingly encountering patients with choledocholithiasis. Endoscopic retrograde cholangiopancreatography (ERCP) has long been considered the gold standard for diagnosis and treatment, offering rapid relief of biliary obstruction- sometimes without general anesthesia. However, ERCP is not without risks and in the pediatric population is often limited by institutional availability, occasionally requiring interfacility transfer and escalating healthcare costs. Recently, transcystic laparoscopic common bile duct ex

Parental Decision-Making for Infant Spinal Anesthesia - A Qualitative Study.

BACKGROUND: Spinal anesthesia is a viable alternative to general anesthesia for infant surgical procedures below the umbilicus. However, choosing spinal anesthesia over general anesthesia is a complex medical decision for parents. Little is known about how parents perceive infant spinal anesthesia as an alternative to general anesthesia and how they make this decision for their children. AIMS: The aim of this research study was to generate a holistic understanding of the parental decision-making process for spinal anesthesia in infants. METHODS: A qualitative research study was conducted at

Foreign body aspiration in children - being safe and flexible.

PURPOSE OF REVIEW: Anesthesia for foreign body removal in children can be quite challenging. Even though rigid bronchoscopy is considered the gold standard for foreign body removal, there is increasing evidence for successful foreign body removal using flexible bronchoscopy. This review discusses the recent implications for flexible bronchoscopy for the purpose of foreign body removal and will compare these findings to rigid bronchoscopy. RECENT FINDINGS: During the last few years, several observational studies on foreign body removal by flexible bronchoscopy have been published, with promisi
