Implementation of multidisciplinary care reduces maternal mortality in women with sickle cell disease living in low-resource setting.

Sickle cell disease (SCD) is associated with adverse pregnancy outcome. In women with SCD living in low-resource settings, pregnancy is associated with significantly increased maternal and perinatal mortality rates. We tested the hypothesis that implementing a multidisciplinary obstetric and hematology care team in a low-resource setting would significantly reduce maternal and perinatal mortality rates. We conducted a before-and-after study, at the Korle-Bu Teaching Hospital in Accra, Ghana, to evaluate the effect of a multidisciplinary obstetric-hematology care team for women with SCD in a co…

Neonatal survival in the Republic of Moldova--cost-effective evidence-based interventions.

AIM: To estimate the number of preventable neonatal deaths based on national data. METHODS: To estimate the number and proportions of avoidable neonatal deaths, we have used the same approach described by Gary L. Darmstadt, using both the official statistical data and data of some assessment studies carried out in the country. RESULTS: According to the calculations, the highest number of neonatal deaths we are able to prevent can be obtained from the implementation of evidence-based interventions in the frame of facility-based immediate neonatal and skilled maternal care (14 death prevented

Maternal and perinatal outcomes among eclamptic patients admitted to Bugando Medical Centre, Mwanza, Tanzania.

Eclampsia continues to be a major problem, particularly in developing countries such as Tanzania, contributing significantly to high maternal and perinatal morbidity and mortality. We conducted a study to establish the incidence of eclampsia and the associated maternal and perinatal outcomes among eclamptic patients admitted to our center. A descriptive cross-sectional study of all women presenting with eclampsia was performed from June 2009 to February 2010. Seventy-six patients presented with eclampsia out of a total 5562 deliveries during the study period (incidence of 1.37%). Antenatal att

Variations and inconsistencies in maternal and child health indicators in Ishikawa Prefecture, Japan.

As part of a community health assessment, variations in maternal and child health-related vital statistics data in Ishikawa Prefecture, Japan, from 1984 to 1993 were studied. Annual vital statistics reports published by the Prefectural government were used for the analysis. The following rates by public health district were calculated: abortion rate, early fetal mortality rate, late fetal mortality rate, perinatal mortality rate, neonatal mortality rate, and infant mortality rate. Maternal mortality rates were calculated for the Prefecture because of the small number of events. A few northern …

CAKUT calculator: a clinical risk prediction tool for in-hospital mortality in neonates with congenital anomalies of the kidney and urinary tract admitted to level IV NICUs.

OBJECTIVE: To predict in-hospital mortality for infants with congenital anomalies of the kidney and urinary tract (CAKUT) using perinatal factors and use the resultant model to create an interactive clinical risk calculator. STUDY DESIGN: A retrospective cohort study included 13,233 neonates with CAKUT diagnoses admitted to forty-six level IV neonatal intensive care units participating in the Children's Hospitals Neonatal Consortium from 2010 to 2024. A predictive model for in-hospital mortality was built using perinatal risk factors. RESULTS: 1353 (10.2%) neonates died. Eight perinatal fact

Gestational age assessment by ultrasound cerebellar measurements in fetal and perinatal deaths.

BACKGROUND: Perinatal mortality remains high in low- and middle-income countries. Accurate assessment of fetal gestational age is crucial to distinguish between prematurity and intrauterine growth restriction, 2 conditions commonly associated with perinatal mortality that require different preventive strategies and management. Ultrasound measurements of the cerebellum have been shown to be accurate in assessing gestational age during pregnancy, but their postmortem performance has not yet been evaluated. OBJECTIVE: We aimed to explore the feasibility and validity of gestational age estimation…

Predictors of perinatal death in the presence of missing data: A birth registry-based study in northern Tanzania.

BACKGROUND: More than five million perinatal deaths occur each year globally. Despite efforts put forward during the millennium development … birth registry between 2000-2015, 4.2% (95%CI 4.0%, 4.3%) ended in perinatal death (equivalent to a perinatal mortality rate (PMR) of 41.6 (95%CI 39.9, 43.3) deaths per 1,000 births). After the imputation of missing values, the proportion of perinatal death remained relatively the same. The risk of perinatal death was significantly higher among deliveries from mother …

An urban perinatal health programme of strategies to improve perinatal health.

Promotion of a healthy pregnancy is a top priority of the health care policy in many European countries. Perinatal mortality is an important indicator of the success of this policy. Recently, it was shown that the Netherlands has relatively high perinatal death rates when compared to other European countries. This is in particular true for large cities where perinatal mortality rates are 20-50% higher than elsewhere. Consequently in the Netherlands, there is heated debate on how to tackle these problems. Without the introduction of measures throughout the entire perinatal health care chain, pr…

Blood pressure, edema and proteinuria in pregnancy. 7. Edema-plus-proteinuria relationships.

1. A total of 488 pregnancies had the combination of two-plus or more proteinuria and edema of the hands and face. Of these, 208 were white and 280 were black gravidas. In the white gravidas, 8 fetal and neonatal deaths occurred with a perinatal mortality of 38.5 per 1,000. There were 13 perinatal deaths in the black subgroup with a perinatal mortality rate of 46.4 per 1,000. The overall perinatal mortality rate was 43.0 per 1,000, which could be compared to the overall perinatal mortality rate of 32.8 per 1,000 for the segment of the study population without edema or proteinuria. This undersc…

VBAC: what does the evidence show?

Declining rates of vaginal birth after cesarean (VBAC) are contributing to rising total cesarean delivery rates. The reasons behind the decreased utilization of VBAC are complex, but concerns about the safety of a trial of labor after cesarean are often cited. This manuscript will present a summary of existing evidence on maternal and fetal/neonatal outcomes associated with trial of labor after cesarean/VBAC, and highlight findings from recent contributions to this literature.

Gestational age at delivery and perinatal outcomes of twin gestations.

OBJECTIVE: The optimal gestational duration for twin gestations is unknown. Epidemiologic studies show that the lowest perinatal mortality rate for twins is at 37-38 weeks, but these studies lack information on pregnancy complications and neonatal morbidities. This study evaluates pregnancy characteristics and perinatal outcomes of twins in order to assess the optimal gestational age for delivery. STUDY DESIGN: This is a retrospective study of twins delivered at ≥36 weeks at our institution from 1991-2009. The composite rate of perinatal morbidity and mortality (including perinatal death, res…

Perinatal death beyond 41 weeks pregnancy: an evaluation of causes and substandard care factors as identified in perinatal audit in the Netherlands.

BACKGROUND: Late- and postterm pregnancy are associated with adverse perinatal outcomes, like perinatal death. We evaluated causes of death and substandard care factors (SSFs) in term and postterm perinatal death. METHODS: We used data from the Perinatal Audit Registry of the Netherlands (PARS). Women with a term perinatal death registered in PARS were stratified by gestational age into early-/full-term (37.0-40.6) and late-/postterm (≥41.0 weeks) death. Cause of death and SSFs ≥41 weeks were scored and classified by the local perinatal audit teams. RESULTS: During 2010-2012, 947/479,097 (0.

Does advanced maternal age confer a survival advantage to infants born at early gestation?

BACKGROUND: Recent studies have shown that older mothers who deliver at preterm gestation have lower neonatal mortality rates compared with younger mothers who deliver at preterm gestation. We examined the effect of maternal age on gestational age-specific perinatal mortality. METHODS: We compared fetal, neonatal and perinatal mortality rates among singleton births in the United States, 2003-2005, to mothers aged ≥35 versus 20-29 years. The analysis was stratified by gestational age and perinatal mortality rates were contrasted by maternal age at earlier (22-33 weeks) and later gestation (≥34…

Unplanned out-of-hospital deliveries in Finland: A national register study on incidence, characteristics and maternal and infant outcomes.

INTRODUCTION: Unplanned out-of-hospital deliveries (UOHDs) have earlier been related to higher perinatal mortality and morbidity, but recent research has not paid much attention to them. Our aim was to evaluate the incidence, characteristics, risk factors, and maternal and perinatal mortality and morbidity in UOHDs in Finland. MATERIAL AND METHODS: We conducted a national register study on births, causes of death and congenital anomalies for all live and stillbirths during 1996-2013. The study group included 1420 infants delivered by mothers with UOHDs. The 1 051 139 infants born in hospitals…
