Regular self-examination or clinical examination for early detection of breast cancer.

BACKGROUND: Breast self-examination and clinical breast examination have been promoted for many years as general screening methods to diagnose breast cancer at an earlier stage in order to decrease morbidity and or mortality. The possible benefits and harms remain unclear. OBJECTIVES: To determine whether screening for breast cancer by regular self-examination or clinical breast examination reduces breast cancer mortality and morbidity. SEARCH STRATEGY: The Cochrane Library and Medline were searched for randomised trials; date of last search October 2002. The specialised register maintained

Assessment and evaluation of hereditary sensory and autonomic neuropathies with autonomic and neurophysiological examinations.

The five different types of the rare hereditary sensory and autonomic neuropathies (HSAN) are classified by their mode of inheritance, pathology, natural history, biochemical, neurophysiologic and autonomic abnormalities. Clinically, the different types of HSANs can be identified by a detailed history and examination and 'bedside' tests of sympathetic or parasympathetic function such as active standing, metronomic breathing or the Valsalva maneuver, sensory and motor nerve conduction studies, quantitative sensory testing of thermal and vibratory perception, and the analysis of sudomotor functi

Physical examinations and laboratory tests in antenatal care visits in Denmark. Do reported practice and current official guidelines concord with results of literature reviews? A nationwide study of the public scheme of shared antenatal care in general practice, centres of midwifery and hospital outpatients' clinics.

OBJECTIVE: To analyse physical examinations and laboratory tests reported in antenatal care visits in relation to official guidelines and reviews of appropriateness. DESIGN: A nationwide cross sectional study based on questionnaires completed by general practitioners (GPs), midwives, and hospital doctors. Physical examinations and laboratory tests in connection with one specific visit were reported. SUBJECTS: The questionnaires were completed by 722 GPs (61% of eligible from a random sample), 584 midwives (86% of eligible), 250 hospital doctors who made health examinations in pregnancy, week…

Functional dystonia in musicians: rehabilitation.

For functional dystonia in musicians, rehabilitation should be principally psychomotor, including psychotherapy based on analysis of the personality and a global physical education of the corporal scheme--it is a compartmental reeducation. From the time of onset, it is of great importance to the musician that the mechanism at the origin of the problem be understood and analyzed. For the therapist, the principal goal is to identify the multiple compensations that mask the real dysfunction. When the musician and the therapist agree on the real cause of the dystonia, it becomes evident that an an

The impact of physical load on duration estimation in sport.

We investigated whether physical load has an influence on the accuracy of duration estimation of sporting activities presented in real time and slow motion. 86 participants were studied in two single sessions of 45 min each. Our results showed no general effects for physical load, when comparing physical load versus rest. However, we could replicate findings of past research (Schütz et al., 2021), showing that the duration of sports performance is estimated more accurately when presented in real time compared to slow motion. Further we found, that under physical load, participants perceiving t

[Examination of the sense of smell].

Olfactory disorders are common. Functional anosmia is present in about 5-6% of the general population, while from the age of about 55 years the olfactory sense is restricted in about 25% of cases. A precise examination is necessary to describe the patient's symptoms and ascertain possible reasons for them. As far as diagnosis and possible treatment of olfactory or gustatory disorders are concerned, the main advantage allowing progress is the availability of standardised methods of testing the chemical senses. In this article procedures for orthonasal and retronasal psychophysical examination o…

Underwater and Scuba Diving Accidents.

The evaluation and care of an injured scuba diver requires an understanding of the different types of underwater activities that may be deemed scuba diving. Such activities may range from the complex (eg, commercial or technical diving) all the way up to basic recreational scuba or snorkeling. A thorough physical examination should be completed as early as possible with a focus on specific areas at risk for injury and etiology, such as a detailed cardiopulmonary, skin, and neurologic examination. Serial reassessments and supportive care are as equally important as consultation with a dive medi…

[Pharmacological Management of Postoperative Delirium].

Postoperative delirium (POD) is a complex disorder with significant implications for health and well-being. Over the last few years, there has been a significant increase in awareness of the pathophysiological processes, the different clinical forms and the prevention of POD. It is known that POD develops when anaesthetic- and surgery-related precipitating factors coincide with the patient's predisposing vulnerability. Consequently, assessing the preoperative physical, cognitive, psychological, social and resilience capabilities of patients scheduled for surgery is critical to assessing overal

Clinical Neurology in Practice: The Tongue (Part 1).

BACKGROUND: The tongue is an essential organ for the development of certain crucial functions, such as swallowing and language. The examination of the tongue can be very useful in neurology, as the various types of lingual alterations can lead to certain specific diagnoses, the tongue being a kind of "mirror" of some neurological function. REVIEW SUMMARY: In this study, we reviewed the literature on anatomy, physiology, and the various aspects of the examination of the tongue. CONCLUSIONS: Examination of the tongue should be an integral part of the clinical examination of the cranial nerves.

Towards a core-set of mobility measures in ageing research: The need to define mobility and its constructs.

BACKGROUND: Mobility is a key determinant and outcome of healthy ageing but its definition, conceptual framework and underlying constructs within the physical domain may need clarification for data comparison and sharing in ageing research. This study aimed to (1) review definitions and conceptual frameworks of mobility, (2) explore agreement on the definition of mobility, conceptual frameworks, constructs and measures of mobility, and (3) define, classify and identify constructs. METHODS: A three-step approach was adopted: a literature review and two rounds of expert questionnaires (n = 64,

Medical assessment of fitness to dive. Part I.

Good physical and mental health is a prerequisite for anyone planning to scuba dive. A fitness to dive certificate for those willing to enter a scuba diving course as well as for active divers, either amateur or occupational, can only be issued if there are no medical contraindications to dive. It is usually within the competence of a diving instructor, a manager of underwater work or a physician to assess a person's mental and physical health and grant them permission to stay under hyperbaric conditions. The legal requirements for issuing a fitness to dive certificate are different for recrea

[Einstein's sign and other signs of aortic aneurysm].

Einstein's sign is a syndrome similar to an acute cholecystitis caused by the rupture of an abdominal aortic aneurysm. In this paper we describe the evolution of the disease that the physicist suffered till it led him to death. We also revise other signs which could be found in some patients with ruptured or dissected abdominal aortic aneurysm.
