In order to verify whether measurement of plasma fibronectin (Fn) could represent a useful tool in acute-phase-response assessment, Fn was measured in 16 previously untreated patients (group A) affected by polymyalgia rheumatica giant cell arteritis (PMR-GCA), both before, during, and after 45 days of steroid therapy, and its course was compared with the behavior of some acute-phase reactants such as erythrocyte sedimentation rate (ESR), fibrinogen (Fng), and prealbumin (Preal). Increasing numbers of patients are being recognized with neurological abnormalities associated with the immunochemical changes of plasma cell disease. Eight patients with sickle cell anemia (SS hemoglobin) were found to have decreased plasma levels of kininogen compared to normal control subjects. The kininogen level of the patients with sickle cell anemia was decreased further during sickle cell crises. The results suggest that components of the kinin system are profoundly affected in patients with sickle cell anemia, and during crises may play a role in the clinical presentation of patients.

Six patients had peripheral neuropathy and nonmalignant IgM plasma cell dyscrasia. In two of them, immunological studies indicated that the monoclonal immunoglobulin reacted with myelin-associated glycoprotein, a constituent of peripheral nerve myelin. A case history of a patient with primary plasma cell leukaemia is presented. Analysis of serum showed an IgD lambda paraprotein, and lambda-light-chains were found in the urine. Immunofluorescence studies of a bone marrow aspirate revealed intracytoplasmatic IgD of lambda-type in plasma cells. Moreover J-chain could be demonstrated in these plasma cells. 1 previously untreated patient with plasma-cell leukaemia and 8 patients with myeloma (4 previously untreated) were treated with high-dose melphalan 100-140 mg/m2 iv. All responded to treatment. 3 of the 5 previously untreated patients achieved biochemical and bone-marrow complete remissions.

beta-glucuronidase activity, estimated cytochemically, is, in more than half cases of myeloma, very increased or decreased, compared to that found in benign dysglobulinemias, where this activity is normal. A very high or very low score makes sure the diagnostic of myeloma. The relations between the beta-glucuronidase activity and the myeloma clinical stade according to Durie and Salmon and the bone lysis are discussed.
