Zygoma and Mandibular Angle Reduction: Contouring Surgery to Correct the Square Face in Asians.

Changing the facial appearance with facial contouring surgery is popular, especially in East Asian countries where a square face is a common chief complaint. Mandibular angle reduction, malar reduction, genioplasty, and chin and body contouring surgery can be performed as independent or ancillary procedures during orthognathic surgery. Many techniques have been developed and different osteotomy designs have been proposed to enhance outcomes and minimize complication risks. Here, we review the surgical techniques and considerations for mandibular angle and malar reduction, the two most commonly

Preparation for Hand Surgery Fellowship: A Comparison of Resident Training Pathways.

BACKGROUND: Hand surgery is a unique subspecialty in which one can train after completion of either a plastic, orthopedic, or general surgical residency. This study compared hand surgery experience in residency among these different training pathways. METHODS: The Accreditation Council for Graduate Medical Education case logs of graduating residents in general surgery, orthopedics, and plastic surgery were evaluated for years 2009 through 2018. Cases were grouped according to the Accreditation Council for Graduate Medical Education-defined categories for hand surgery. Comparisons between spec

Endoscope-assisted greater neurovascular palatal bundle release in cleft palatoplasty.

Performing surgery in the oral cavity is difficult because of the limited view of the surgical field. Intraoral surgery for infantile oral disorders, such as cleft palate, is even more challenging. Endoscopy provides a minimally invasive approach and clear surgical view in surgeries with a constrained field of view. To date, very few reports have described endoscope-assisted palate surgery for children with cleft palate. At the authors' institution, endoscopes have been used in primary palatoplasty using the double-opposing Z-plasty technique. A novel endoscope-assisted procedure is described

What is the anatomical basis of labiaplasty? A review of normative datasets for female genital anatomy.

BACKGROUND: Despite increasing numbers of labiaplasties being performed, there is little quantitative information on normal labial diversity to guide medical education, patient education and surgical treatment. AIM: This scoping review will determine what is known in the published literature about the anatomical basis of normal for labia and female genital cosmetic surgery (FCGS). MATERIALS AND METHODS: The scoping review identified ten population-based studies that recorded labial dimensions by searching three electronic databases utilising a Preferred Reporting Items for Systematic Reviews

Reconstruction after open surgery for skull-base malignancies.

PURPOSE: Resection of skull base malignancies that would have been associated with unacceptable morbidity and mortality in the past are now performed with reliable results due in large part to advancements in reconstructive surgery. The goal of this review is to describe the best evidence-based methods of reconstruction following open surgery for skull base tumors in order to attain improved outcomes for patients. METHODS: A review of recent studies involving reconstruction following open skull base surgeries was performed. RESULTS: Free flaps are now the most commonly recommended method for

A Personal Approach to Bilateral Cleft Lip Repair.

Dr Michael B. Lewis has been using his personal approach to bilateral cleft lip (BCL) repair for 30 years to achieve a loose, pouting upper lip, a 90-degree columellar-labial angle, and a projecting nasal tip. This article describes technique and results.

Polydactyly of the Hand.

Polydactyly is one of the most common congenital hand deformities managed by orthopaedic surgeons. It is most often found in isolation; however, rarely, it may be associated with genetic syndromes. Polydactyly is classified as postaxial, preaxial, or central depending on the radioulnar location of the duplicated digits. Postaxial polydactyly, which affects the ulnar side of the hand, is most common and is typically managed with excision or suture ligation of the supernumerary digit. Preaxial polydactyly, which affects the thumb or radial side of the hand, often requires reconstructive techniqu

TIP hypospadias repair: A pediatric urology indicator operation.

OBJECTIVE: We review development and evolution of TIP hypospadias repair, including technical changes made to improve its results. We also discuss general risk factors for hypospadias surgical complications. METHODS: We describe use of a database with prospective data entry to first identify our most common complications and their frequency, and then to monitor results of technical modifications made to reduce their occurrence. Multiple logistic regression of various factors recorded in the database was done to identify those predicting increased risk for urethroplasty complications. RESULTS

Female Urethral Diverticulum: Current Diagnosis and Management.

While urethral diverticulum (UD) affects less than 20 per 1,000,000 women overall, it is thought to represent 1.4% of women with incontinence presenting to urology practices. It is hypothesized to evolve from periurethral glands that become obstructed, infected, and dilated over time, and patients typically present with dyspareunia, bothersome lower urinary tract symptoms (LUTS), and/or recurrent UTIs. In many patients, a periurethral mass can be appreciated on exam. In recent years, magnetic resonance imaging (MRI) has become the imaging test of choice for diagnosis of UD, but ultrasound (US)

Aesthetic surgery of the female genitalia: labiaplasty and beyond.

Vaginal rejuvenation has been on the rise for the past 10 years. Women seem to prefer no pubic hair and minimal if any labia minora dangling beyond the majora. Labiaplasty or labia minora reduction is the most common procedure requested by women concerned with the appearance of their vulvar area. Safe and effective surgical procedures exist to trim the labia minora and the labia majora and clitoral hood as indicated. In this article, patient selection and consent, surgical techniques, avoidance of complications, and adjunctive procedures to further enhance the aesthetics of the region are disc

[Surgical treatment of fistula-in-ano--a journey between Skylla and Charybdis].

This review discusses the pathogenesis, symptomatology, diagnostic work-up, and treatment options for fistula-in-ano, which is a common condition that affects ~ 20 in 100,000 per year with a predominance for young males. Fistula-in-ano normally presents as an acute anorectal abscess that subsequently becomes established as a chronic discharging fistula. The illness is characterized by chronic perianal discharge and intermittent pain. The aim of surgical treatment is permanent cure of the fistula whilst maintaining patient continence. This principle forms the basis of surgical decision-making a

Expert's comment concerning Grand Rounds case entitled "Closing-opening wedge osteotomy for severe, rigid thoraco-lumbar post-tubercular kyphosis" (by S. Rajasekaran, P. Rishimugesh Kanna and Ajoy Prasad Shetty).

Prevention or correction of severe kyphotic deformity in addition to eradication of the infective focus has become the modern standard of management of tuberculosis of the spine. Circumferential excision of the kyphus is now technically feasible with the development of rigid pedicle screw fixation system and intraoperative spinal cord monitoring in the past two decades.

Urinary bladder wall substitution using autologous tunica vaginalis in male dogs.

Fresh autologous tunica vaginalis was experimentally used for partial substitution of the excised urinary bladder wall in ten male mongrel dogs. The substituted areas of two dogs were examined macroscopically and histologically at 2, 4, 6, 8, and 10 weeks after surgery. Two control dogs underwent partial cystectomy and primary wall closure without substitution. The regenerated transitional epithelium completely covered the substituted portion and smooth muscle regeneration was present at 6 weeks. The bladder walls at the closure area of one control dog and at the substituted portions of two do
