[Infection prevention following splenectomy: protective effect of a new 17-valent pneumococcal vaccine].

The antibody response to a new, 17-valent pneumococcal vaccine (Moniarix; Smith Kline-RIT) was tested in 43 splenectomised or functionally asplenic patients. The vaccine contained the following types of capsular antigens: 1, 2, 3, 4, 6A, 7F, 8, 9N, 11A, 12F, 14, 15F, 17F, 18C, 19F, 23F, 25 (Danish nomenclature). The vaccination was well tolerated. Local, reversible reddening and swelling was seen in 22 patients. The only systemic side effect was a rise of temperature in 13 patients. Serum titers of more than 300 ng/ml of specific antibodies were considered protective. A complete immunization o

Methotrexate reduces vaccine-specific immunoglobulin levels but not numbers of circulating antibody-producing B cells in rheumatoid arthritis after vaccination with a conjugate pneumococcal vaccine.

BACKGROUND: Treatment with methotrexate (MTX) in patients with rheumatoid arthritis (RA) leads to decreased total immunoglobulin (Ig) levels and impairs vaccine-specific IgG antibody levels following pneumococcal vaccination. The mechanisms by which MTX exerts these effects in RA are unknown. We aimed to evaluate whether MTX reduces vaccine-specific serum Ig levels and their functionality in RA patients following vaccination with pneumococcal conjugate vaccine, and if numbers of antigen-specific circulating plasmablasts are affected. METHODS: Ten patients with RA on MTX and 10 RA patients wit

Impact of 13-valent pneumococcal conjugate vaccine on laboratory-confirmed pneumococcal meningitis and purulent meningitis among children ˂5 years in Cameroon, 2011-2018.

BACKGROUND: The 13-valent pneumococcal conjugate vaccine (PCV13) entered Cameroon's childhood national immunization programme (NIP) in July 2011 under a 3-dose schedule (6, 10, 14 weeks of age) without any catch-up. We described the impact of PCV13 onserotype distribution among pneumococcal meningitis cases over time. METHODS: We used laboratory-based sentinel surveillance data to identify meningitis cases among 2- to 59-month-old children with clinically-suspected bacterial meningitis (CSBM) admitted to hospitals in Yaoundé (August 2011-December 2018). Purulent meningitis cases had a cerebro

Streptococcus pneumoniae serogroups 15 and 33: an increasing cause of pneumococcal infections in children in the United States after the introduction of the pneumococcal 7-valent conjugate vaccine.

BACKGROUND: After the widespread use of the 7-valent pneumococcal conjugate vaccine, replacement serotypes have emerged. Serogroups 15 and 33 have emerged as nonvaccine serotypes causing invasive disease. We describe the clinical characteristics of children with infections caused by these serogroups and determined the genetic relationship of the strains. MATERIALS AND METHODS: The United States Pediatric Multicenter Pneumococcal Surveillance Group has prospectively identified children with pneumococcal infections since 1993. Charts were reviewed retrospectively, isolates were serogrouped and

Serotype-specific Immunity and Association With Pneumococcal Carriage Following 3+1, 3+0, and 2+1 Schedules of 10-valent Pneumococcal Conjugate Vaccine.

BACKGROUND: Correlates of protection for pneumococcal carriage, a precursor for invasive pneumococcal disease, are not well-defined. The relationship between memory B cells (Bmem) with carriage remains unclear. We assessed serotype-specific Bmem, IgG, and opsonophagocytic (OPA) responses following different infant schedules of 10-valent pneumococcal conjugate vaccine (PCV10, Synflorix) and their association with pneumococcal carriage. METHODS: We analyzed secondary outcome data from a phase II/III randomized controlled trial in Vietnam (trial registration NCT01953510). Infants received PCV10

Impact of a 21-valent pneumococcal conjugate vaccine on pneumococcal disease incidence among adults 65-84 years of age in Belgium: An epidemiological modeling study.

Pneumococcal disease (PD) imposes a high clinical and economic burden in Belgium. Effective multivalent vaccines are available and recommended for adults at elevated risk of PD, but do not protect against certain clinically important bacterial serotypes. In response, a new 21-valent pneumococcal conjugate vaccine, PCV21, was designed to protect adults against a greater proportion of pneumococcal disease. This study estimated the impact of implementing PCV21 versus the currently recommended vaccine (PCV20) in Belgium on the incidence of invasive PD (IPD) and non-bacteremic pneumococcal pneumoni

Clonal expansion of global pneumococcal sequence cluster 3 within serotype 8 after 13-valent pneumococcal conjugate vaccine introduction, South Africa.

Background. Serotype 8 has emerged as a common non-vaccine serotype causing invasive pneumococcal disease (IPD) in children and adults globally after the introduction of pneumococcal conjugate vaccines (PCV). We aimed to determine serotype 8 incidence and genomic epidemiology in South Africa.Methods. From national, laboratory-based surveillance for IPD in South Africa, we calculated the incidence of serotype 8 and global pneumococcal sequence cluster 3 (GPSC3) before and after PCV introduction. We performed phylodynamic analysis of genomes from clonal complex (CC) 53 and described South Africa

Effect of use of 13-valent pneumococcal conjugate vaccine in children on invasive pneumococcal disease in children and adults in the USA: analysis of multisite, population-based surveillance.

BACKGROUND: In 2000, seven-valent pneumococcal conjugate vaccine (PCV7) was introduced in the USA and resulted in dramatic reductions in invasive pneumococcal disease (IPD) and moderate increases in non-PCV7 type IPD. In 2010, PCV13 replaced PCV7 in the US immunisation schedule. We aimed to assess the effect of use of PCV13 in children on IPD in children and adults in the USA. METHODS: We used laboratory-based and population-based data on incidence of IPD from the Active Bacterial Core surveillance (part of the Centers for Disease Control and Prevention's Emerging Infections Program) in a tim

Initial vaccination platform against S. pneumoniae shapes antibody profiles upon revaccination with pneumococcal conjugate vaccine in older adults.

Currently, two types of Streptococcus pneumoniae vaccines are approved by the Food and Drug Administration (FDA): pneumococcal conjugate vaccine (PCV) and plain polysaccharide vaccine (PPSV). Here, we characterized how antibody profiles against select S. pneumoniae serotypes (Pn1, Pn3, Pn6A, Pn6B, Pn7F, Pn19A, Pn19F, and Pn23F) were shaped after a dose of PCV13 or PPSV23 in aged adults and how these initial responses influenced a PCV13 revaccination. Numerous humoral features were significantly induced by one dose of PCV13 compared to baseline, while one dose of PPSV23 showed an increase only

Pneumococcal vaccination rates in immunocompromised patients in Germany: A retrospective cohort study to assess sequential vaccination rates and changes over time.

BACKGROUND: Pneumococcal vaccination is recommended by the German Standing Committee on Vaccination (STIKO) for infants, elderly 60+ years and patients at risk. In 2016, a sequential pneumococcal vaccination schedule (conjugate vaccine followed by polysaccharide vaccine 6-12 months later) supplemented this recommendation for immunocompromised patients ≥2 years of age. Previous research showed low pneumococcal vaccination rates (pnc-VR) in this vulnerable group. Moreover, no evidence is available on adherence to the newer sequential schedule. This study aimed to analyze the development of pnc-V
