Perforated peptic ulcer in an adolescent girl.

A perforated peptic ulcer in a child is a rare entity. Severe abdominal pain in an ill-appearing child with a rigid abdomen and possibly with signs of shock is the typical presenting feature of this life-threatening complication of peptic ulcer disease. We present a case of a 14.5-year-old adolescent girl who developed abdominal and shoulder pain that resolved after 1 day. She was then completely well for 2 days until the abdominal and shoulder pain recurred. On examination, she appeared well, but in pain. A chest radiograph revealed a large pneumoperitoneum. She underwent emergent laparoscopi…

Haemodynamic conditions enhancing gas embolism after venous injury during laparoscopy: a study in pigs.

This study was designed to determine the conditions that promote carbon dioxide embolism after venous injury during laparoscopy in pigs. Injury to an iliac vein was filmed during laparoscopy in the presence of a pneumoperitoneum created at increasing pressures from 0 to 30 mm Hg in 5-mm Hg increments. At intraperitoneal pressures less than 20 mm Hg, there was a parallel increase in femoral venous pressures, resulting in haemorrhage, with persistent blood flow to the inferior vena cava. At intraperitoneal pressures of 20-30 mm Hg, there was collapse of the femoral vein, occurring earlier in the…

Laparoscopic-assisted colectomy and lymphadenectomy without peritoneal insufflation for sigmoid colon cancer patients.

PURPOSE: A new method for laparoscopic-assisted sigmoid colectomy and lymphadenectomy is presented. The purpose of this method is to avoid complications and restrictions associated with pneumoperitoneum. METHODS: Abdominal wall is lifted up with Kirschner wires, which are placed in the middle and left lower abdomen; then a minilaparotomy, which can be created because air-tightness is not necessary, is extended at the beginning of the operation to later remove the resected specimen. Two laparoscopic instruments are inserted through the right lower abdomen. Through the minilaparotomy, the surge…

Morphological changes on the human liver during minimally invasive surgery: Implications for image-guided interventions and surgical navigation.

BACKGROUND: Minimally invasive liver resection and ablation depend on surgical planning and image guidance. Surgical planning is normally based on preoperative imaging. The position, shape, and volume of the abdominal organs change during laparoscopy, which challenges image registration and reduces surgical precision. This study aims to analyze the morphological changes of the liver and spleen from pre- to intraoperative (with pneumoperitoneum) computed tomography (CT) images. METHODS: We used portal venous phase pre- and intraoperative CT images from 15 patients who underwent laparoscopic li…

A case of transanal barotrauma by high-pressure compressed air leading to transverse colon perforation with extensive colon serosal tear.

INTRODUCTION AND IMPORTANCE: Compressed air is used to apply paint, wash vehicles or machines, and remove water droplets after washing the p … CLINICAL DISCUSSION: The management of colon injury due to compressed air has two aspects: tension pneumoperitoneum and colon injury. The initial management of tension pneumoperitoneum is converted to open pneumoperitoneum and early emergency operation for colon injury is recommended as soon as full-thickness perforation is diagnosed. CONCLUSION: Transanal high- …

Robotic right colectomy in a patient with ventriculoperitoneal shunt. Report of a case.

INTRODUCTION: Ventriculoperitoneal shunt procedure has become the most common neurosurgical method for hydrocephalus because it considerably improves patients prognosis. Pneumoperitoneum has been considered a contraindication to laparoscopic surgery because of risk for increased intracranial pressure during pneumoperitoneum and/or malfunction/infection of the VP shunt itself. Laparoscopic resection of the cecum and of the rectum for cancer has been reported. PRESENTATION OF CASE: A 74-year old man with ventriculoperitoneal shunt for normal pressure hydrocephalus referred to the emergency medi…

Changes in the coelomic microclimate during carbon dioxide laparoscopy: morphological and functional implications.

In this article the adverse effects of laparoscopic CO2 pneumoperitoneum and coelomic climate change, and their potential prevention by warmed, humidified carbon dioxide insufflation are reviewed. The use of pressurized cold, dry carbon dioxide (C02) pneumoperitoneum causes a number of local effects on the peritoneal mesothelium, as well as systemic effects. These can be observed at a macroscopic, microscopic, cellular and metabolic level. Local effects include evaporative cooling, oxidative stress, desiccation of mesothelium, disruption of mesothelial cell junctions and glycocalyx, diminished…

Pneumatic colon injury following high pressure blow gun dust cleaner spray to the perineum.

INTRODUCTION: A pneumatic tool or air tool such as blow gun dust cleaner is a tool driven by compressed air and spraying of the perineum can insufflate the colon due to its high pressure and high flow rate. PRESENTATION OF CASE: We present a case of 4 year old boy who developed sudden onset of tense abdominal distention and developed peritonitis. Patient's family initially denied a history of trauma. Radiologic examination showed pneumoperitoneum and colon dilatation. Exploratory laparotomy revealed a tension pneumoperitoneum, bloody ascitic fluid, multiple site of ecchymosis and serosal tear…

The effects of different intraabdominal pressure protocols in laparoscopic procedures on oxidative stress markers and morphology in rat ovaries.

BACKGROUND: To determine the effects of different intraabdominal pressure (IAP) on the ovaries in a laparoscopic rat model. OBJECTIVES: The … sesment scores were significantly higher in Group Pp15 than in Groups Pp10 and IPp15. CONCLUSIONS: Pneumoperitoneum causes injuries to abdominal organ such as the ovaries. The ischemic preconditioning method is more effective in reducing oxidative stress due to laparoscopic pneumoperitoneum than low-pressure pneumoperitoneum methods.

Gastric perforation in a 5-day-old infant: A case report.

Gastric perforation in a new-born signifies a surgical emergency requiring immediate attention. Therefore, early diagnosis is expected to lead to a better prognosis. Any infant presenting with sudden and severe abdominal distention should be suspected of gastric perforation. Diagnosis is confirmed by pneumoperitoneum in the plain abdominal X-ray, including anteroposterior and lateral views. In this study, we report a case of a 5-day-old neonate, who was diagnosed with gastric perforation and accordingly proceeded with primary surgical repair.

Meconium peritonitis-observations in 115 cases and antenatal diagnosis.

In 20 years the authors have met with 115 cases of meconium peritonitis (MP). The high incidence in China is shown. In all cases there were intraabdominal calcifications. There were no cases of fibrocystic disease. The authors classify 3 types: 1. Neonatal obstructive type 41 cases. 2. Free perforation with: Free pneumoperitoneum 15 cases. Localized pneumoperitoneum 23 cases. 3. No signs or symptoms, with potential intestinal obstruction 23 cases. The series shows a low incidence of atresia. The authors suggest necrotizing enterocolitis as a cause because of the high incidence of stenosis. Thr…
