Immune checkpoint inhibitors, also known as immunotherapy, have revolutionised the treatment of metastatic melanoma, but are frequently associated with immune-related adverse events (irAEs) affecting a variety of organ systems. Here, we present a case of a patient with metastatic melanoma, being treated with combination ipilimumab-nivolumab, who developed a foot drop. MRI demonstrated enhancement of the nerve roots of the cauda equina. The patient had other irAEs, which warranted cessation of immunotherapy and the introduction of corticosteroids, and this also resulted in improvement in the pa

STUDY DESIGN: A prospective, observational cohort study.
OBJECTIVE: The aim of this study was to determine the role of pre and postvoid bladder scan in predicting cauda equina syndrome (CES). The thesis was that bladder scanning [specifically postvoid residual (PVR) volume] would have higher diagnostic accuracy than physical examination alone.
SUMMARY OF BACKGROUND DATA: CES is an ill-defined condition with a spectrum of presenting symptoms. There is neither a combination of clinical symptoms and/or signs that reliably predicts cauda equina compression nor single defining clinical criterion

STUDY DESIGN: Retrospective.
OBJECTIVES: The objective of the study was to describe the type of ejaculation in patients with cauda equina (CE) and conus medullaris (CM) lesions, and to analyse sperm quality.
SETTING: France.
METHODS: One hundred sixty-six patients with CE and CM lesions were included. Diagnosis was based on clinical (no motor responses, sensation or sacral reflexes) and urodynamic assessments (no detrusor activity). Vibromassage (VM) was used to induce ejaculation according to the recommendations for patients with spinal cord injury. If ejaculation did not occur, oral midod

A 77-year-old woman presented with conus medullaris and cauda equina syndrome following a sudden pain in the bilateral lower abdomen and right buttock. Lumbar magnetic resonance imaging (MRI) showed not only a conus medullaris lesion, but also several lesions in the vertebral bodies (L1, L2), right major psoas muscle, right multifidus muscle and bilateral erector spinae muscles. As these areas receive blood supply from each branch of the same segmental artery, we considered all of the lesions as infarctions that were a result of a single parent vessel occlusion. It is known that a vertebral bo

AIMS: Sacral neurophysiologic studies have demonstrated their utility in men with suspected neurogenic sacral dysfunction. However, no similar studies have been performed in women. The present study aimed to test the utility of sacral neurophysiologic assessment in women with chronic cauda equina lesions.
METHODS: Twenty-four women with clinical and radiological signs supportive of chronic cauda equina lesions, and a group of 60 control women without clinical symptoms or signs of sacral neuropathic lesion were included. Clinical examination, including testing of saddle sensation, and neurophy

This report describes a case of cauda equina syndrome possibly caused by arachnoiditis due to levobupivacaine after spinal-epidural anesthesia. A 39-yr-old woman delivered by cesarean section under a combined spinal-epidural anesthesia. After an uneventful procedure and surgery, she complained of weakness in her lower extremities, which increased in a few hours. Neurologic examination revealed severe weakness in both her lower extremities, perianal anesthesia, and absence of muscle stretch reflexes. She was unable to urinate. Magnetic resonance imaging performed immediately revealed entirely n

OBJECTIVE: This investigation attempted to determine the incidence of cauda equine syndrome (CES) within a closed North American population from 2001 to 2010. This study also aimed to define risk factors influential in its development.
METHODS: A search was performed using the Defense Medical Epidemiology Database and the ICD-9 code for CES (344.6). Demographic information including sex, race, military rank, branch of service, and age was obtained for the entire military population, as well as those diagnosed with CES. Incidence rates were calculated for the population as a whole, as well as

While there have been a few reports on cases of intradural spinal arteriovenous fistula located on the filum terminale, no cases of its location in a nerve root of the cauda equina have been reported to date.We describe two such cases and describe the intraoperative findings. A 40-year-old man presented weakness of his left leg. Another 62-year-old man presented paraparesis dominantly in his left leg with urinary hesitation. In both cases, spinal T2-weighted magnetic resonance images showed edema of the spinal cord, indicating a flow void around it. Digital subtraction angiography disclosed an

Pseudomeningocele is an uncommon complication of spinal surgery. The condition is most often asymptomatic and self-limiting. A proportion of patients may present with lower back pain, dural fistulas, radiculopathy or myelopathy. Cauda equina syndrome due to a pseudomeningocele has been reported due to herniation of the Cauda equina roots through the dural defect. We report a case of large pseudomeningocele causing an impending Cauda equina syndrome by acting as an extradural mass lesion.

BACKGROUND: Posterior epidural migration is an interesting but rare path taken by a prolapsed intervertebral disc fragment. There are only seven cases reported of a similar migration of the disc fragment in the lumbar spine.
PURPOSE: To diagnose the unusual presentation and interpretation of the sequestrated disc in the posterior epidural space.
STUDY DESIGN: Case report.
METHODS: Two cases of migration of the sequestrated disc into the posterior epidural space are presented. In one patient there was weakness of the ankle dorsiflexors, foot invertors, and toe extensors of the left foot, wit

AIMS: Data on lower urinary tract (LUT) function obtained in larger, systematically studied populations of patients with cauda equina lesions are limited. We report urinary function in 65 patients with chronic lesions.
METHODS: A urinary function questionnaire was used, and neurological examination, quantitative electromyography (EMG) of the external anal sphincter (EAS) muscles, EMG assessment of the sacral reflex, and conventional urodynamic measurements were performed.
RESULTS: Severe LUT dysfunction was found in 14%/15%, moderate in 27%/46%, and mild in 46%/39% men/women. Urinary symptom

INTRODUCTION: Sacral insufficiency fractures are a well recognised cause for low back, buttock and groin pain in the elderly. However, over a 4 year period, four patients have presented with symptoms of cauda equina syndrome, who were found on investigation to have acute sacral insufficiency fracture without any other aetiological spinal abnormality.
PATIENTS AND METHOD: Four patients who presented to the spinal surgeons of our institution with symptoms of cauda equina syndrome were referred for spinal MR. Sagittal and axial T1 and T2 weighted turbo spin echo sequences of the lower thoracic a

PURPOSE: To evaluate outcome measures and the factors affecting them in patients treated between 1962 and 2000 at Loewenstein Rehabilitation Hospital, Israel.
METHOD: This retrospective cohort study included 262 patients with spinal neurological lesions (spinal cord or cauda equina lesions) following degenerative spinal stenosis. Data were collected retrospectively. Survival was assessed using the Kaplan-Meier method and the relative mortality risk by the Cox model. Neurological recovery was evaluated by the change in Frankel grades, and factors that affect it were assessed by logistic regres

The Patient Injury Claims Department in Sweden grants compensation to patients who have incurred injury in the national health service system. All claims involving spinal and epidural anaesthesia were studied during the period 1997-1999. Serious neurological complications were behind 29 out of 65 claims. These occurred more frequently after epidural anaesthesia (1:4,000) than after spinal anaesthesia (1:13,000) with the exclusion of obstetric epidural anaesthesia (1:40,000). Epidural haematoma occurred in 1:30,000 after epidural anaesthesia, in 1:200,000 after spinal anaesthesia. One epidural

Cerebral potentials were evoked by stimulating sensory fibers of the median, ulnar, radial, musculocutaneous, sural, superficial peroneal, and saphenous nerves. The sensory nerve action potential was simultaneously recorded from these nerves, allowing comparison of peripheral and central sensory conduction times. Somatosensory potentials (SEPs) elicited by sensory nerve stimulation are of particular value in evaluating proximal segments of peripheral as well as central somatosensory pathways.

In early January, 1976, an outbreak of gastroenteritis caused by contamination of the water supply system occurred in Salt, Jordan. This outbreak was followed by the appearance of peripheral neuropathy in 19 cases diagnosed as Guillain-Barré Syndrome. Clinically, gastroenteritis was suggestive of Shigellosis and one of nine patients with Guillain-Barré Syndrome, grew Shigella boydii. Epidemiology of the 19 cases with peripheral neuropathy is detailed. Seventeen patients had definite gastroenteritis symptoms prior to their neuropathy. The median incubation period was seven days.

The neurological and muscular complications seen in coeliac disease in adults are usually attributed to deficiency secondary to malabsorption. Amongst them, however, there exists a very rare cateogory, described by Cooke et al. (1966) taking the form of a chronic myeloneuropathy which cannot be explained in terms of the malabsorption syndrome. Our two cases of gluten intolerance enteropathy, confirmed by biopsy before and after diet, fell into this group of polyneuropathies. The patients, both women, suffered from an essentially sensory ataxic polyneuropathy with accessory motor component with

Variations of the main immunoglobulin fractions in CSF were studied in 79 children with various inflammatory processes of CNS (purulent and viral meningitis, post-infectious meningoencephalitis, polyradiculoneuritis, etc). This study was extended to a group of non-inflammatory neurological diseases. Increased values of CSF immunoglobulins (especially IgG and IgA) were found in the group of purulent meningitis and in the group of post-infectious meningoencephalitis. IgM was detected in very few cases. Generally, a correlation was observed between highest concentrations of CSF immunoglobulins an
