PURPOSE: The apnea-hypopnea index (AHI) has long dominated the diagnosis of obstructive sleep apnea (OSA); however, mounting evidence indicates that AHI alone cannot capture the disorder's heterogeneity. This review synthesizes current evidence on polysomnography-based phenotyping and endotyping into a practical framework for precision OSA management. METHODS: PubMed/MEDLINE and Scopus were searched from inception through December 2025 using terms related to OSA, phenotyping, endotyping, and polysomnographic parameters. Randomized trials, cohort studies, cluster analyses, and machine learning …

Sleep-disordered breathing (SDB), particularly obstructive sleep apnea (OSA), is a highly prevalent group of disorders associated with disrupted sleep, intermittent hypoxemia, excessive daytime sleepiness, neurocognitive impairment, and cardiometabolic morbidity. Although laboratory polysomnography remains central to diagnosis, conventional sleep testing can be costly, labor-intensive, and difficult to scale. Recent advances in data mining, machine learning, deep learning, wearable sensing, mobile health, multimodal analytics, and large language models are creating new opportunities for more a…

Background/Objectives: Polysomnography and cephalometry have been used for studying obstructive sleep apnea (OSA) etiology. The association between craniofacial skeleton and OSA severity remains controversial. To study OSA's etiology, cephalometry, fiberoptic pharyngoscopy, polysomnography, and sleep endoscopy have been used; however, airway obstructions cannot be located. Recent research suggested ultrasonography for OSA screening and upper airway obstruction localization. Thus, this study aims to investigate the relationship between specific craniofacial cephalometric and ultrasonic airway p…

KEY CLINICAL MESSAGE: This study suggests that severe obstructive sleep apnea can present as sleep-related epileptic or non-epileptic seizures. A detailed history and physical examination, along with polysomnography and video electroencephalography findings can lead to the correct diagnosis.
ABSTRACT: Obstructive sleep apnea (OSA) is defined by recurrent episodes of the upper airway complete or partial collapse while sleeping. The obstructive episodes result in gradual suffocation that increases breathing attempts till the person is awakened. The main manifestations are excessive daytime slee…

The aims of the study were to evaluate the prevalence of sleep-disordered breathing (SDB) by using polysomnography (PSG) in children with MPS IVA and MPS VI who underwent enzyme replacement therapy (ERT) and to analyze the effect on SDB of having upper airway surgery, pulmonary functions, and exercise capacity. A retrospective cross-sectional study was conducted on patients with MPS IVA (n:17) and MPS VI (n:11) aged under 19 years who underwent polysomnography. Descriptive and nonparametric analyses were performed for demographic, PSG, pulmonary function and exercise capacity variables. The fr…

Because of unspecific diagnostic criteria, there is much controversy around narcolepsy type 2, its existence and its frequency. With this retrospective and purely descriptive study, we aimed to compare the frequency of narcolepsy type 2 compared to the well-described narcolepsy type 1, in a large (n = 3,782) retrospective sample from a single tertiary sleep centre. After 2 weeks washout of sleep-wake active medication, all patients with excessive daytime sleepiness (n = 1,392) underwent 2 weeks actigraphy, polysomnography and multiple sleep latency test, and all diagnoses were made along curre…

There is no satisfactory treatment for obstructive sleep apnoea (OSA). Supplemental low-flow oxygen therapy (LFO2) has been shown to reduce hypoxaemia and is well tolerated by patients with OSA. However, oxygen therapy may be beneficial only to certain subsets of patients with OSA. In this study, we evaluated a 10-min awake ventilatory chemoreflex test in predicting individual OSA response to 2 months of LFO2 therapy. At baseline, patients with OSA underwent ventilatory chemoreflex testing in the afternoon, prior to the overnight polysomnography. Subjects were reassessed with polysomnography af…

Snoring sounds generated by different vibrators of the upper airway may be useful indicators of obstruction sites in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). This study aimed to investigate associations between snoring sounds, obstruction sites, and surgical responses (≥50% reduction in the apnea-hypopnea index [AHI] and <10 events/hour) in patients with OSAHS. This prospective cohort study recruited 36 OSAHS patients for 6-hour snoring sound recordings during in-lab full-night polysomnography, drug-induced sleep endoscopy (DISE), and relocation pharyngoplasty. All pati…

Distinguishing obstructive sleep apnea from central apnea depends upon accurate measure of chest and abdominal movement. American Academy of Sleep Medicine (AASM) polysomnography guidelines recommend the use of respiratory inductive plethysmography (RIP) belts but not piezoelectrode (PE) belts for measuring chest and abdominal movements. To compare these two sensors, we measured the signal amplitude for 10 RIP belts and 10 PE belts stretched by mechanical distraction across six distances (2.5 to 15.0 centimeters) and replicated 10 times for each belt. Amplitudes were measured using the Stellat…

The European Sleep Apnoea Database (ESADA) reflects a network of 22 sleep disorder centres in Europe enabled by a COST action B26 programme. This ongoing project aims to describe differences in standard clinical care of patients with obstructive sleep apnoea (OSA) and to establish a resource for genetic research in this disorder. Patients with suspected OSA are consecutively included and followed up according to local clinical standards. Anthropometrics, medical history, medication, daytime symptoms and sleep data (polysomnography or cardiorespiratory polygraphy) are recorded in a structured w…

Although sleep apnoea is very common in patients with end-stage renal disease, the physiological mechanisms for this association have not yet been determined. The current authors hypothesised that altered respiratory chemo-responsiveness may play an important role. In total, 58 patients receiving treatment with chronic dialysis were recruited for overnight polysomnography. A modified Read rebreathing technique, which is used to assess basal ventilation, ventilatory sensitivity and threshold, was completed before and after overnight polysomnography. Patients were divided into apnoeic (n = 38; a…

Long acting beta 2-agonists belong to the basic therapy of COPD. Especially patients with nocturnal respiratory problems may benefit from this therapy. Long term recording of lung sounds is a new method for quantitative measurements of bronchial obstructions. In combination with polysomnography an evaluation of cardiorespiratory parameters and sleep structure is possible. A total of 10 patients (8 male and 2 female) with moderate COPD (FEV1 58 +/- 11 %) and signs of bronchial obstruction were investigated. The combination of acoustic long term recording and polysomnography was done for 2 or 3 …

Although polysomnography is routinely performed to evaluate children and adolescents with sleep-disordered breathing, normal polysomnographic values for the pediatric age group have not yet been established. We therefore performed overnight polysomnography in 50 normal children and adolescents (mean age 9.7 +/- 4.6 SD yr, range 1.1 to 17.4 yr). Of the children 56% were male. Chest wall motion, ECG, oronasal airflow, end-tidal PCO2 (PETCO2), arterial oxygen saturation (SaO2), and electrooculogram were monitored. Children had 0.1 +/- 0.5 (range 0 to 3.1) obstructive apneas per hour of total slee…

Healthy children, aged 3, 9, and 13 years referred with sleep disordered breathing had marked central apnoea and bradypnoea on polysomnography, necessitating the use of non-invasive bilevel ventilation in two cases. Each had normal neurological examinations and an Arnold Chiari type 1 malformation was confirmed on magnetic resonance imaging. All underwent urgent posterior fossa decompression which normalised their sleep breathing.
