In porphyria cutanea tarda (PCT), the submicroscopic event causing the formation of spontaneous blisters is the appearance of membrane-limited vacuoles in the superficial dermis. These vacuoles are apparently formed through cytolysis of dermal cells and swelling of the pseudopodia of the basal cells protruding into the dermis, probably as a consequence of photochemically-induced lysosomal damage. The number and size of these vacuoles progressively increase near the cleavage area of the blisters, where their limiting membranes merge and break, giving rise to the cavity of the blister, which is

Erythropoietic porphyria also known as Gunther's disease is the result of a deficiency of the enzyme uroporphyrinogen III cosynthetase. This abnormality deviates the biosynthesis of the porphyrins towards the formation of isomers belonging to the series I. These molecules are not utilized for the synthesis of the hem group and therefore accumulate in the tissues and body fluids, giving rise to the clinical and chemical findings of the disease. The present work constitutes the first report of this unusual genodermatosis in costa Rica. The propositus, a 46 years old woman, presented since childh

Three patients with acute intermittent porphyria were noted to have retinal branch vessel occlusion. Branch "vein" occlusion, segmental optic atrophy, and soft exudate were the most common ocular manifestation. Two patients had labile elevated hypertension. When patients present with retinal branch vessel occlusion and a constellation of bizarre symptoms that might include hypertension, abdominal pain, acute psychotic behavior and/or cutaneous photosensitivity, the diagnosis of acute intermittent porphyria should be considered.

In the present study the participation of lysosomal enzymes in the primary photosensitizing process in the skin of porphyric patients were investigated. Cutaneous photosensitivity reactions were produced in patients with porphyria cutanea tarda by irradiation with near UV light and also to a lesser degree with visible light. The reactions occurred almost exclusively in patients with urinary uroporphyrin excretion exceeding 900 nmol/24 h and preceeded any noticeable increase of lysosomal enzyme activity by several hours. These results indicate that lysosomal enzymes are not primarily involved i

Porphyrin and iron distributions in the liver biopsies of patients with Porphyria cutanea tarda (PCT) were investigated by comparison of composite photographs from fluorescence microscope and after iron stain. Three distinct areas are visible: 1. red fluorescent areas with porphyrins; 2. blue areas corresponding to iron; 3. areas with neither iron nor porphyrin. The areas with iron or porphyrin do not overlap, therefore, this experiment indicates lack of direct correlation between iron and prophyrin distribution in livers of PCT patients.

Among 500 patients on maintenance hemodialysis, 6 patients (5 young women and a 49-year-old man) developed bullous dermatosis, 2-54 months after initiating dialysis treatment. The skin lesions occurred mainly in sunlight-exposed areas, and 4 out of the 6 patients showed increased cutaneous fragility in response to trauma. Skin biopsy revealed subepidermal blisters for all of them, and skin immunofluorescence studies were negative for 2 patients. No increase in fecal or red cell coproporhyrin and protoporphyrin levels was found in any of the 6 patients. The syndrome was clinically and histologi

Acute intermittent porphyria is an inborn error of metabolism characterized by the excretion of excess porphyrin precursors (porphobilinogen and usually delta-aminolevulinic acid) in the urine, and by sporadic attacks of neurologic dysfunction. The disease is complex, involving variable patterns of autonomic and peripheral neuropathy as well as the central nervous system manifestations. There may be alterations in carbohydrate, lipid, water, and electrolyte metabolism in addition to clinically inapparent endocrine abnormalities. The fundamental defect is thought to be a 50% decrease of uroporp
