Features of Inorganic Ion Binding by Bovine Pancreatic Trypsin Inhibitor.

Inorganic ions play a significant role in biological systems, including their effects on protein folding, stability, and protein-protein interactions. This contribution presents an in silico study of selective ion binding by the protein bovine pancreatic trypsin inhibitor (BPTI) in NaCl(aq), KCl(aq), MgCl2(aq), and CaCl2(aq). The results were obtained within the framework of the theory of molecular liquids using the 3D-RISM (three-Dimensional Reference Interaction Site Model) approach. As established, the cation affinity to the BPTI surface decreases as Mg2+ > Ca2+ > Na+ > K+, which indicates

[Hypokalemia: Not Just Tubulopathies].

Hypokalemia is among the most common electrolyte abnormalities, often well tolerated, but sometimes responsible for an increase in morbidity and mortality due to cardiovascular causes. The kidneys play a key role in potassium homeostasis, making the nephrologist the professional directly involved in the diagnosis and treatment of this condition. We present the clinical case of a 24-year-old man who came in with multiple episodes of emesis and hyperpyrexia. The patient rapidly developed severe hypokalemia with ascending flaccid paralysis. Despite early treatment with potassium chloride, intensi…

Resolution of Left Bundle Branch Block After Calcium Administration in the Prehospital Setting.

Hyperkalemia is a medical emergency with potentially severe consequences that can be avoided by early recognition and effective treatment. Electrocardiogram (ECG) changes can help elucidate hyperkalemia prior to obtaining lab results and assist in early decisions on treatment, especially in the prehospital setting. ECG changes commonly associated with hyperkalemia are peaked T-waves, PR prolongation, P-wave flattening, QRS widening, or a sine-wave pattern at severely elevated potassium levels. Bundle branch blocks (BBBs) are associated with hyperkalemia but are less common and less well known

Gitelman Syndrome: A Rare Case of Hypokalaemia and a Novel Mutation.

UNLABELLED: Gitelman syndrome (GS) is a hereditary renal tubulopathy caused by mutations in the SLC12A3 gene which encodes the thiazide-sensitive apical sodium-chloride cotransporter. GS is characterized by hypokalaemia, hypomagnesaemia and metabolic alkalosis. Treatment is based on potassium and magnesium replacement ad eternum. We present the case of a young man with palpitations and persistent hypokalaemia, who was diagnosed with GS. Genetic testing revealed 2 mutations in the gene SLC12A3 of combined heterozygosity, both considered pathological. Interestingly, 1 of these mutations was not

Bartter's and Gitelman's syndromes: their relationship to the actions of loop and thiazide diuretics.

The wider recognition of inherited Bartter's and Bartter's-like syndromes, especially Gitelman's, has come largely as a result of the advances in, and application of, molecular genetics. By exploiting pre-existing renal cell models of ion transport, specifically for sodium and potassium, the known mechanisms and sites of action of loop and thiazide diuretics and the similarity of their chronic effects to these syndromes, it was possible for geneticists to take a candidate gene approach. This was initially successful but, when not all cases could be explained on this basis, it forced a more det

Effect of pre-treatment with lysine acetyl salicylate on suxamethonium-induced myalgia.

The hypothesis that prostaglandin inhibitors might reduce the incidence and severity of suxamethonium-induced myalgia was investigated using lysine acetyl salicylate (LAS) 13 mg kg-1 i.v. 3 min before the administration of suxamethonium in 20 patients. A comparison was made with atracurium 0.09 mg kg-1 (and placebo) in a double-blind prospective randomized trial. LAS and atracurium were effective in reducing the incidence and severity of postsuxamethonium myalgia and the increases in serum potassium concentration. There were no appreciable changes in serum calcium, sodium, chloride, phosphate,

Changes in serum electrolyte levels during marathon running.

Serum electrolytes were measured in 18 well-trained, experienced long-distance runners before and after a standard marathon run (42 km), during which they ingested no electrolytes. Their sweat losses and water deficits after completion of the marathon were also measured. In 12 of the subjects, the percentage change in plasma volume and in total circulating plasma electrolytes was determined. There was a highly significant fall in serum magnesium concentration, with an increase in both potassium and sodium levels. Changes in total circulating plasma sodium and chloride were closely correlated w

The novel salt-containing high internal phase emulsions stabilized by complex coacervation of ovalbumin and chitosan: Application for enhancing saltiness and flavor of low-salt meat sausages.

The objective of this study was to examine the impact of varying concentrations of potassium chloride (KCl) on the complex coacervation between ovalbumin (OVA) and chitosan (CS), and the properties of high internal phase emulsions (HIPEs) that were stabilized by the complex coacervate. Additionally, HIPEs were applied to sausages to achieve reductions in salt and fat content without compromising quality. By investigating the aggregation behavior of OVA-CS and the structural and intrinsic properties of HIPEs stabilized by them, including the simulation of HIPEs mastication kinetics, it was reve…

Chloride-Dependent Cation Transport via SLC12 Carriers at Atomic Resolution.

The SLC12 family of genes encodes electroneutral Cl--dependent cation transporters (i.e., Na-Cl, K-Cl, Na-K-2Cl cotransporters), which play significant roles in maintaining cell and body homeostasis. Recent resolution of their structures at the atomic level provides a new understanding how these transporters operate in health and disease and how they are targeted for therapeutic intervention. Overall, the SLC12 transporter cryo-EM structures confirm some key features established by traditional biochemical and molecular methods, such as the presence of 12 transmembrane domains and the formation

[Congenital chloride diarrhea mimicking meconium ileus in newborn].

Congenital chloride diarrhoea is a rare autosomal recessive disease and the diagnosis is frequently delayed. The disease is most common in Saudi Arabia and Kuwait 1:3200-13 000 births, Finland - 1:30 000-40 000, and in Poland - 1:200 000. Congenital chloride diarrhoea begins in fetal life. The main clinical sign is watery diarrhea that in utero leads to dilated bowel loops, polyhydramnios and often premature birth. Newborns have distended abdomens, absence of meconium, dilated bowel loops in ultrasonography and watery diarrhea which can sometimes be mistaken for urine. The absence of meconium

Further studies of effects of sodium aluminosilicate on egg shell quality.

Five experiments were conducted using 36 dietary treatments to compare chloride salts and HCl as chemical sources of Cl for the adjustment of dietary Cl when using sodium aluminosilicate (SAS), to compare SAS to natural zeolites (clinoptilolite and mordenite), and to determine the appropriate level of dietary SAS for optimum egg specific gravity. The methods of Na and Cl correction used in the various treatments included altering the levels of NaCl, calcium chloride (CaCl2), potassium chloride (KCl), magnesium chloride (MgCl2), iron chloride (FeCl3), or hydrochloric acid (HCl). Experimental di…

Effects of potassium supplements on glucose metabolism in African Americans with prediabetes: a pilot trial.

Background: Low potassium has been identified both as a risk factor for type 2 diabetes and as a mediator of the racial disparity in diabetes risk. Low potassium could be a potentially modifiable risk factor, particularly for African Americans. Objective: We sought to determine the effects of potassium chloride (KCl) supplements, at a commonly prescribed dose, on measures of potassium and glucose metabolism.Design: Among African-American adults with prediabetes, we conducted a double-blinded pilot randomized controlled trial that compared the effects of 40 mEq K/d as KCl supplements with a matc…
