High Rates of Diabetic Ketoacidosis in Patients with New and Known Type 1 Diabetes over a Six-Year Period.

Aims To analyse all paediatric patients who presented with diabetic ketoacidosis (DKA) from 2012 to 2017. Methods A retrospective observatio … cases (25%) presented in severe DKA and 37 cases (71%) occurred in adolescents aged over 12 years. Precipitating factors included chronic suboptimal control and psychosocial factors (28/52), acute illness (16/52), and pump technical failure (5/52). There were two cases treated for suspected cerebral oedema and one case each of subarachnoid haemorrhage and cardiac arrh …

[Sudden infant death--an emotional trigger mechanism].

Southall and coworkers have demonstrated in a recent study that attacks of lifelessness with sudden and severe hypoxemia and cyanosis are caused by a combination of respiratory arrest in expiration, and a right-to-left shunting of the blood through the lungs due to increased pulmonary vascular resistance. This mechanism is thought to be the cause of death. They have also defined the precipitating factors which are sudden, naturally occurring stimuli causing fear, anger or pain. They underline the importance of elements of surprise and unexpectedness. These observations lend strong support to t…

Transient global amnesia and the amnestic syndrome.

Transient global amnesia is an unusual form of the amnestic syndrome in that it is completely reversible in most patients. The etiology of transient global amnesia is poorly understood, although involvement of the medial temporal lobe is most likely. The literature suggests that once an underlying structural lesion has been ruled out, cases can be divided into three groups: patients with a past history of migraine headaches, those who have no obvious underlying precipitating factors, and those with associated neurologic deficits during a spell and risk factors for cerebrovascular disease. Pati…

Selected developments in the understanding of diabetic ketoacidosis.

Advances in the understanding of diabetic ketoacidosis have contributed to the recent decrease in the morbidity and mortality associated with this condition. The role of counterregulatory hormones in its pathogenesis is considerable, but insulin deficiency is necessary for diabetic ketoacidosis to develop. Therapy begins with identification and treatment of the factors precipitating ketosis. Isotonic saline is the fluid of choice for initial intravenous therapy; subsequently 0.45% saline is appropriate. Sodium bicarbonate is necessary only if the arterial pH is less than 7.1, and phosphate sho

Evidence for Stress-induced Bleeding in a Patient with von Willebrand Factor Deficiency.

Literature reveals that psychological stress is related to hemostatic mechanisms and that excess stress can lead to prothrombotic events. Patients with chronic bleeding disorders report increased levels of subjective distress. The psychobiological link between stress and bleeding tendencies is rarely investigated when compared to the wealth of the studies on stress and clotting mechanisms. We present the case of a female with recurrent depressive disorder in whom episodic stress precipitated acute bleeding spells. An extensive hematopathological investigation revealed that she had von Willebra

Do NSAIDs contribute to acute fatty liver of pregnancy?

Acute fatty liver of pregnancy (AFLP) and the childhood encephalopathy known as Reye's syndrome are both characterised by microvesicular steatosis. Mothers with AFLP are frequently heterozygous for a mutation which reduces the activity of the trifunctional protein (TP) of fatty-acid oxidation. Several lines of evidence suggest that blockade of fatty-acid oxidation may also be the underlying cause of Reye's syndrome, and epidemiological studies have identified aspirin taken during a viral illness as a contributing factor to the development of the disease. The hypotheses are presented:* that chi

[Study of lethal arrhythmias using Holter ECG monitoring].

Holter monitorings were performed on 2733 cases (male 1547, female 1186; age 16-92 yrs.) from 1973 to 1986 in our hospital. Among these cases, 18 patients (0.66%) died suddenly (group A), 47 patients (1.72%) died of cardiac failure (group B) and 142 patients (5.27%) survived irrespective of the presence of advanced arrhythmias (group C). To evaluate the precipitating factors for sudden cardiac death (SD), underlying diseases, electrical instability and clinical characteristics were compared between each group. 1. The incidence of coronary artery disease (CAD) was 6 cases (33.3%) in group A, 13…

Massive splenic infarction in children with sickle cell anemia and the role of splenectomy.

BACKGROUND: Massive splenic infarction (MSI) is a very rare condition. Few reports of splenic infarction of various etiologies including hematological and non-hematological causes have been published. On the other hand, MSI in patients with sickle cell anemia (SCA) is extremely rare. This report describes our experience with 15 children with SCA and MSI outlining aspects of presentation, diagnosis and management. PATIENTS AND METHODS: The records of all children with MSI were retrospectively reviewed for age at diagnosis, sex, clinical features, precipitating factors, investigations, manageme…

High-resolution computed tomography in patients with bronchial asthma: correlation with clinical features, pulmonary functions and bronchial hyperresponsiveness.

The high-resolution computed tomography (HRCT) studies for bronchial asthma (BA) have revealed abnormal radiologic findings such as bronchial wall thickening, bronchiectasis, emphysema and mosaic pattern of lung attenuation. But the clinical significance of these findings are not yet clarified. In this study, we quantified the bronchial wall thickness and evaluated HRCT features in 57 BA subjects (338 bronchi) who had precipitating factors of irreversible airway remodeling, 19 COPD subjects (70 bronchi) and 10 healthy subjects (23 bronchi). Then we correlated HRCT findings with the clinical fe…

Glymphatic Dysfunction in Hepatic Encephalopathy: A Novel Pathogenetic Pathway and Potential Target for New Therapeutic Opportunities.

Hepatic encephalopathy (HE) is a common complication of decompensated liver cirrhosis and acute-on-chronic liver failure. It encompasses a range of symptoms, from minimal cognitive impairments to severe personality changes and even coma. Liver dysfunction, coupled with the development of portosystemic shunting (PSS), leads to the accumulation of toxic substances in the brain, adversely affecting its function. Common precipitating factors for HE include variceal bleeding, infections, and electrolyte imbalances. However, other mechanisms likely contribute to the onset of HE, as some patients exp…

Prevalence and risk factors of insomnia among older adults living alone: A cross-sectional study based on Spielman's 3P behavioral model.

BACKGROUND: Insomnia is a prevalent issue among older adults, particularly those living alone. Spielman's 3P behavioral model highlights the role of predisposing, precipitating, and perpetuating factors in the development of insomnia. OBJECTIVES: This cross-sectional study aimed to identify the prevalence of insomnia and its association with anxiety, depression, stress, and loneliness among older adults living alone in Seoul, Korea. METHODS: A total of 153 participants were recruited from four welfare centers in Seoul between 1 July and 31 August 2022. Data were collected using validated ins
