Minding the Gap in Vertebroplasty: Vertebral Body Fracture Clefts and Cement Nonunion.

BACKGROUND: Vertebroplasty and kyphoplasty are leading treatments for patients with vertebral body compression fractures. Although cement augmentation has been shown to help relieve pain and instability from fractures containing a cleft, there is some controversy in the literature regarding the procedure's efficacy in these cases. Additionally, some of the literature blurs the distinction between clefts and cement patterns (including cement nonunion and cement fill pattern). Both clefts and cement patterns have been mentioned in the literature as risks for poorer outcomes following cement augm

Robotic Burch colposuspension: anatomical and technical considerations.

INTRODUCTION AND HYPOTHESIS: Up to 13.6% of women will undergo surgical treatment for stress urinary incontinence during their lifetime. Midurethral slings are the mainstay of stress incontinence treatment; however, diversity of surgical options is needed to serve the large number of patients desiring treatment. The Burch colposuspension remains a viable treatment option for appropriately selected patients. Currently, information on procedural standardization and tools for surgical training on robot-assisted colposuspension is limited. METHODS: We describe a stepwise robotic approach aimed at

Simulation in teaching regional anesthesia: current perspectives.

The emerging subspecialty of regional anesthesiology and acute pain medicine represents an opportunity to evaluate critically the current methods of teaching regional anesthesia techniques and the practice of acute pain medicine. To date, there have been a wide variety of simulation applications in this field, and efficacy has largely been assumed. However, a thorough review of the literature reveals that effective teaching strategies, including simulation, in regional anesthesiology and acute pain medicine are not established completely yet. Future research should be directed toward comparati

Spinal intradural hematoma after intrathecal drug delivery system implantation in a patient with possible heparin-induced thrombocytopenia: a case report.

BACKGROUND: Intrathecal drug delivery systems (IDDSs) can provide analgesia for selected patients with refractory cancer pain. Neuraxial implantation in patients receiving antithrombotic therapy carries a risk of spinal bleeding. Heparin-induced thrombocytopenia (HIT) is mainly prothrombotic and should be diagnosed by combining clinical probability with laboratory testing. CASE PRESENTATION: A 50-year-old woman with metastatic rectal adenocarcinoma, refractory cancer pain, previous lower-extremity venous thromboembolism, and chronic aspirin use underwent image-guided IDDS implantation after p

Clinical Trial: The Optimal Timing of the Endoscope Insertion After Oral Lidocaine Spray. A Randomized Controlled Trial.

BACKGROUND AND AIM: Esophagogastroduodenoscopy (EGD) is a critical diagnostic tool for assessing upper gastrointestinal disorders, yet it can induce significant discomfort due to gag reflexes and pain. For patients with a higher risk for sedation, topical lidocaine spray at the hypopharynx is an alternative method. Yet, no standardized protocol exists regarding the optimal waiting period between administering lidocaine spray and commencing EGD. This study investigates the optimal waiting time between the application of topical lidocaine spray and the initiation of EGD to enhance patient comfor

[Physiopathology and management of pain in multiple myeloma].

Pain is a troublesome problem in multiple myeloma (MM) patients which may be affected by several pain syndromes sustained by different underlying causes and mechanisms- In this setting may be due to the disease itself, to its complications, to diagnostic procedures and therapeutic interventions. Most pain syndromes origin from the skeleton and are related to several aspects of the myeloma bone disease (osteolysis and osteopenia). Other remarkable pains frequently observed in MM are due to disease-related and iatrogenic neuropathies. For most MM patients the World Health Organization's three-st

Rupture of an unsuspected ectopic pregnancy following a hysterosalpingography-A case report.

INTRODUCTION: Hysterosalpingography (HSG) is routinely performed in the mid-follicular phase of a woman's menstrual cycle for cavity and tubal patency assessment as a part of the infertility screening. A pre-procedural pregnancy test is not routinely required unless the patient reports abnormal menstrual bleeding or irregular menstrual pattern. PRESENTATION OF CASE: We present the case of a 29 year-old sub fertile woman who had a HSG performed on day 12 of her menstrual cycle. She developed abdominal pain shortly after and was diagnosed with a ruptured ectopic pregnancy, resulting in a right

Gender-based analysis of perioperative outcomes associated with lower extremity bypass.

We analyzed gender-based differences in preoperative factors, procedural characteristics, and 30-day outcomes after lower extremity bypass (LEB). LEB procedures were identified from the American College of Surgeons National Surgical Quality Improvement Program Participant User File. Groupwise comparisons of preoperative and procedural variables were made using chi square, t tests, and nonparametric methods; gender influences on mortality, systemic, and surgical site complications were evaluated using logistic regression. Women (4,107 of 11,011 [37.3%]) were older and had greater prevalence of

The influence of pain memories on children's and adolescents' post-surgical pain experience: A longitudinal dyadic analysis.

OBJECTIVE: Although children's pain memories have been shown to be a powerful predictor of subsequent pain experiences in acute procedural and experimental pain settings, little is known about the influence of children's and parents' pain memories on children's future pain experiences in other painful contexts. This study used a dyadic approach to examine the roles of children's and parents' memories of pain on their subsequent reporting of postsurgical pain several months after the child underwent a major surgical procedure. METHOD: The sample included 66 parent-child dyads (Mage youth = 14.
