Long-Term Results of Patch Repair in Destructive Valve Endocarditis.

OBJECTIVES: Treatment of destructive endocarditis with abscess formation is a surgical challenge and associated with significant morbidity and mortality. A root replacement is often performed in case of an annular abscess. This retrospective study was designed to assess the long-term outcome of extensive debridement and patch reconstruction as an alternative approach. METHODS: Between November 2007 and November 2016, a selected group of 79 patients (29.6% of all surgical endocarditis cases) with native valve endocarditis (NVE, 53.2%) or prosthetic valve endocarditis (PVE, 46.8%) valve endocar

[18F-fluoro-deoxy-glucose positron emission tomography/computer tomography (18F-FDG-PET/CT) imaging in cardiology].

Over the past decades, 18F-FDG-PET/CT imaging has been recognized as an indispensable tool in the diagnosis, staging and treatment monitoring of cancer. This modern imaging technique combining functional and morphologic information has approved indications not only in oncology but in clinical cardiology as well. In the current review we discuss the specific requirements of patient preparation and image acquisition protocol for cardiac 18F-FDG-PET/CT. We review the literature in some cases highlighted by our own examinations of well-known "gold standard" viability and onco-cardiology examinatio

[Two-Stage Revision for Periprosthetic Infection of the Total Hip Arthroplasty: a Comparison of Two Methods].

PURPOSE OF THE STUDY The purpose of the study is a retrospective comparison of results of the two-stage revision total hip arthroplasty using a non-articulating and an articulating spacer to treat periprosthetic joint infection (PJI). Two basic hypotheses are evaluated: (1) the clinical outcomes of the patients treated with "hand made" articulating cement spacer are better than in non-articulating patient's group in two-stage revision for PJI of the total hip arthroplasty and (2) PJI recurrence is higher in the group of patients treated with an articulating spacer group. Comparison of Genus Specific PCR and Culture with or without Sonication for Microbiological Diagnosis of Vascular Graft Infection.

OBJECTIVES: Vascular graft infections (VGIs) are severe and require prolonged adequate antimicrobial therapy. However, up to 45% of conventional cultures are negative. Sonication and genus specific PCRs for microbiological diagnosis of VGI was evaluated. METHODS: Samples were prospectively obtained from explanted vascular grafts in Bordeaux University Hospital. Conventional bacterial cultures with and without prior sonication of samples were performed. A genus specific PCR assay panel, targeting the most frequent bacteria involved in VGI (Staphylococcus, Streptococcus, Enterococcus, and Enter

Severe prosthetic joint infection in an immunocompetent male patient due to a therapy refractory Pseudallescheria apiosperma.

Prosthetic joint infections (PJI) are rarely due to fungal agents and if so they are mainly caused by Candida strains. This case represents a PJI caused by a multi-drug resistant Pseudallescheria apiosperma, with poor in vivo response to itraconazole and voriconazole. This case differs also by the way of infection, since the joint infection did not follow a penetrating trauma. In the majority of cases, Scedosporium extremity infections remain local in immunocompetent individuals. We report a persistent joint infection with multiple therapeutic failures, and subsequent amputation of the left le

[Lower extremity deep vein as arterial replacement in the treatment of prosthetic graft infection].

In June 2007, an autologous lower extremity deep vein was used for arterial reconstruction for the first time in the Czech Republic. For this procedure, the superficial femoral vein was harvested in its full length, including part of the popliteal vein. Due to previous explantation of an infected prosthesis, the patient had critical ischemia and progressive gangrene of the lower extremity. His condition required an iliac-femoral artery reconstruction. Because of the high risk of recurrent infection, none of the conventional approaches involving a vascular prosthesis could be used. The implanta

A systematic review and meta-analysis of treatments for aortic graft infection.

OBJECTIVE: We compared pooled estimates of event rates for amputations, conduit failures, reinfections, early mortalities, and late mortalities in patients with aortic graft infection who were treated by extra-anatomic bypass, rifampicin-bonded prostheses, cryopreserved allografts, or autogenous veins. METHODS: A systematic review was conducted of English language reports in MEDLINE back to 1985 and a meta-analysis was performed on the results. Studies were selected on the basis of medical subject headings aortic, graft, and infection, and also by a standardized and independent quality rating

Conquest of a worldwide human disease: particle-induced periprosthetic osteolysis.

It is rare in the long history of human disease to encounter a completely unique disease. In the 21st century, when it does occur, the disease commonly is man-made, as is true for periprosthetic osteolysis. However, it also is rare to unravel the mysteries of a biologic process as complex as periprosthetic osteolysis in slightly longer than one generation and even less common to create the means for the worldwide prevention of an entire disease. Such is the story of periprosthetic osteolysis. The adventure from the iatrogenic creation of the disease to the identification of its pathology, to t

Alternative approach for management of infected aortic grafts.

PURPOSE: Prosthetic infection after aortic reconstructive surgery historically has been treated with extraanatomical bypass, graft excision, and aortic stump closure, but at the cost of substantial mortality and amputation rates. Alternatives to this strategy include in situ prosthetic replacement in the infected area, as well as autogenous reconstructions. Inherent to all of these procedures, however, is either the creation of an aortic stump, which carries a significant risk of subsequent blowout, or the placement of a bypass conduit in the infected field, thereby maintaining the potential f

Management and outcome of infrapopliteal arterial graft infections with distal graft involvement.

PURPOSE: The purpose of this study was to determine the outcome of patients with infrapopliteal artery graft infections (InfraPopGIs) who presented with graft infection distal to the popliteal artery. PATIENTS AND METHODS: Between July 1, 1979 and June 30, 1994, 27 patients presented with infrapopliteal artery graft infections (18 polytetrafluoroethylene [PTFE], 9 autologous vein). The infection involved the anastomosis in 22 cases (8 anterior tibial, 8 posterior tibial, 4 peroneal, 2 dorsalis pedis arteries) and was localized to the body of the graft in 5 cases (4 calf, 1 ankle). All bypasse

The case against the conservative nonresectional management of infected prosthetic grafts.

The perception that there is an expanding role for conservative management of patients with aortic graft infection is unfounded. There is,in fact, a striking paucity of convincing data indicating that outcome following nonresectional therapy for aortic prosthetic graft infection is equivalent to modern-day results utilizing extra-anatomic bypass and graft excision. Drainage with localized antibiotic irrigation and biologic coverage may be attempted in unusual circumstances such as the unfortunate patient with an infected thoracoabdominal aortic graft, in whom graft excision is not feasible. Infective endocarditis and occult splenic abscess caused by Brucella melitensis infection: a case report and review of the literature.

This report describes a patient with prosthetic aortic valve endocarditis caused by Brucella melitensis infection with an initial relapse after apparent recovery due to associated occult splenic abscess. In spite of a prolonged period of recovery (6 months) achieved with splenectomy and antibrucella antimicrobial therapy, a second relapse occurred and cure was achieved after replacement of the infected prosthetic valve and prolonged courses of antibrucella antimicrobial therapy. There is controversy on the optimal therapy of such cases, but this report suggests that successful management requi

[Axillo-femoral bypass in the treatment of aorta prosthesis infection. A special case].

The authors report a case recently brought to their observation: T.A., a 77 year old patient, operated on in July 1990, for aortic aneurysm with axillo femoral by-pass. The patient was reoperated on the 5th day for ischaemic necrosis of the descending colon. In 1992 he was operated in another hospital for right iliac branch removal of the by-pass and performing of extra-anatomic bifemoral by-pass for right iliac branch infection of anatomic prosthesis. He was brought to our observation in March 1993, after the patient was refused in other Centres because of purulent abscess located near the ao

Heart transplantation for Q fever endocarditis.

A 30-year-old man underwent aortic and mitral valve replacement for the treatment of Q fever endocarditis. Postoperatively, paravalvular leak of the mitral prosthesis, progressive deterioration in cardiac function, and intractable heart failure developed. As a result, the patient underwent orthotopic heart transplantation 15 months after his initial valve operation. The patient is clinically well 14 months after transplantation.

Total ureteral replacement by subcutaneous pyelovesical bypass in ureteral necrosis after renal transplantation.

OBJECTIVES: Ureteral necrosis is a rare complication of renal transplantation, and is seldom cured by endoscopic management alone. To avoid the potential hazard to the graft created by an open ureteral reconstruction in cases of renal transplant ureteral necrosis, we have appiled a new minimally invasive technique of total ureteral replacement, initially described for the palliative treatment of ureteral obstructions. The subcutaneous bypass technique is based on the use of a silicone-PTFE-bonded tube tunnelled underneath the skin. METHODS: Total ureteral replacement by subcutaneous pyelovesi

Current status of ureteral stent technologies: comfort and antimicrobial resistance.

The placement of a ureteral stent is one of the most commonly performed urologic procedures. Indwelling ureteral stents are often accompanied by significant patient morbidity, including lower urinary tract symptoms, flank pain, and urinary tract infections. This article reviews the current state of ureteral stent technology developed to address the problem of stent discomfort and infection.

A practical overview of considerations for penile prosthesis placement.

Penile prostheses have remained the gold-standard therapy for medically refractory erectile dysfunction (ED) since their popularization. Advances in device design and surgical techniques have yielded improved rates of infection, satisfaction, and mechanical survival of devices. Operative techniques in penile prosthesis surgery include the use of adjunctive procedures (such as ventral phalloplasty and release of the suspensory ligament), management of penile fibrosis, and manoeuvres to correct Peyronie's-disease-related curvature. Complications include urethral and corporal perforation, crossov

Periprosthetic joint infection following Staphylococcus aureus bacteremia.

OBJECTIVES: The incidence of haematogenous periprosthetic joint infections (PJI) among patients with remote infections has been reported to be less than 1%. This incidence may be much higher in cases after documented Staphylococcus aureus bacteremia (SAB). We evaluated the incidence of haematogenous PJI following SAB in patients with previously uninfected arthroplasties. METHODS: A retrospective analysis of our cohort including patients with SAB and prosthetic joints at the Basel University Medical Clinic Liestal from 1998 to 2008. RESULTS: We identified 31 patients with 45 uninfected prosth
