Speed of onset of oesophageal acid reduction with different proton-pump inhibitors in patients with reflux oesophagitis.

OBJECTIVE: Proton-pump inhibitors are the most effective drug treatment for gastro-oesophageal reflux disease. With the increasing trend toward 'on demand' therapy, it is important to determine how quickly oesophageal acid reflux is reduced, and whether this differs between the available compounds. DESIGN: A 2 x 2 double-blind crossover study. METHOD: Eight patients with Savary-Miller grade II oesophagitis underwent 24 h pre-treatment oesophageal pH monitoring. Each patient was randomly allocated to receive daily omeprazole 20 mg and lansoprazole 30 mg for 2 days, in two separate double-blin

The membrane potential of the cellular slime mold Dictyostelium discoideum is mainly generated by an electrogenic proton pump.

Trans membrane potential or ionic current changes may play a role in signal transduction and differentiation in the cellular slime mold dictyostelium discoideum. Therefore, the contribution of electrogenic ion pumps to the membrane potential of D. discoideum cells was investigated. the (negative) peak-value of the rapid potential transient, seen upon microelectrode impalement, was used to detect membrane potential changes upon changes in the external pH in the range of 5.5 to 8.0. The membrane potential was close to the Nernstian potential for protons over the pH range 5.5 to 7.5. The acid-ind

Effects of a proton pump inhibitor, omeprazole, on gastric secretion and gastric and duodenal ulcers or erosions in rats.

The effects of omeprazole, a proton pump inhibitor, on gastric secretion and gastric or duodenal ulcers or erosions in rats were studied. Omeprazole, given intraduodenally, dose-dependently inhibited the gastric secretion (volume, acid and pepsin output) of pylorus-ligated rats. The antisecretory activity of omeprazole at 100 mg/kg persisted for 14 hr after treatment. Acutely induced gastric ulcers or erosions such as Shay ulcers, water-immersion stress-, indomethacin-, aspirin-, or prednisolone-induced erosions were all markedly inhibited by oral or intraduodenal administration of 10-100 mg/k

An App-Based Proton Pump Inhibitor Discontinuation Tool: A Multicentre, Open-Label, Randomised Superiority Trial.

BACKGROUND: Proton pump inhibitors (PPIs) are frequently used without long-term indication. Discontinuation remains challenging because of rebound symptoms. Whether digital guidance tools improve discontinuation is unknown. We evaluated whether an app improves PPI discontinuation. METHODS: In this open-label adaptive randomised controlled superiority trial, adults using daily PPI for ≥ 4 weeks without chronic PPI indication, according to the Dutch College of General Practitioners guideline, were recruited from outpatient clinics of Internal Medicine, Gastroenterology, Rheumatology and Nephrol

Proton pump inhibitor use after kidney transplantation and graft outcomes: A matched national cohort study.

This study evaluates the impact of proton pump inhibitor (PPI) use on graft-related outcomes following renal transplant. Proton pump inhibitors (PPIs) are frequently continued after kidney transplantation, often without a clear ongoing indication. Although their use has been associated with adverse renal outcomes in non-transplant populations, their clinical relevance in transplant recipients remains uncertain. We performed a retrospective cohort study using the TriNetX database of US health networks. We included 23,306 patients from 2000-2021 enrolled 4-6 weeks following their transplant. Pat

Effect of Butyrate-Producing Enterobacteria and Proton Pump Inhibitors on Advanced Hepatocellular Carcinoma Treatment With Durvalumab and Tremelimumab.

AIM: The gut microbiome modulates immune responses, and butyrate-producing bacteria have been linked to improved immune checkpoint inhibitor (ICI) efficacy. Conversely, proton pump inhibitors (PPIs) may negatively impact ICI outcomes by altering gut microbiota. This study aims to elucidate their effects in hepatocellular carcinoma (HCC). METHODS: This retrospective multicenter cohort study included 208 HCC patients treated with durvalumab plus tremelimumab at 25 hospitals in Japan. Patients were classified into a butyric acid group (n = 27), who ingested drugs containing butyrate-producing en

Cobblestone-like Gastric Mucosal Changes on Endoscopy in Dogs with a History of Prolonged Proton Pump Inhibitor Therapy.

This study describes the clinicopathological features of seven canine cases showing a diffuse cobblestone-like gastric mucosal pattern on endoscopy. Cases were retrospectively retrieved from endoscopic databases (2017-2025). Clinical data, treatment history, endoscopic findings, and histology were reviewed. Endoscopically, all dogs exhibited thickened, irregular, and poorly distensible gastric folds. Histopathologic examination showed mild-to-moderate foveolar hyperplasia, variable cystic dilation of the fundic glands, mild chronic lymphoplasmacytic inflammation, and interstitial fibrosis. Par

CYP2C19 genotype-guided dosing of proton pump inhibitors: progress in clinical trials and real-world use.

INTRODUCTION: Proton pump inhibitors (PPIs) are widely used to treat acid-related disorders; however, treatment response varies significantly due to Cytochrome P450 2C19 (CYP2C19) genetic polymorphisms that alter individual drug metabolism. Such variation can lead to insufficient acid suppression, resulting in treatment failure or adverse events. Genotype-guided PPI therapy represents an important step toward personalized gastroenterology by optimizing drug efficacy and safety. AREAS COVERED: This review summarizes evidence from clinical trials and meta-analyses examining CYP2C19-mediated dif

Proton pump inhibitor induced hypocalcemia presenting with carpopedal spasm- a rare case report from Nepal.

BACKGROUND: A frequent electrolyte imbalance, hypocalcaemia can cause anything from asymptomatic symptoms to potentially fatal neuromuscular and cardiac symptoms. Tetany, carpopedal spasm, and ECG abnormalities such QT prolongation can result from acute drops in serum calcium. Long-term usage of proton pump inhibitors (PPIs) is frequently disregarded as a reversible etiology, despite the fact that drugs are known causes. To avoid major issues and guarantee timely treatment, early detection is essential. CASE PRESENTATION: Acute painful carpopedal spasms and perioral numbness were reported by

Chronic kidney disease progression with proton pump inhibitors versus H2 receptor antagonists in NSAID users.

BACKGROUND: Clinical evidence comparing the renal impact of proton pump inhibitors (PPIs) and Histamine-2 receptor antagonists (H2RAs) in patients with chronic kidney disease (CKD) receiving traditional non-steroidal anti-inflammatory drugs (tNSAIDs) is lacking. We evaluated CKD progression risk associated with PPIs versus H2RAs within a tNSAID-treated cohort to inform clinical prescribing. METHODS: This retrospective cohort study used South Korean nationwide claims data (2018-2023). Stabilized inverse probability of treatment weighting (SIPTW) balanced the cohorts. Posterior Reversible Encephalopathy Syndrome Associated With Severe Proton Pump Inhibitor-Induced Hypomagnesemia: A Rare Case.

Posterior reversible encephalopathy syndrome (PRES) is a potentially reversible cause of acute neurological deterioration. Hypertension is the most common trigger, but metabolic and drug-related causes must also be considered. We describe a patient who developed PRES with posterior circulation involvement in the absence of hypertension. The patient showed severe hypomagnesemia after prolonged proton pump inhibitor (PPI) use, where the onset of neurological symptoms occurred after the increase of esomeprazole (40 mg/day). Brain magnetic resonance imaging (MRI) showed PRES findings in the cerebe

Balancing therapeutic benefits and hidden risks of proton pump inhibitors in pediatric practice: a narrative review and update.

Proton pump inhibitors (PPIs) are drugs whose main effect is catalytic and long-lasting suppression of gastric acid secretion, with an anti-inflammatory effect. The main indications for PPIs use include the treatment of gastroesophageal reflux disease, peptic ulcer disease, functional dyspepsia, Barrett esophagus, eosinophilic esophagitis, and hypersecretory diseases such as gastrinoma and Zollinger-Ellison syndrome. Their contribution to eradication therapy for Helicobacter pylori is also important. However, the long-term use of PPIs has been associated with various adverse drug reactions and

Nonvariceal Upper GI Bleeding in Established Users of Proton-Pump Inhibitors: Higher Incidence But Better Outcomes.

INTRODUCTION: Nonvariceal upper gastrointestinal bleeding (NVUGIB) still occurs despite taking proton-pump inhibitors (PPIs). We tested the hypothesis that PPI efficacy fades with time. METHODS: This is a prospective and controlled observational comparison of cases of NVUGIB arising from the adult PPI-users' and nonusers' subpopulations of a well-defined community. Patients presenting with NVUGIB were grouped into PPI-users (index group) and nonusers (control group). RESULTS: The incidence of NVUGIB in our overall adult population was 89.7 in 2019 and 115.1 per 100,000 in 2022 ( P = 0.01). O

Association of Long-Term Proton Pump Inhibitor Use With Nutrient Deficiencies: A Retrospective Cross-Sectional Study.

Background Proton pump inhibitors (PPIs) are generally employed to treat acid-related disorders. However, chronic acid blockade can reduce the absorption of essential micronutrients, such as vitamin B12, magnesium, calcium, and iron, potentially contributing to clinically significant deficiencies as defined by standard laboratory reference ranges. Methods A retrospective observational study was conducted using electronic medical records collected between July and December 2025. The study included adults (18 years and older) who had been taking PPIs continuously for six months or more. Nutrient

Proton Pump Inhibitors Versus H2 Blockers and the Risk of Incident Dementia in Adults Younger Than 65.

BACKGROUND: Proton pump inhibitors (PPIs) and histamine-2 receptor antagonists (H2 blockers) are routinely prescribed for the long-term management of chronic acid-related gastrointestinal conditions. Although prior research has explored a potential link between PPI exposure and cognitive decline, findings across studies have been inconsistent, and most investigations have concentrated on elderly populations. OBJECTIVE: This study aimed to evaluate whether PPI therapy, relative to H2 blocker therapy, was associated with differential incident dementia risk among adults below the age of 65 who c
