Appendices removed at cesarean section: histopathology.

Appendices removed from 100 healthy Nigerian Igbo women during elective cesarean operation and examined microscopically as three random cross sections showed that lesions categorized as luminal pus, luminal fibrosis, mucosal ulceration, muscular inflammation, crypt abscess and crypt necrosis were present in numerous sections. As elsewhere, these asymptomatic patients might have had clinical acute appendicitis in the puerperium. to explain the possible outcome in the rest of such patients, the crypt of Lieberkühn was advanced as a research-worthy histopathologic unit. Thus, in the natural histo

Gonorrhea, Trichomonas and Candida among gravid and nongravid women in cameroon.

A case-control study of 1358 pregnant and nonpregnant women with and without symptoms of a lower genital tract infection were investigated for evidence of venereal disease. The prevalence of gonorrhea, Trichomonas and Candida in pregnancy was 15.0%, 20.6% and 41.3%, respectively, while the equivalent percentages in the nonpregnant women varied from 16.5% to 21.6%, 2.4% to 22.8% and 33.3% to 39.3%. However, these differences are not statistically significant. The implications of the findings are discussed and a screening program suggested.

Clinical chorioamnionitis at term: definition, pathogenesis, microbiology, diagnosis, and treatment.

Clinical chorioamnionitis, the most common infection-related diagnosis in labor and delivery units, is an antecedent of puerperal infection and neonatal sepsis. The condition is suspected when intrapartum fever is associated with two other maternal and fetal signs of local or systemic inflammation (eg, maternal tachycardia, uterine tenderness, maternal leukocytosis, malodorous vaginal discharge or amniotic fluid, and fetal tachycardia). Clinical chorioamnionitis is a syndrome caused by intraamniotic infection, sterile intraamniotic inflammation (inflammation without bacteria), or systemic mate…

Maternal and Neonatal Infectious Outcomes Compared According to Rupture of Membrane Duration and Antibiotic Administration: A Retrospective Study.

BACKGROUND: Prolonged rupture of membranes (ROM) is associated with peripartum infections; the optimal timing to initiate prophylactic antibiotic treatment is inconclusive. We compared maternal and neonatal infectious morbidity and bacterial distribution in chorioamniotic-membrane cultures according to a ROM-to-delivery interval of 12-18 versus ≥18 hours. METHODS: This retrospective cohort study was conducted in a single tertiary university-affiliated hospital from January 2020 to January 2023. Labor was induced in term singleton pregnant women with ROM ≥12 hours who did not deliver spontaneo

Prevalence of postpartum infections: a population-based observational study.

We investigated the prevalence of postpartum infections among women giving birth during 1 year in a population-based observational/questionnaire study at seven hospitals in the southeast region of Sweden. Of the women >99% (n = 11,124) received a questionnaire to inquire if they had endometritis, mastitis, or wound, urinary tract or any other infection within 2 months postpartum and whether they received antibiotics for this. Prevalence rates for infections and antibiotic treatment were estimated. The response rate was 60.1%. At least one infectious episode was reported by 10.3% of the women a

Early recognition of serious infections in obstetrics and gynecology.

Pelvic infections commonly occur in pregnant and postoperative women. Most of these infections are readily diagnosed because of their typical clinical manifestations and prompt antimicrobial therapy that leads to the resolution of disease. However, uncommon cases may escape detection and the severity of the process may also go unrecognized without a careful assessment of clues available through physical examination and laboratory testing. In some cases a particularly virulent pathogen can lead to severe infection, septic shock and its consequences, even death, despite clinicians acting appropr

Randomized clinical trial of extended spectrum antibiotic prophylaxis with coverage for Ureaplasma urealyticum to reduce post-cesarean delivery endometritis.

OBJECTIVE: To determine if extended spectrum prophylactic antibiotic treatment (with efficacy against Ureaplasma urealyticum) reduces post-cesarean delivery clinical endometritis. METHODS: After cord clamping at cesarean delivery, subjects received prophylaxis with cefotetan. Subjects were then simultaneously randomized (double blind) to receive doxycyline plus azithromycin versus placebo. Post-cesarean delivery endometritis was defined clinically as fever of 100.4F or higher with one or more supporting clinical signs or a physician diagnosis of endometritis plus the absence of a nonpelvic so

[Fundamental and clinical studies on aztreonam in the field of obstetrics and gynecology].

Aztreonam (SQ 26,776, AZT), a new monobactam antibiotic, was fundamentally and clinically studied with the following results. Uterine and adnexal concentrations of AZT after intravenous injection of 1 g were highest 3 hours after administration in the ranges of between 18.6-23.4 micrograms/g 16.5-28.2 micrograms/g, respectively, and and rapidly decreased thereafter. Penetration of AZT into the pelvic dead space exudate was quickly recognized after intravenous injection of 1 g and its concentration 30 minutes after administration was 14.08 +/- 7.08 micrograms/ml and highest (22.35 +/- 5.85 micr

[Importance of sanitary-antiepidemic measures in preventing staphylococcal infections in maternity hospitals].

The results of the realization of antistaphylococcal measures in maternity hospitals, carried out under the guidance and control of the sanitary and epidemiological service, are presented. The sanitary and epidemiological station registered each case of staphylococcal infection in nursing mothers and infants and investigated its epidemiological aspects. The exposure of the causes responsible for the disease allowed to take up the necessary antiepidemic measures in due direction, thus preventing the spread of hospital infections.

Postpartum infections and antimicrobial resistance of responsible pathogens in Ukraine: results a multicenter study (2020-2022).

OBJECTIVE: Aim: To determine the current prevalence of postpartum infections and antimicrobial resistance and antimicrobial resistance of responsible pathogens in Ukraine. PATIENTS AND METHODS: Materials and Methods: Multicenter prospective cohort study was conducted from January 2020 to December 2022 in fifteen hospitals from twelve regions of Ukraine. Definitions of healthcare- associated postpartum infection were adapted from the Centers for Disease Control and Prevention's National Healthcare Safety Network. Antibiotic susceptibility was done by the disc diffusion test as recommended by E

A model to predict risk of postpartum infection after Caesarean delivery.

PURPOSE: The purpose of this study is to build and validate a statistical model to predict infection after caesarean delivery (CD). METHODS: Patient and surgical variables within 30 d of CD were collected on 2419 women. Postpartum infection included surgical site infection, urinary tract infection, endomyometritis and pneumonia. The data were split into model development and internal validation (1 January-31 August; N = 1641) and temporal validation subsets (1 September-31 December; N = 778). Logistic regression models were fit to the data with concordance index and calibration curves used to

Group A streptococcal infections in obstetrics and gynecology.

Group A streptococcal (GAS) infections continue to be an infrequent, but potentially lethal infections in women despite the victory over childbed fever in the 1800s. Invasive group A streptococcal infection still causes 40% of septic deaths among patients with postpartum endometritis, necrotizing fasciitis, and toxic shock syndrome. Many times symptoms and signs of this infection are nonspecific, but laboratory evaluation can suggest serious infection. The prudent combination of antibiotic and surgical therapy can be lifesaving.
