Patient-Reported Outcomes in Radiation Oncology.

Patient-reported outcomes (PROs) assessments arguably provide the most accurate description of the patient experience, as they are directly derived from the patient without the filter of a provider. Utilizing instruments to assess PROs in radiation oncology enables a provider to measure pretreatment, on-treatment, and posttreatment symptoms. In the clinic, PROs are supplemental to physician-derived ratings that help create a complete clinical picture of a patient at a given time point to inform shared decision-making. A compilation of PROs that arise within trials, specific for given treatment…

Radiation-Induced Hydrogel for Water Treatment.

Along with serving as drug delivery sensors and flexible devices, hydrogels are playing pioneering roles in water purification. Both chemical and radiation methods can produce hydrogels, with the latter method gaining preference for its pure adducts. The water treatment process entails the removal of heavy and toxic metals (above the threshold amount), dyes, and solid wastes from industrial effluents, seawater, and groundwater, as well as sterilization for microorganism destruction. This review analyzed the different types of hydrogels produced by applying various radiations for water treatmen…

Commando procedure for radiation heart disease.

The Commando procedure is an important tool to address extensive calcification of the aortic and mitral valves associated with radiation heart disease. We present a symptomatic patient with radiation heart disease associated with calcification of the mitral and aortic valves and the aortomitral curtain, which is typical of this pathology. The surgical approach consisted of exposure through aortotomy and left atrial dome, followed by aortic and mitral valve debridement, aortic and mitral valve replacement, with aortomitral curtain reconstruction using bovine pericardial patch. This procedure av…

Evolution of radiation-induced dermatitis treatment.

Radiation-induced skin damage (RID) is the most prevalent, significant side effect of radiotherapy (RT). Nearly 95% of patients experience moderate to severe skin reactions after receiving radiation therapy. However, criteria for acute radiation dermatitis (ARD) treatment remain unavailable. Topical agents with anti-inflammatory properties may protect the skin and facilitate tissue regeneration in patients with RID. Many of these topical agents function through nuclear factor kappa B pathway regulation. They either reduce the levels of inflammatory factors or elicit anti-inflammatory propertie…

Radiation for inflammatory breast cancer: Updates.

Inflammatory breast cancer (IBC) is a diagnosis based on a constellation of clinical features of edema (peau d'orange) of a third or more of the skin of the breast with a palpable border and a rapid onset of breast erythema. Incidence of IBC has increased over time, although it still makes up only 1-4% of all breast cancer diagnoses. Despite recent encouraging data on clinical outcomes, the published local-regional control rates remain consistently lower than the rates for non-IBC. In this review, we focus on radiotherapy, provide a framework for multi-disciplinary care for IBC, describe local

Radiation therapy options in kidney cancer.

PURPOSE OF REVIEW: In this review, the authors discuss the use of stereotactic body radiation therapy (SBRT) for the treatment of primary and metastatic renal cell carcinoma (RCC). RECENT FINDINGS: For primary RCC treated with SBRT, local control is estimated at >95%, and grade 3-4 toxicity is limited at ≤5%. The difference in glomerular filtration rate pretreatment versus posttreatment was about 7.7 ml/min. For metastatic RCC treated with SBRT, the 1-year local control is ~90%. The incidence of any grade 3-4 toxicity is ~1%. Several ongoing trials are evaluating SBRT in combination or in lie…

Combination intraoral radiation mouthguard-positioning stent.

The sequelae of head-and-neck radiation may include hyposalivation, dysgeusia, trismus, mucositis, and osteoradionecrosis. A mouthguard used during radiation therapy can mitigate the effects of backscatter radiation from dental restorations. In addition, an intraoral positioning stent can assist in repositioning oral structures, such as the tongue, away from the field of radiation during treatment, thereby limiting dose delivery. The purpose of this article is to provide a technique to fabricate a combination prosthesis, which functions to reposition oral structures as well as mitigate the eff…

Radiation-induced optic neuropathy: a review.

Radiation is a commonly used treatment modality for head and neck as well as CNS tumours, both benign and malignant. As newer oncology treatments such as immunotherapies allow for longer survival, complications from radiation therapy are becoming more common. Radiation-induced optic neuropathy is a feared complication due to rapid onset and potential for severe and bilateral vision loss. Careful monitoring of high-risk patients and early recognition are crucial for initiating treatment to prevent severe vision loss due to a narrow therapeutic window. This review discusses presentation, aetiolo…

Ionizing radiation and volumetric mammographic density.

OBJECTIVES: Mammographic density (MD) refers to the percentage of dense tissue of an entire breast and was proposed to be used as a surrogate marker for breast cancer. High-dose ionizing radiation (IR) has been recognized as a breast cancer risk factor. The aim of our study was to investigate association between lifetime low dose ionizing radiation (LDIR) and MD. MATERIAL AND METHODS: A cross-sectional study included 467 women aged 40-60 years who underwent screening mammography in Łódź, Poland. The digital mammography examination of the breasts included both craniocaudal and mediolateral obl…

Proton Radiation Therapy for Pediatric Craniopharyngioma.

BACKGROUND: Radiation therapy (RT) is used for pediatric craniopharyngioma in the definitive, adjuvant, or salvage settings. Proton RT may be useful owing to tumor proximity to eloquent anatomy. We report clinical outcomes for a large cohort treated with proton therapy. METHODS: We conducted a retrospective review of pediatric patients (≤21 years) treated with surgery and proton therapy for craniopharyngioma between August 2002 and October 2018. Clinical characteristics, treatment course, and outcomes were recorded. Acute toxicity was graded using Common Terminology Criteria for Adverse Event…

Radiation recall reactions: An oncologic enigma.

Radiation recall reactions (RRR) are uncommon but are a well-known phenomenon to oncologists. Tissue damage in a prior irradiation portal is 'recalled' after the administration of a drug, historically cytotoxics, or more recently, targeted or immunotherapeutic agents. Even COVID-19 vaccines are a reported cause. RRR are enigmatic in that their cause is unknown, but they generally have the histopathological and clinical features of acute or chronic inflammation. They can occur in a variety of tissues, the commonest being skin, which accounts for two-thirds of reported cases. They are generally …

Radiation-induced lung injury: current evidence.

Chemo-radiotherapy and systemic therapies have proven satisfactory outcomes as standard treatments for various thoracic malignancies; however, adverse pulmonary effects, like pneumonitis, can be life-threatening. Pneumonitis is caused by direct cytotoxic effect, oxidative stress, and immune-mediated injury. Radiotherapy Induced Lung Injury (RILI) encompasses two phases: an early phase known as Radiation Pneumonitis (RP), characterized by acute lung tissue inflammation as a result of exposure to radiation; and a late phase called Radiation Fibrosis (RF), a clinical syndrome that results from ch…

Radiation-Induced Optic Neuropathy: Literature Review.

Radiation-induced optic neuropathy (RION) is a rare disease caused by exposure of the optic nerves to radiation during radiotherapy procedures for head and neck tumours. The purpose of this study was to review and summarise the epidemiology, risk factors, clinical presentations, pathphysiology characteristics, diagnosis, and management of RION. Its occurrence is associated with cumulative doses of radiation above 50 Gy, presence of multi-morbidities and the presence of concomitant chemotherapy and radiotherapy. It manifests with acute, painless, and monocular loss of vision, and these symptoms…

Reducing radiation dose: Yes I can!

There are multiple ways to reduce radiation dose by: (a) not using radiation unless absolutely needed; (b) utilizing settings that give the lowest possible dose when using radiation; and (c) shielding. Disposable radiation drapes, such as the SEPARPROCATH® can significantly reduce operator radiation dose. Radiation is bad for both patient and operator, therefore do something about it, starting with your next case!

Pathology of radiation induced heart disease.

The aim of this paper is dual - to review the relevant literature on pathology of radiation induced heart disease (RIHD), and to present an illustrative case of our own. Therapeutic ionizing radiation, such as that used in the treatment of Hodgkin´s lymphoma and cancers of left breast, lungs, esophagus, and thymus, can cause cardiac damage that may take several years to manifest. The spectrum of RIHD is broad and includes [1] pericarditis and pericardial effusion; [2] endocardial fibrosis and valvular dysfunction; [3] nonischemic myocardial fibrosis; [4] obstructive coronary artery disease wit…
