Paradental (mandibular inflammatory buccal) cyst.

The paradental cyst is commonly misinterpreted when associated with atypical clinical and radiographic characteristics, in turn causing diagnostic problems. For this reason, the study of the differential diagnosis of this lesion has become extremely important. In addition, the correlation of clinical, histologic, and radiographic findings are also of great value in obtaining accurate diagnoses. The minor variations in the clinical appearance of paradental cysts make it feasible to consider the two main groups of cysts separately: those associated with 1st and 2nd permanent molars of the mandib

Combined immunohistochemistry for the differential diagnosis of cystic jaw lesions: its practical use in surgical pathology.

AIMS: Histopathological distinction of cystic jaw lesions, including odontogenic tumours, is challenging because their lining epithelia, which are basically stratified squamous epithelia, resemble each other, especially when they become hyperplastic from inflammatory reaction. The aim of this study was to seek practical measures to differentiate such lesions. METHODS AND RESULTS: Nineteen surgical specimens from unicystic ameloblastomas (UAs), 17 from keratocystic odontogenic tumours (KCOTs), 13 from dentigerous cysts (DCs), 10 from lateral periodontal cysts (LPCs) and 20 from radicular cysts…

Enzyme histochemical studies on the formation of hyalin bodies in the epithelium of odontogenic cysts.

Frozen sections of eight odontogenic cysts, including one keratocyst, were incubated to show the following enzyme activities: NADH2 diaphorase, NADPH2 diaphorase, glucose-6-phosphate dehydrogenase, acid phosphatase and leucine aminopeptidase. The disbribution of lipid was shown by the oil red 0 method. The activities of all three oxidative enzymes were strongest in epithelial cells bordering hyalin bodies and in basal cells in the epithelial lining. Hydrolytic enzyme activity was absent from all but the most superficial epithelial cells but was present in macrophages and, in lesser amounts, in

Incidental bony pathology when reporting trauma orthopantomograms.

Radiologists frequently report orthopantomograms (OPTs) and other views of the mandible, most often in patients who have suffered facial trauma. These examinations may reveal incidental pathology. It is important that radiologists are aware of the radiological appearances and the clinical significance of these lesions. In this review we will present examples of the more common odontogenic lesions including: radicular cyst, odontogenic keratocyst, dentigerous cyst, ameloblastoma, and also examples of non-odontogenic pathology: bisphosphonate-related osteonecrosis of the jaw (BRONJ) and chronic …

A pigmented dentigerous cyst in a patient with multiple dentigerous cysts of the jaws: a case report.

AIM: The aim of this article is to report an unusual and interesting case of non-syndromic multiple dentigerous cysts of the maxilla and mandible associated with melanin pigment and melanocytes in the epithelial lining in one of the cysts. In addition, embryologic aspect and origin of melanocytes are briefly discussed. BACKGROUND: Dentigerous cysts are the second most common odontogenic cysts after radicular cysts. They are usually solitary with multiple cysts reported on occasion in association with syndromes. While melanocytes and melanin-pigment are widely distributed in the skin, the nerv…

High expression of osteopontin and CD44v6 in odontogenic keratocysts.

BACKGROUND/PURPOSE: Odontogenic keratocysts (OKCs) are more aggressive and more osteolytic lesions than dentigerous cysts (DCs) and radicular cysts (RCs). Osteopontin (OPN) is related to cancer metastasis and bone destruction. Binding of OPN to its cell membrane receptors integrin alphav and CD44v6 can enhance tumor cell motility, migration, invasion and spread. This study assessed the possible contribution of OPN, integrin alphav and CD44v6 to the local aggressive behavior and osteolytic ability of OKCs. METHODS: We used an immunohistochemical method to examine the expression of OPN, integri…

The root of the problem: Occurrence of typical and atypical periapical pathoses.

BACKGROUND: A preponderance of periapical radiolucencies are of inflammatory etiology (radicular cysts or periapical granulomas) secondary to pulpal disease. In some instances, however, a suspected periapical inflammatory lesion is not a consequence of pulpal disease but instead represents a lesion of noninflammatory origin. The differential diagnosis for such lesions is broad, ranging from odontogenic cysts and tumors to metastatic disease. As the biological behavior of such lesions is varied, the distinction between inflammatory odontogenic periapical lesions and lesions of noninflammatory o…

Magnetic resonance imaging of a thoracic intraspinal synovial cyst.

A 50-year-old woman presented with mid-thoracic back pain that radiated to the left rib cage in a radicular manner. Magnetic resonance imaging revealed an extradural mass compressing the left T9 nerve root. The mass was adjacent to a degenerated T9-10 facet joint with an enhancing low signal intensity rim. Recognition of the characteristic imaging findings of an intraspinal synovial cyst resulted in the correct preoperative diagnosis despite the unusual location of this lesion at T9-10.

Adjunctive Plasma Rich in Growth Factors in the Treatment of Osteomyelitis and Large Odontogenic Cysts Prior to Successful Implant Rehabilitation: Case Report.

Osteomyelitis of the jaws presents a clinical challenge to conventional treatment, often requiring multiple surgical interventions resulting in significant deformity and presenting significant problems to satisfactory rehabilitation. While benign odontogenic cysts, such as the radicular cyst, are generally predictably treated, they can cause significant localized bone destruction and thus can present significant problems in satisfactory rehabilitation. In this case report, patients were treated combining conventionally appropriate surgical debridement and oral antibiotics with adjunctive plasm…
