REFINE (Rapid Feedback for quality Improvement in Neonatal rEsuscitation): an observational study of neonatal resuscitation training and practice in a tertiary hospital in Nepal.

BACKGROUND: Simulation-based training in neonatal resuscitation is more effective when reinforced by both practice and continuous improvement processes. We aim to evaluate the effectiveness of a quality improvement program combined with an innovative provider feedback device on neonatal resuscitation practice and outcomes in a public referral hospital of Nepal. METHODS: A pre- and post-intervention study will be implemented in Pokhara Academy of Health Sciences, a hospital with 8610 deliveries per year. The intervention package will include simulation-based training (Helping Babies Breathe) e…

A novel strategy sequentially linking mechanical cardiopulmonary resuscitation with extracorporeal cardiopulmonary resuscitation optimizes prognosis of refractory cardiac arrest: an illustrative case series.

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) to support cardiopulmonary resuscitation (CPR), also known as extracorporeal cardiopulmonary resuscitation (ECPR), has shown encouraging results in refractory cardiac arrest (RCA) resuscitation. However, its therapeutic benefits are linked to instant and uninterrupted chest compression (CC), besides early implementation. Mechanical CC can overcome the shortcomings of conventional manual CC, including fatigue and labor consumption, and ensure adequate blood perfusion. A strategy sequentially linking mechanical CPR with ECPR may earn extra f…

Temporary hemodynamic response with resuscitative endovascular balloon occlusion of the aorta (REBOA) during cardiopulmonary resuscitation in a case of traumatic cardiac arrest.

Uncontrolled hemorrhagic shock due to trauma is a life-threatening condition requiring rapid intervention. Every minute in the man-agement of these patients is valuable. In recent years, resuscitative endovascular aortic balloon occlusion (REBOA) has emerged as a valuable tool in emergency and trauma practice, providing additional time for the patient, particularly in cases where surgical hemosta-sis is delayed. A 62-year-old male patient with no known comorbidities was transported to our tertiary trauma center after receiving five minutes of cardiopulmonary resuscitation (CPR) and intubation …

A Case Report on Successful Resuscitation of a Two-Month-Old Infant in the Emergency Room: Neonatal Resuscitation Program (NRP) Guidelines in Practice.

Infants that appear with respiratory distress or cardiac arrest require immediate attention, and neonatal resuscitation is a crucial skill that can significantly impact the outcome. Here, we discuss a case of a two-month-old baby who needed ER resuscitation. The patient needed immediate assistance due to respiratory distress and cyanosis. This case study emphasizes how crucial it is to follow the Neonatal Resuscitation Program (NRP) algorithm because it allows the patient to have a successful outcome. Regarding the decision on whether to use NRP or Pediatric Advanced Life Support (PALS) guidel…

Hospital resuscitation in patients with primary hypothermic cardiac arrest.

Hypothermia is recognised as a reversible cause of cardiac arrest along with hypoxia, hypovolaemia and hypo or hyperkalaemia. However, hypothermia is the rarer one of these 'four Hs'. Because cardiac arrest caused primarily by hypothermia is a rare occurrence, resuscitation teams may lack real-world experience of it. This article sets out essential information for emergency department staff to understand this presentation and its management. It provides background information on hypothermia, describes the usual pattern of deterioration leading to cardiac arrest, and explains the key interventi…

Vasopressor Use in Acute Burn Resuscitation: A Retrospective Study.

BACKGROUND: Vasopressors are often introduced in severe burn patients who respond poorly to fluid resuscitation to maintain adequate blood pressure and avoid fluid creep. This study aims to characterise these patients and explore potential correlations between vasopressor use and patient outcomes. METHODS: Medical records from 2010 to 2023 for patients aged ≥ 18 years with burns ≥ 20% of the total body surface area (TBSA) were extracted from the TriNetX Research Network, a global federated health research platform aggregating de-identified electronic health records. The patients were divided …

Clinical and Nonclinical Factors and Advanced Neonatal Resuscitation Interventions.

IMPORTANCE: Approximately 1% of newborns receive advanced neonatal resuscitation interventions (ANRIs; consisting of endotracheal intubation, chest compressions, and epinephrine administration), the likelihood of which increases with known clinical factors. Associations of socioeconomic status (SES) and health system factors with ANRI are unknown. OBJECTIVE: To examine the associations among maternal SES, remoteness of maternal residence, birth-site level of service, clinical factors, and ANRIs. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used an administrative dataset of a…

Guidelines for Neonatal Cardiopulmonary Resuscitation (2025 Update). Executive summary.

The 2025 update on neonatal cardiopulmonary resuscitation from the American Heart Association (AHA), the American Academy of Pediatrics (AAP), ILCOR, and the European Resuscitation Council (ERC), adapted by the Sociedad Argentina de Pediatría (SAP, by its Spanish acronym), retains the core neonatal resuscitation algorithm, but incorporates a systemic approach based on the "Newborn Chain of Care". Emphasis is placed on prenatal anticipation, early physiological monitoring, temperature control, rational oxygen titration, and post-resuscitation care. Effective ventilation remains the most importa…

Ergonomic Considerations in Neonatal Cardiopulmonary Resuscitation: Implications for Practice.

BACKGROUND: Prolonged neonatal cardiopulmonary resuscitation (CPR) is critical for survival in cases of severe bradycardia due to asphyxia. High-performance CPR, emphasizing consistent compression depth and full recoil, improves survival rates. PURPOSE: This pilot study explored how environmental and provider-specific factors-including bed height, positioning, and anthropometry-affect the consistency of neonatal CPR using the 2-thumb technique. METHODS: Twenty-two neonatal intensive care unit providers performed 2-minute CPR trials on a neonatal simulator under 2 conditions: self-selected be…

Fluid resuscitation dilemma: anticipating massive blood loss in hysterectomy.

Massive bleeding during obstetric surgeries, such as hysterectomy, is a significant cause of maternal mortality and morbidity worldwide. In such cases, fluid resuscitation is imperative. However, intraoperative guidelines and fluid requirements cannot be universally standardized to accommodate every patient, surgical procedure, or condition. The debate between restrictive and liberal fluid resuscitation during massive intraoperative bleeding has persisted for a long time. Both approaches have been associated with side effects that impact patient morbidity and mortality. Despite the fact that t…

A contemporary analysis of prehospital crystalloid resuscitation after trauma.

INTRODUCTION: Minimizing crystalloid administration to hemorrhaging trauma patients has been shown to decrease morbidity and mortality. Iatrogenic harm from 'over-resuscitation' may be a concern for trauma patients undergoing prolonged EMS transport. Our primary objective was to quantify the volume of prehospital crystalloid administered to hypotensive trauma patients with at least 30 min of exposure to prehospital care for whom fluid administration was not indicated in the intervention arm of prior randomized trials of fluid restriction. In addition, we aimed to identify factors associated wi…

Pulmonary fat embolism in non-survivors after cardiopulmonary resuscitation.

BACKGROUND: Blunt trauma acting against the human body presents the fundamental cause of pulmonary fat embolism (PFE) and fat embolism syndrome. The aim of the present study was to investigate PFE in non-survivors after cardiopulmonary resuscitation (CPR). METHODS: This was a prospective cohort study conducted in University Hospital Ostrava, Czech Republic. Within a 4-year study period, all non-survivors after CPR because of out-of-hospital cardiac arrest were assessed for the study eligibility. The presence/seriousness of PFE was determined by microscopic examination of cryo-sections of lung…

Patient's airway monitoring during cardiopulmonary resuscitation using deep networks.

Cardiopulmonary resuscitation (CPR) is a crucial life-saving technique commonly administered to individuals experiencing cardiac arrest. Among the important aspects of CPR is ensuring the correct airway position of the patient, which is typically monitored by human tutors or supervisors. This study aims to utilize deep transfer learning for the detection of the patient's correct and incorrect airway position during cardiopulmonary resuscitation. To address the challenge of identifying the airway position, we curated a dataset consisting of 198 recorded video sequences, each lasting 6-8 s, show…
