Risk assessment and prevention of disease progression: new concepts in bronchiectasis management.

Bronchiectasis management is traditionally guided by exacerbation frequency. Previous guidelines have recommended treatment escalation only in patients experiencing three or more exacerbations per year. In contrast, the 2025 European Respiratory Society (ERS) guideline introduces a paradigm shift by advocating individualised risk assessment to identify patients at high risk of future complications, instead of relying solely on prior exacerbation. High-risk features include severe daily symptoms, underlying aetiologies or comorbidities, such as chronic obstructive pulmonary disease, rheumatoid

Lifetime management of valvular heart disease in younger and low risk patients.

Valvular heart disease in younger and low-risk patients presents unique therapeutic challenges because treatment decisions must account not only for immediate procedural success but also for the durability of the index intervention, the feasibility of future reinterventions, and the preservation of subsequent treatment options. Consequently, lifetime management has emerged as a longitudinal treatment paradigm in which every intervention influences the sequence, complexity, and outcomes of future procedures. Over the past decade, transcatheter valve therapies have expanded rapidly from a treatm

Pathophysiology, risk factors and clinical management for polymyxin-associated acute kidney injury.

Polymyxins serve as a 'last-line' defence against Gram-negative bacterial infections and are frequently used in critically ill patients with multidrug-resistant pathogens. However, polymyxin-associated acute kidney injury (PA-AKI) remains a major factor limiting their clinical application. This review examines the pathophysiology, risk factors and clinical management of PA-AKI, providing updated perspectives on its prevention and treatment. Reducing exposure to concomitant nephrotoxic agents, together with timely and standardized monitoring of serum creatinine, urine volume and polymyxin conce

Echocardiography-based intelligent diagnosis and risk stratification management for tetralogy of Fallot.

BACKGROUND: Early and accurate detection of Tetralogy of Fallot (TOF), along with proper risk stratification management, is critical for improving patient survival and prognosis. We developed an end-to-end automated framework for TOF, aimed at supporting decisions from preoperative diagnosis through postoperative risk prediction. METHODS: A total of 1986 filtered participants (1018 healthy controls, 480 TOF mimics, and 488 patients with TOF) from four centres were recruited for the development and validation of DynaTOF, an integrated diagnostic and predictive system. The DynaTOF system compri

Do spatially targeted weed management strategies increase risk of herbicide resistance evolution?

BACKGROUND: Spatially targeted spraying is an alternative to broadcast herbicide application that reduces herbicide use by restricting treatment to areas where weeds are detected. However, weed detection using remote or proximal sensing and classification algorithms is imperfect, which may allow weeds to escape treatment. This study examined the evolutionary consequences of spatially targeted spraying and evaluated whether applying herbicide to spray-buffer zones surrounding detected weeds could compensate for reduced detection accuracy. We tested two hypotheses: first, that spray-buffer zones

Pituitary adenomas associated with hydrocephalus: clinical characteristics, risk stratification, and clinical management.

OBJECTIVE: To characterize pituitary adenomas (PAs) complicated by hydrocephalus, identify factors associated with hydrocephalus occurrence and cerebrospinal fluid (CSF) diversion requirement, and develop a practical management framework. METHODS: We retrospectively reviewed consecutive patients with histopathologically confirmed PAs who underwent endoscopic endonasal surgery between January 2020 and June 2024. Hydrocephalus was defined radiologically by an Evans' index > 0.30. Forty-five hydrocephalus cases were propensity score-matched 1:3 to 135 non-hydrocephalus controls. A temporally sep

Dural Tears in Endoscopic Spine Surgery: Incidence, Risk Factors, Prevention, and Management.

Dural tear risk in endoscopic spine surgery is determined more by procedural complexity and surgeon experience than by the endoscopic technique alone, with the highest rates observed in interlaminar decompression for lumbar stenosis and the lowest in transforaminal procedures. Most incidental dural tears can now be managed endoscopically without routine conversion to open surgery, particularly when tears are recognized early and stratified according to size, location, cerebrospinal fluid flow, and neural element involvement. Patch- and graft-based techniques represent the current workhorse str

Perioperative management of SGLT2 inhibitors: balancing euglycaemic ketoacidosis risk and cardiorenal benefits.

Sodium-glucose cotransporter 2 (SGLT2) inhibitors are established therapies for type 2 diabetes, heart failure, and chronic kidney disease, creating a perioperative need to balance prevention of euglycaemic diabetic ketoacidosis (euDKA) against cardiorenal protection. Fasting, surgical stress, infection, reduced carbohydrate intake, insulin dose reduction, and hypovolaemia can amplify glycosuria and ketogenesis and precipitate euDKA. Most guidelines recommend withholding SGLT2 inhibitors for 3-4 days before elective surgery, although recommendations vary by agent, procedure, fasting duration,

Orthorexia nervosa: assessment, prevalence, high-risk groups, co-occurring features and management.

PURPOSE OF REVIEW: This review synthesizes research on orthorexia nervosa published in 2025 and early 2026. RECENT FINDINGS: Orthorexia nervosa is characterized by an excessive focus on healthy eating. Widely used measures, particularly the ORTO-15, have been criticized for poor reliability, prompting the development of new assessment instruments. Recent prevalence estimates of orthorexia nervosa vary widely. Meta-analyses based on ORTO-15 assessment in highly heterogeneous settings suggest an overall prevalence of 27.5%, with men and women equally at risk. However, more recent studies indica

HBV reactivation during immunotherapy for hepatocellular carcinoma: risk factors and clinical management.

Hepatitis B virus reactivation (HBVr) poses a serious clinical challenge and potentially life-threatening complication in patients with hepatocellular carcinoma (HCC), particularly amid the expanding use of modern immunotherapeutic agents. Despite progress in antiviral prophylaxis and refined risk-stratification strategies, HBVr continues to compromise treatment efficacy and survival outcomes, especially in patients receiving immune checkpoint inhibitors, tyrosine kinase inhibitors, or combination regimens. This review comprehensively synthesizes current evidence on the virological foundations

Nasopharyngeal carcinoma: epidemiology, risk factors, signaling pathways, clinical features, diagnosis, and management.

Nasopharyngeal carcinoma (NPC) is a distinctive epithelial malignancy characterized by marked geographic and ethnic clustering, with the greatest burden concentrated in East and Southeast Asia despite declining age-standardized incidence and mortality rates in many regions. NPC arises from a complex interaction among persistent Epstein-Barr virus (EBV) infection, inherited susceptibility, and environmental exposures. Among these factors, EBV plays a central role in endemic NPC through latent gene products, particularly latent membrane protein 1 (LMP1), latent membrane protein 2A (LMP2A), and E

Management of high-risk acute pulmonary embolism: an emulated target trial analysis.

BACKGROUND: High-risk acute pulmonary embolism (PE) is a life-threatening condition necessitating hemodynamic stabilization and rapid restoration of pulmonary perfusion. In this context, evidence regarding the benefit of advanced circulatory support and pulmonary recanalization strategies is still limited. METHODS: In this observational study, we assessed data of 1060 patients treated for high-risk acute PE with 991 being included in a target trial emulation to investigate all-cause in-hospital mortality estimates with different advanced treatment strategies. The four treatment groups consist

TP53 -Mutated Myeloid Neoplasms: 2024 Update on Diagnosis, Risk-Stratification, and Management.

Alterations in the tumor suppressor gene TP53 are common in human cancers and are associated with an aggressive nature. Approximately 8%-12% of myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML) harbor TP53 mutations (TP53 mut) and present immense challenges due to inherent chemoresistance and poor outcomes. As TP53 mut are more common in older individuals and those with secondary/therapy-related myeloid neoplasms (MN), their incidence is expected to increase with an aging population and rising proportion of cancer survivors. Treatments used for other MN-intensive chemotherapy, hy

Fatigue in inflammatory bowel disease: Prevalence, risk factors, assessment, outcomes, and management.

Fatigue is a prevalent and often debilitating symptom in individuals with inflammatory bowel disease (IBD), affecting a substantial proportion of patients, even during periods of disease remission. The prevalence of fatigue in IBD remains high, affecting around half of the IBD patients and being more common in patients with active disease than those in remission. Several risk factors contribute to fatigue in IBD, including active disease and pro-inflammatory state, nutritional deficiencies and anemia, sleep disturbances, psychological comorbidities, microbiota changes and the gut-brain axis, m

A Management Algorithm for High-Grade Acute Cholecystitis in High-Risk Patients.

BACKGROUND: Acute cholecystitis (AC) is among the most frequently encountered surgical problems. Current management typically includes laparoscopic cholecystectomy (LC). Suboptimal outcomes of LC can include bile duct injury, open conversion (OC), and/or subtotal cholecystectomy (SC). Percutaneous cholecystostomy tube (PCT) drainage with interval cholecystectomy has emerged as an alternative in high-risk patients but outcomes vary widely. We describe an evidence-based algorithm for managing AC in high-risk patients via PCT followed by minimally invasive cholecystectomy (MIS-C). We hypothesized
