Treatment Sequencing in Renal Ewing's Sarcoma With Inferior Vena Cava (IVC) Thrombus: A Comparative Case-Based Analysis.

Renal Ewing's sarcoma with inferior vena cava (IVC) tumour thrombus is a rare and aggressive malignancy that may mimic renal cell carcinoma (RCC) on imaging, creating diagnostic and therapeutic challenges. We report two young female patients presenting with large renal masses and level II IVC tumour thrombus who were managed using contrasting treatment sequences. In the first patient, image-guided biopsy established the diagnosis, allowing neoadjuvant systemic chemotherapy that resulted in substantial tumour and thrombus downsizing and facilitated subsequent surgical resection. In the second p

Functional Elbow Reconstruction After Complete Ulnar Resection for Ewing's Sarcoma: Radial Neck Transposition as a Biomechanical Alternative.

INTRODUCTION: Ewing's sarcoma of the ulna is rare, and its wide resection poses challenges for preserving upper limb function. Elbow reconstruction must ensure joint stability and forearm mobility. Conventional alternatives, such as bone grafts and prostheses, have limitations, including a high risk of complications. OBJECTIVE: This study was aimed at reporting a case of elbow reconstruction using radial neck transposition to the humeral trochlea after ulnar resection for Ewing's sarcoma, evaluating the functional outcomes of this technique. METHODS: A 17-year-old male patient with Ewing's s

Recurrent Ewing's Sarcoma of the Chest Wall in an Adolescent Male Patient: A Complex Multimodal Management and Progressive Disease Course.

Ewing's sarcoma is a rare, aggressive malignant tumor of bone and soft tissue that predominantly affects adolescents and young adults. Chest wall involvement, though uncommon, presents unique surgical and oncologic challenges. We report a 17-year-old male patient who initially presented with thoracic pain and swelling. Imaging revealed a posterior chest wall mass consistent with Ewing's sarcoma. He underwent surgical resection with rib removal, followed by multiple lines of chemotherapy (vincristine, adriamycin (doxorubicin), and cyclophosphamide/ifosfamide and etoposide (VAC/IE) ×15 cycles; c

Sinonasal extraosseous Ewing's sarcoma of the nasal cavity with EWSR1: FLI1 fusion - a rare case report from Nepal.

INTRODUCTION: Ewing's sarcoma is a rare, aggressive malignancy primarily originating in bones. Sinonasal extraosseous Ewing sarcoma (EES) displays morphologic heterogeneity that often correlates with underlying genotypes, most commonly the EWSR1::FLI1 fusion. EES, a soft tissue variant, constitutes less than 15% of all Ewing's sarcoma cases and is exceptionally rare in the nasal cavity. Nasal EES presents with nonspecific symptoms such as nasal obstruction or bleeding, often leading to misdiagnosis and delayed treatment. Timely diagnosis through histopathological, immunohistochemical, and mole

Successful extracorporeal membrane oxygenation therapy in a child with coronavirus 2019 disease-associated ARDS and chemotherapy for Ewing's sarcoma.

Most children and adolescents with SARS-CoV-2 infection show no or mild symptoms, but those with medical histories can be susceptible to more severe forms of disease. There are few reported cases of extracorporeal membrane oxygenation (ECMO) in pediatric patients with coronavirus disease 2019 (COVID-19). We present a previously healthy 13-year-old male diagnosed with metastatic Ewing's sarcoma at the same time as catastrophic acute respiratory distress syndrome due to COVID-19, who was successfully supported by venovenous-ECMO while he received the corresponding chemotherapy protocol.ECMO can

Extraskeletal Ewing's Sarcoma with CD7 Positivity and T-cell Receptor/Immunoglobulin Rearrangement Masquerading as T-lymphoblastic Lymphoma.

Background: Extraskeletal Ewing's Sarcoma (EES) may harbor more than one tumor-specific genetic abnormality, leading to diagnostic difficulties. Case report: We report a nine-year-old boy with recurrent mass of his right thigh. Tumor cells were round, with scant cytoplasm, finely dispersed chromatin, and inapparent, small nucleoli. The initial misdiagnosis was T-lymphoblastic lymphoma due to CD7 and TCR/Ig monoclonal rearrangement. As it expressed NKX2.2 and harbored an EWSR1-FLI1 fusion transcript, the diagnosis was changed to EES. The child underwent EES therapy with good initial response, b

Evaluation of PAX8 Expression and Its Potential Diagnostic and Prognostic Value in Renal and Extra-Renal Ewing Sarcomas/PNETs.

PAX8 is a transcription factor involved in the regulation of organogenesis of the thyroid gland, kidney, and Müllerian system. It is commonly expressed in epithelial tumors of thyroid and parathyroid glands, kidney, thymus, and female genital tract. PAX8 is increasingly used in the establishment of tissue of origin in carcinomas and has recently been identified in a subset of small blue round cell tumors including Ewing sarcomas/PNETs. However, it is unclear if this association in ES/PNETs is due to renal origin or is PNET specific. In this study we investigated the PAX8 staining pattern of pr

Dual-fibular reconstruction of a massive tibial defect after Ewing's sarcoma resection in a pediatric patient with a vascular variation.

BACKGROUND: In the management of malignancies of the extremities, limb salvage procedures have recently taken on greater significance. For those patients under intense adjuvant chemotherapy and with massive bone loss, free vascularized fibular grafting is currently advocated as a reliable reconstructive option, maybe because of the controversial results of bone transport in similar situations. However, when there is a vascular abnormality of either the recipient or donor extremity, microsurgical procedures are not feasible, further limiting potential reconstructive alternatives. METHODS: We p

Optimizing local control in Ewing sarcoma: a critical discussion of radiotherapy and surgical approaches with proposed decision-making algorithm; a narrative review.

Ewing sarcoma (ES) is a rare, aggressive bone malignancy where local control remains central to cure. Modern systemic therapy has improved 5-year overall survival for localized disease to ~ 70%, but outcomes vary by tumor site, size, and patient age. Surgery offers durable control when negative margins (R0) can be achieved without major functional loss, while radiotherapy (RT) is essential for unresectable tumors, close/positive margins, or anatomically challenging locations. This narrative review critically appraises the efficacy, limitations, and late effects of RT and surgery, synthesizing

Adamantinoma‑like Ewing Sarcoma of Head and Neck: A Clinicopathologic Study of Three Cases Including One EWSR1-Negative Case with NF1 Mutation.

Adamantinoma-like Ewing sarcoma (ALES) is a rare variant of Ewing sarcoma that predominantly occurs in the head and neck region, characterized by epithelial differentiation and overlapping morphology with other small round cell tumors. We present three cases of head and neck ALES (two in the parotid gland and one in the nasal cavity) in patients aged 23-42 years. Histologically, the tumors exhibited a spectrum of features, including small round blue cells and basaloid squamous morphologies. Immunohistochemically, the tumor cells variably expressed CKpan, p63, CD99, NKX2.2, FLI1, CD56, and Syn.

Evolution and Discovery of Matrine Derivatives as a New Class of Anti-Hepatic Fibrosis Agents Targeting Ewing Sarcoma Breakpoint Region 1 (EWSR1).

A series of new tricyclic matrinane derivatives were continuously synthesized and evaluated for their inhibitory effects on genes and proteins related to hepatic fibrosis at the cellular level, including collagen type I α1 chain (COL1A1), α smooth muscle actin (α-SMA), connective tissue growth factor (CTGF), and matrix metalloprotein 2 (MMP-2). Among them, compound 6k exerted an appealing potency and significantly reduced liver injury and fibrosis in both bile duct ligation (BDL) rats and Mdr2 knockout mice. An activity-based protein profiling (ABPP) assay indicated that 6k might directly bind
