Skin cancer after X-ray treatment for scalp ringworm.

Some 2,224 children given X-ray therapy for tinea capitis (ringworm of the scalp) have been followed for up to 50 years to determine cancer incidence, along with a control group of 1,380 tinea capitis patients given only topical medications. The study found a relative risk (RR) of 3.6 (95% confidence interval, 2.3-5.9) for basal cell skin cancer (BCC) of the head and neck among irradiated Caucasians (124 irradiated cases and 21 control cases), in response to a scalp dose of about 4.8 Gy. No melanomas of the head and neck have been seen, and only a few squamous cell carcinomas. About 40% of irr…

Erosive pustular dermatosis of the scalp after skin grafting.

Erosive pustular dermatosis of the scalp is a rare condition of unknown etiology that usually occurs in the elderly and is characterized by pustules that appear on the scalp leading to scarring alopecia. The histopathology is not specific. Its onset has been related with previous trauma on the scalp. Only three cases after skin grafting have been reported. We describe a case of erosive pustular dermatosis of the scalp appearing on a split-thickness skin graft placed after excision of a basal cell carcinoma.

Scalp potentials evoked by amplitude-modulated tones in dyslexia.

We recorded the far-field EEG potential evoked by amplitude modulation of acoustic stimuli (the amplitude modulation following response, AMFR) in adults with developmental dyslexia and in a matched control group of adults with no history of reading problems. The mean AMFR recorded from participants with dyslexia was significantly smaller than that recorded from members of the control group. In contrast, the amplitude of the click-evoked auditory brainstem response (ABR) was not significantly different between participant groups. Also, there was no difference between participant groups in the l

Fetal paroxysaml supraventricular tachycardia recorded by intrauterine scalp electrocardiography.

Fetal and neonatal paroxysmal supraventricular tachycardia (PST) requires attention because it may be associated with cardiac pathology and neonatal demise. Continuous monitoring of fetal heart rate and the ability to record clearly interpretable electrocardiograms from fetal scalp electrodes makes discovery and diagnoses of these fetal arrhythmias relatively simple. This report describes the discovery of this arrhythmia during routine DOPPLER ultrasound fetal monitoring and its documentation as PST by fetal scalp electrocardiography. (Fig. 1) The fetal heart rate during PST was 300 BPM, highe…

Folliculosebaceous cystic hamartoma presenting as a large scalp mass.

Folliculosebaceous cystic hamartoma (FSCH) is a rare cutaneous hamartoma, composed of follicular, sebaceous and mesenchymal elements, that usually presents as a small papule or nodule on the central face. We report the case of a man in his early 30s with a 10-year history of a large, reddish to violaceous scalp mass measuring 4.5 cm in greatest dimension. The initial clinical impression was a cylindroma. Histological examination revealed a dermal lesion with multiple sebaceous lobules clustered around a central infundibular structure within a fibrovascular stroma separated by cleft-like spaces…

Managing Scalp Psoriasis Through Ayurvedic Therapies: A Case Report.

BACKGROUND: Psoriasis, a chronic inflammatory skin condition, poses significant management challenges due to its persistent nature, recurrences, and side effects associated with conventional therapies. Ayurveda correlates psoriasis with Ekakushtha (a type of skin disorder), primarily involving Vata-Kapha dosha imbalance. OBJECTIVE: To evaluate the efficacy of Ayurvedic therapies, including Shodhana (Vamana, Raktamokshana) and Shamana (pacification medications), in managing scalp psoriasis. METHODOLOGY: A 35-year-old male with chronic scalp psoriasis underwent Panchakarma, including Vamana (t…

Unilobed rotational flap for traumatic scalp soft tissue loss.

Soft tissue injuries of the head and neck are the most common injuries encountered in trauma emergency department, accounting for over 50% of injuries. Scalp soft tissue loss continues to pose a challenge for reconstruction due to limited skin mobility in the area. We present a case report involving two patients with traumatic soft tissue loss affecting the scalp who underwent rotational flap closure with favourable cosmetic results. Case 1: A male patient in his early 30s arrived at the emergency department with a history of road traffic accident, sustaining head injury. Soft tissue loss was …

Simplified treatment of chronic scalp wounds with exposed skull.

BACKGROUND: Exposed cranial bone can present a considerable challenge to the reconstructive surgeon. Removal of the outer cortex of exposed skull bone has proven effective in the management of complex scalp wounds for which traditional reconstruction efforts were limited. OBJECTIVE: To demonstrate a simplified approach for management of scalp wounds with exposed skull. METHODS: Chronic wounds with exposed skull bone in elderly patients who were poor candidates for complex reconstructions were treated with removal of the exposed, outer bone cortex. The wounds involved desiccated, exposed bone…

Integra® Dermal Regeneration Template in Complex Scalp Reconstruction.

Background/Objectives: The need for surgical reconstruction of scalp defects following the excision of cutaneous skin cancers is an increasingly common procedure. Particular challenges arise when considering options for reconstruction of large defects not amenable to local skin flap coverage. The use of skin grafts poses the risk of donor site morbidity. This paper investigates the emerging use of Integra®, a synthetic acellular dermal regeneration template, as an alternative or adjunct to skin grafting in scalp reconstruction. Methods: The study presents a retrospective analysis of 101 patien…

Complex Scalp Reconstruction with Super Thin DIEP Free Flap.

Scalp reconstruction provides a unique challenge to the reconstructive surgeon, especially in terms of matching skin color and contour. We present a case of a 67-year-old man with a chronic scalp wound requiring soft tissue coverage after major debridement and coverage of a skull defect. We chose to use a super thin deep inferior epigastric perforator (DIEP) flap to provide good skin coverage along with good contour to the scalp. The use of a super thin DIEP flap, dissected above the superficial fascial plane, is a newer technique in the realm of free tissue transfers that has shown improved o…

Sensitive Scalp and Trichodynia: Epidemiology, Etiopathogenesis, Diagnosis, and Management.

Sensitive scalp (SSc) is considered a sensitive skin on the scalp, with its particularities. Although it is not rare in the dermatological practice and the term is commonly present in personal care products, this entity is poorly investigated in the medical literature. The etiopathogenesis is still uncertain, and the sensitivity may be associated with hair loss. Clinical manifestations are subjective symptoms of pruritus, burning, pain, pricking, and/or trichodynia, often with scalp erythema. SSc can be triggered by several factors (endogenous or exogenous). The diagnosis is guided by the anam…

Covering of large scalp defects prior to postoperative irradiation.

BACKGROUND AND OBJECTIVE: After R0 resection of extensive cutaneous squamous cell carcinoma of the scalp with indication for postoperative radiotherapy, closure techniques should be chosen that allow rapid initiation of radiotherapy. The aim of this retrospective analysis is to evaluate defect coverage by transverse transposition flap and split skin grafting of the donor site in such a scenario with regard to oncologic safety (recurrence rate) and permanence of wound closure. PATIENTS AND METHODS: Eleven patients were identified who had histologic cutaneous squamous cell carcinoma treated by …
