INTRODUCTION: A scaphoid fracture is the most common carpal fracture. When healing of the fracture fails (nonunion), a specific pattern of osteoarthrosis occurs, resulting in pain, restricted wrist motion and disability. Scaphoid fracture classification systems recognize fragment displacement as an important cause of nonunion. The fracture is considered unstable if the fragments are displaced. However, whether and how displaced bone fragments move with respect to one another has not yet been investigated in vivo. With a four-dimensional (4D) computed tomographic (CT) imaging technique we aim t

PURPOSE: Internal fixation has become a well-established alternative to casting for acute scaphoid fractures. Screw design has evolved, and several different types of screws of varying sizes are now available. The purpose of this study was to establish morphometric data for the human scaphoid, document variation in scaphoid dimensions between genders, and to evaluate symmetry in scaphoid measurements between the two sides.
METHODS: We measured length, width, and morphology of the scaphoid in 30 paired cadaveric specimens with reference to the long axis of the scaphoid from the proximal pole t

OBJECTIVES: Two to 5% of scaphoid fractures are missed on initial presentation. The failure of early recognition and treatment are considered to contribute to delayed union and non-union. Despite advances in diagnostic imaging, a dogmatic approach has persisted in the management of patients with clinical suspicion but no radiographic evidence of scaphoid fracture. A critical analysis of the current treatment protocol of indiscriminate cast immobilization and serial clinical and radiographic follow-up is presented.
METHODS: A prospective study involving 90 patients with clinical signs suggesti

BACKGROUND: Cadaveric specimens used in research are commonly frozen when stored and may undergo multiple freeze-thaw cycles during experime … computed tomography (CT) scans were performed on a cadaver wrist in both frozen and thawed states. Scaphoid bones were segmented using two approaches: a tight (native surface) segmentation and a smoothened version optimized for prosthetic articulation. The resulting 3D models were registered, and volume and shape differences between frozen and thawed states, a …

An intraosseous ganglion is a relatively uncommon, benign cystic lesion that occurs in young and middle-aged adults. Bilateral and symmetrical lesions of the wrist are rare. Intraosseous ganglia of the carpal bones are uncommon causes of chronic wrist pain. Isolated cases of intraosseous ganglion have been reported most commonly in the lunate and scaphoid. The lunate was most frequently affected, followed by the capitate, scaphoid, and triquetrum bones. Radiolucent lesions in the carpal bones are not uncommon and are often seen incidentally in asymptomatic patients. The differential diagnosis

BACKGROUND: Avascular necrosis (AVN) of the proximal pole is a well-known complication of scaphoid fractures. Avascular necrosis is poorly understood, including the transition from ischemia to necrosis, optimal treatment, and why some AVN heals but others do not. The primary purpose of this study is to evaluate patient-related factors that are associated with healing outcomes in individuals with proximal pole AVN following scaphoid fractures.
METHODS: This is a retrospective review of all patients diagnosed with scaphoid proximal pole AVN secondary to a fracture from 2018 to 2024 in a single

BACKGROUND: The purpose of this retrospective study was to examine the clinical and radiographic results of treatment of proximal scaphoid nonunion and avascular necrosis with vascularized bone graft from the distal part of the radius in adolescent patients.
METHODS: Between 1993 and 1996, three adolescents with a proximal scaphoid nonunion and avascular necrosis underwent vascularized bone-grafting and internal fixation. The mean age at the time of the fracture was 14.8 years (14.4, 14.6, and 15.3 years), and the mean time interval between the fracture and the surgery was 19.3 months (six, s

INTRODUCTION: Scaphoid is the most common carpal to be fractured (60%), caused by fall on outstretched hand, and proximal pole fractures are difficult to manage due to the high risk of non-union and osteonecrosis due to retrograde blood supply. Conservative management of proximal pole fractures leads to high rates of non-union; hence, different surgical techniques have been devised to address bone loss and optimize vascularity at fracture site to promote healing, such as vascularized bone grafting (VBG) and non-VBG. We report the use of proximal hamate as a replacement arthroplasty in the cont

All perilunate fracture-dislocations combine ligament ruptures, bone avulsions, and fractures in a variety of clinical forms. The most frequent is the dorsal trans-scaphoid perilunate dislocation. In rare cases, however, these dislocations also have been associated with capitate fractures, triquetral fractures, or lunate fracture. We report a combined scaphoid and lunate fracture of the wrist that was not associated with perilunate dislocation.

PURPOSE: Fractures of the trapezium are rarely diagnosed on plain radiographs after acute wrist trauma. High-resolution cross-sectional imaging identifies fractures of the trapezium as the most common radiographically occult carpal bone fracture. We review the fracture frequency, mechanisms and patterns of trapezium fractures.
METHODS: Cone beam CT was performed in patients with suspected radiographically occult radiocarpal fracture following acute injury. The frequency of carpal bone fractures was assessed and compared.
RESULTS: Ninety-three radiographically occult wrist fractures were iden

PURPOSE: To assess the results of a hybrid Russe procedure using a corticocancellous strut, cancellous autologous nonvascularized bone graft, and cannulated headless compression screw to reduce the deformity reliably from a collapsed scaphoid nonunion, provide osteoinductive stimulus, and stabilize the fracture for predictable union.
METHODS: A hybrid Russe procedure was performed for scaphoid waist fracture nonunions with humpback deformity and no evidence of avascular necrosis. A volar distal radius autologous bone graft was harvested and a strut of cortical bone was fashioned and placed in

PURPOSE: The disability and pain after a neglected scaphoid non-union fracture are well recorded in the literature. We aimed to compare and detect the short-term results of non-united scaphoid waist fracture treated by internal fixation and bone graft with the volar buttress plate utilization versus the Herbert screw.
METHODS: This is a therapeutic study. This randomized, prospective comparative an intervention study was carried out on 30 cases with non-union scaphoid waist fractures. They were randomly categorized into two equal groups, group (A) treated by volar buttress plate fixation with

The scaphoid is the most injured carpal bone, with an incidence of 12 per 100,000 per annum in the United Kingdom (UK). Fractures of the scaphoid can be challenging to diagnose, both clinically and radiographically, and have a propensity to fail to unite or develop necrosis. There is debate regarding whether to manage acute fractures conservatively or offer early surgical fixation. The poor predictive value of initial examination and plain imaging not only results in missed fractures but also, conversely, in a fivefold overtreatment, which has subsequent significant socioeconomic consequences.

A displaced fracture of the scaphoid is one in which the fragments have moved from their anatomical position or there is movement between them when stressed by physiological loads. Displacement is seen in about 20% of fractures of the waist of the scaphoid, as shown by translation, a gap, angulation or rotation. A CT scan in the true longitudinal axis of the scaphoid demonstrates the shape of the bone and displacement of the fracture more accurately than do plain radiographs. Displaced fractures can be treated in a plaster cast, accepting the risk of malunion and nonunion. Surgically the displ

Occult fractures of the scaphoid bone occur frequently and may lead to nonunions. In a prospective blind study, we performed magnetic resonance imaging (MRI) examinations on 32 patients who had sustained a wrist injury and in whom a scaphoid fracture was clinically suspected, but could not be confirmed on the original set of two routine and four scaphoid view radiographs. The MRI examinations were performed an average time of 2.8 days after the trauma. This prospective study proved that MRI is able to diagnose occult scaphoid fractures without delay and without the use of radioactive diagnosti…

OBJECTIVE: Pseudoarthrosis and necrosis of the proximal pole of the scaphoid require complex treatment. If primary treatments fail, there are only techniques that sacrifice part of the mobility. We present the design and in vitro results of an anatomical partial prosthesis of the scaphoid bone.
MATERIAL AND METHOD: The kinematics of the carpus are tested in vitro on six cadaveric forearms before and after prosthetic replacement, applying active loads on the main muscles. Pre- and post-intervention movements are recorded by Kinescan/IBV®, translating an angular value.
RESULTS: After prostheti…

Scaphoid non-union, particularly following internal fixation, is a vexing problem. A retrospective review was conducted analysing the outcome of 4 patients who failed initial open reduction and internal fixation of scaphoid fractures. Three fractures were located in the waist and the fourth in the proximal pole. All patients underwent screw exchange and Bone Morphogenic Protein (BMP)-2 sponge placement with no additional bone grafting. Patients were immobilised for 4 weeks and followed with serial radiographs in all cases and CT scans in 3 cases. All patients demonstrated evidence of bony unio

The management of scaphoid nonunion in heavy manual workers is challenging. Symptoms appear earlier and manual labor-related stress at the operated nonunion site may worsen the postoperative outcome. This study involved 16 heavy manual workers with scaphoid nonunion (11 cases involving the dominant hand) treated by internal fixation and autologous bone grafting. The nonunion was in 12 patients in the waist, in 3 patients in the proximal, and in 1 patient in the distal pole of the scaphoid. Clinical and radiographic evaluation of union was done according to the criteria of Bynum et al. and Fern

Scaphoid nonunion is a common complication of missed or inadequately treated scaphoid fractures and is generally considered an indication for surgical treatment because of the risk of progressive carpal collapse and degenerative arthritis. Spontaneous union of an established scaphoid nonunion is exceptionally rare, particularly in the presence of radiological findings suggestive of proximal pole vascular compromise. We report the case of a 23-year-old right-handed manual worker with a chronic scaphoid nonunion. The patient sustained an initial wrist injury in 2017 that was treated conservative
