[Tissue blood flow of the sigmoid colon for safe anastomosis following ligation of the inferior mesenteric artery].

We examined the tissue blood flow (TBF) of the sigmoid colon to determine which level of inferior mesenteric artery. In 13 patients, TBF of the sigmoid colon was measured in the control state and the clamped state, in which the inferior mesenteric artery (IMA), left colic artery and the rectal were wall clamped. In the clamped state, means of TBF at 10 cm and 15 cm distal to the descend-sigmoid junction were 1.87 +/- 0.77 and 1.10 +/- 0.40, respectively. Mean of TBF at 15 cm was significantly lower than that at 10 cm. In conclusion, TBF which is enough for safe anastomosis reaches as far as 10

Clear Cell Carcinoma of the Left Colon: A Case Report.

BACKGROUND Primary clear cell carcinoma of the colon is an exceptionally rare condition. It is still uncertain whether this carcinoma represents a separate biological category or simply a variant of typical colorectal cancer. From a diagnostic standpoint, the presence of clear cell morphology introduces significant challenges, as similar features can be observed in various other carcinomas, including renal, ovarian, and endometrial carcinoma. Typically, primary colorectal tumors demonstrate positivity for markers such as CK20 and CDX2, supporting intestinal origin. CASE REPORT We present a cas

Preserving the left colonic artery in radical sigmoid and rectal cancer surgery is feasible: A meta-analysis.

BACKGROUND: This study aims to investigate the safety and feasibility of preserving left colonic artery (LCA) in radical sigmoid and rectal cancer surgery. METHODS: Relevant articles were systematically searched on the PubMed, Embase, and Cochrane Library. The quality of included studies was evaluated using the Cochrane Handbook. A meta-analysis was conducted to assess the surgical outcomes and oncological outcomes by RevMan 5.4 software. RESULTS: Fifteen studies with a total of 5054 patients, including 2432 patients with LCA preservation and 2622 patients without LCA preservation, were incl

The double challenge (preoperative diagnosis and surgical approach) of primary leiomyosarcoma of the sigmoid colon.

BACKGROUND AND AIM: Primary colonic leiomyosarcoma (cLMS) is a rare malignancy of muscularis mucosae or muscularis propria showing highly aggressive behaviour and poor prognosis. To date, making a preoperative diagnosis and performing the most appropriate treatment represent laborious tasks for the clinicians. On the one hand, in fact, cLMS diagnosis is often difficult to achieve preoperatively because of the low specificity of clinical, radiological and bioptical features: for these motives, the diagnosis is usually obtained at postoperative histology/immune-histochemistry. On the other hand,

The Extraperitoneal Approach to Left-Sided Colorectal Resections: A Human Cadaveric Study.

INTRODUCTION: Technical challenges during laparoscopic and robotic anterior resection include identification of key retroperitoneal structures and obtaining clear views of the inferior mesenteric artery (IMA) pedicle and total mesorectal excision (TME) plane. Steep head-down position improves surgical exposure but is associated with cerebral oedema, high intrapulmonary pressures, and rare neurological complications. In this article we describe the key steps of an anterior resection performed via the extra-peritoneal (XP) space in the supine position. METHODS: The technique of same-side latera

Endometriosis-associated intestinal tumors: a new challenge in clinical practice.

A 66-year-old female consulted due to headache associated with diaphoresis, constipation, and melena. A urinalysis revealed elevated metanephrines, and colonoscopy identified an extrinsic sigmoid stricture. CT and MRI showed a presacral mass infiltrating the sigma. Scintigraphy and SPECT ruled out a diagnosis of paraganglioma. Based on these findings, the patient was proposed to undergo surgery.

[A Case of Elective Laparoscopic Resection for Sigmoid Colon Cancer with Irreducible Intussusception].

An 85-year-old man was hospitalized for a right greater trochanteric fracture. Rectal intussusception was found by diagnostic imaging but left untreated because of minor gastrointestinal symptoms. As a result of work-up for persistent mucous stool, he was diagnosed with sigmoid colon cancer with intussusception. The intussusception could not be reduced during barium enema examination but could undergo elective laparoscopic surgery with a good postoperative course. Adult intussusception may be asymptomatic and require no emergency treatment. In such a case, elective surgery can be performed. Ma

Definition of the Rectum: An International, Expert-based Delphi Consensus.

BACKGROUND: The wide global variation in the definition of the rectum has led to significant inconsistencies in trial recruitment, clinical management, and outcomes. Surgical technique and use of preoperative treatment for a cancer of the rectum and sigmoid colon are radically different and dependent on the local definitions employed by the clinical team. A consensus definition of the rectum is needed to standardise treatment. METHODS: The consensus was conducted using the Delphi technique with multidisciplinary colorectal experts from October, 2017 to April, 2018. RESULTS: Eleven different

Laparoscopic sigmoid colectomy for a patient with sigmoid colon cancer and crossed-fused renal ectopia: a case report.

Crossed-fused renal ectopia (CFRE) is a very rare congenital renal malformation. This condition comprises several anatomic anomalies, including unilateral ureteral intersection of the midline, anteriorly-placed renal pelvises, and aberrant renal blood vessels, all of which increase the difficulty of colectomy. This report describes a case of laparoscopic sigmoidectomy with sufficient lymphadenectomy for a patient with sigmoid colon cancer and left-sided L-shaped CFRE. Preoperative computed tomography demonstrated that the origin of the inferior mesenteric artery (IMA) was free from anomalies a

High serum levels of both carcinoembryonic antigen and carbohydrate antigen 19-9 in a patient with sigmoid colon cancer without metastasis.

Carcinoembryonic antigen (CEA) and carbohydrate antigen (CA)19-9 are well known as the most common tumor markers of colon cancer, and levels are used not only for preoperative assessment of extent and outcome of cancer, but also postoperative monitoring of recurrence. We encountered a patient with sigmoid colon cancer showing abnormally high serum levels of CEA (311.1 ng/ml) and CA19-9 (5731.2 U/ml) preoperatively. We could not detect any metastases on computed tomography (CT) or (18)F-fluorodeoxyglucose positron emission tomography/CT. Sigmoidectomy and lymph node dissection were performed. P

[A case of mucosal Schwann cell hamartoma].

A 40-year-old man was referred to our hospital because of a positive fecal occult blood test. Colonoscopy revealed many small whitish nodules in the mucosa of the sigmoid colon. Specimens endoscopically resected from the lesions revealed spindle cell proliferation in the lamina propria. Immunohistochemical study revealed strong and diffuse positivity for S-100 protein. Results of staining for neurofilament protein and epithelial membrane antigen were negative. The neurogenic tumors were diagnosed as mucosal Schwann cell hamartoma. No clinical features of multiple endocrine neoplasia type 2B or

A two-step laparoscopy-assisted curative resection for a patient with obstructive colitis accompanied by advanced sigmoid colon carcinoma: Report of a case.

This report presents the case of a two-step laparoscopic resection and reconstruction for obstructive colitis accompanied by advanced sigmoid colon cancer. An 81-year-old woman was admitted with a diagnosis of ileus. Computed tomography revealed a circumferential tumor in the sigmoid colon and a diffuse dilated large intestine on the oral side of the tumor. On the 7th day after admission, her temperature was 38.8°C, she had increased white blood cell count (24 610 cells/mm(3)), and suffered persistent severe abdominal pain. An emergency laparoscopy-assisted Hartmann procedure was performed, ba

[Non Hodgkin lymphoma of the sigmoid colon: case report].

Non-Hodgkin lymphoma (NHL) occurs in extranodal location in approximately 20% of patients with limited stage, high-grade disease. Colon is infrequently involved as a primary location, accounting for 4% of all extranodal NHL and far less than 1% of all colonic malignancies. Colonic NHL differs significantly in terms of presentation, therapy and outcome relative to other more common gastrointestinal sites, like stomach or small bowel. The most common location is the cecum (60-74% of cases), while the sigmoid is involved in 2.5-14%. Largely related to the lack of specific signs and symptoms, pati
