The dynamics of recovery from impaired decision-making after total sleep deprivation (TSD) are not well understood. We investigated the impact of TSD and subsequent 3 h sleep recovery night period on decision-making. After one baseline night (8 h Time in Bed [TIB], 23:00-07:00), 40 study participants (50 per cent women) followed a sleep deprivation protocol including one night of TSD (40 h continuous awakening), followed by 3 h TIB sleep recovery period (23:00-02:00) and 8 h TIB sleep recovery period. Indices derived from reaction time (RT), Go/No-Go, and complex Go/No-Go tasks (involving perc…

Isolated REM sleep behavior disorder (iRBD) is a key prodromal marker of Parkinson's disease (PD). Video-polysomnography (vPSG) remains the diagnostic gold standard, but manual sleep staging is particularly time-consuming and challenging in neurodegenerative disease. We adapted U-Sleep, a deep neural network, for automated sleep staging in PD and iRBD. A pretrained model (PUB, 19,236 PSGs), was finetuned on multicenter datasets (PACE, CBC: 112 PD, 138 iRBD, 89 controls) and evaluated on a clinical hold-out (DCSM: 81 PD, 36 iRBD, 87 controls). Predictors of staging agreement were analyzed, and …

The United States continues to face a mental health crisis. A strong body of evidence supports poor sleep as a significant contributor to the development and exacerbation of mental health conditions. To achieve public health goals, it is critical to understand the associations among sleep health characteristics and mental health in the general population, and how practicing healthy sleep behaviors can promote better mental health across communities. National Sleep Foundation calls for focused actions to promote sleep health and mental health from a wide range of society members. Efforts to inc…

BACKGROUND: Obstructive sleep apnea (OSA) and sleep bruxism (SB) are both sleep-related conditions that may affect sleep architecture and potentially contribute to daytime sleepiness (DS). AIM: This study aimed to explore the association between SB and DS as measured by Epworth Sleepiness Scale in patients with OSA. METHODS: A cross-sectional study was conducted involving 345 patients diagnosed with OSA. The presence of SB was assessed through multidisciplinary clinical examination and audio-video recordings during polysomnography. DS was measured using the Epworth Sleepiness Scale (ESS). Demo…

The correct identification of respiratory events and estimation of total sleep time (TST), done in a sleep clinic with polysomnography (PSG), are central elements in the diagnostic process of sleep-disordered breathing and in the quantification of its severity with the apnoea-hypopnea index (AHI). However, total sleep time may not be available in home-based diagnostic procedures with polygraphy (PG), so the respiratory event index (REI), based on total recording time (TRT), is used in place of AHI. Because of that, severity can be greatly under-estimated in people with high sleep fragmentation…

Understanding the neural mechanisms that facilitate the transition from wakefulness to sleep is a fundamental goal in neuroscience, yet the neurochemical dynamics underlying human sleep onset remain poorly understood. Here, we investigated whether changes in inhibitory γ-aminobutyric acid (GABA) and excitatory glutamate plus glutamine (Glx) concentrations in the medial prefrontal cortex (mPFC) are associated with sleep initiation. We leveraged the first-night effect, a transient sleep disruption induced by a novel environment, to manipulate sleep quality within participants. Twenty healthy you…

PURPOSE: Polysomnography (PSG) defines sleep stage, respiratory event type, and respiratory effort, but does not directly identify the visible anatomic correlate of individual events. We examined whether concurrent natural sleep endoscopy (NSE) with PSG could support event-level interpretation of selected sleep-disordered breathing events.
METHODS: Four adults were selected from archived synchronized NSE-PSG recordings acquired between 2007 and 2009. Cases were chosen to illustrate interpretive scenarios in which PSG context changed, refined, or constrained visible airway interpretation.

Children with neurodevelopmental disabilities are at high risk for sleep problems, which can negatively affect their health and that of their families. Improving sleep may be one of the most effective ways to improve behaviour, mood, positive social interaction, attention and learning, and reduce future risks for poor metabolic and mental health. While insomnia is the most common concern, increased rates of other sleep disorders are also found in this population. Sleep problems require prompt identification and intervention, which includes recognizing or ruling out contributory medical conditi…

OBJECTIVE: To develop and validate the Italian version of the Quebec Sleep Questionnaire (I-QSQ) for assessing quality of life (QoL) in patients with obstructive sleep apnoea (OSA).
METHODS: The questionnaire was translated, culturally adapted, and back-translated. Internal consistency and test-retest reliability were evaluated in patients with moderate OSA. Normative values were obtained in healthy subjects. Construct and criterion validity were assessed by comparing scores of patients and controls, and by correlating I-QSQ with the Epworth Sleepiness Scale. Responsiveness was examined in pa…

OBJECTIVE: The apnea-hypopnea index (AHI) defines obstructive sleep apnea (OSA) severity but fails to describe nuances in disease burden. The modified sleep apnea severity index (mSASI) combines patient anatomy, weight, sleep study metrics, and symptoms to provide a composite OSA index ranging from 1 to 3. While prior studies have associated mSASI with quality of life and hypertension, its utility in continuous positive pressure intolerant (CPAPi) surgical patients remains unexplored.
METHODS: A retrospective cohort study of surgically treated CPAPi patients who underwent upper airway stimula…

Obstructive sleep apnea (OSA) is a well-established risk factor for hypertension, with sleep duration being a modifiable factor influencing this risk. However, sleep misperception among OSA patients makes it unclear how subjective and objective sleep duration are associated with the prevalence and incidence of hypertension in this population. This study aims to examine these associations using data from the Sleep Heart Health Study cohort. Participants with OSA (apnea-hypopnea index ≥ 15 events/hour) were categorized based on objective sleep duration from polysomnography (PSGTST) and subjectiv…

OBJECTIVE: Dravet syndrome (DS) is a developmental and epileptic encephalopathy with a wide spectrum of comorbidities comprising sleep disorders, reported in up to 85% of cases. For this, a sleep study is recommended in patients with a sleep complaint. However, no data are available on sleep architecture in DS or on the impact of seizures on sleep quality and macrostructure. We aim to investigate the impact of epileptic phenomena on sleep in DS.
METHODS: In this study, we report seizure type and frequency, seizures during sleep, and concomitant antiseizure medications (ASMs) of 30 patients wi…

INTRODUCTION: Obstructive sleep apnea (OSA) is linked to increased cardiovascular and metabolic risks. Although body mass index is a recognized risk factor, the roles of fat mass, muscle mass, and hydration status in influencing OSA severity and sleep architecture remain unclear. This study examined the associations between body composition and apnea-hypopnea index, sleep efficiency, and sleep stages across sex, age, and BMI strata.
MATERIALS AND METHODS: In this cross-sectional study, data from 5,381 adults undergoing overnight polysomnography and bioimpedance analysis were analyzed. Multiva…

Sleep staging is a critical task for sleep health assessment. Recent work has shown that breathing variability may be characteristic of particular sleep stages. In this work, we train machine learning models on sleep airflow signals from pediatric patients to assess how well sleep stage can be classified from airflow signals, particularly in subjects with sleep-disordered breathing. We perform a 3-class classification task (Wake/NREM/REM) and achieve 66.9% balanced accuracy (BA) when classifying the sleep stage of healthy subjects, 61.4% BA when classifying the sleep stage of subjects with mil…

BACKGROUND: Sleep is a fundamental determinant of health, yet little is known about sleep in carceral environments where structural, environmental, and psychosocial factors uniquely disrupt rest. Incarcerated individuals face disproportionately high burdens of illness, with sleep disturbances contributing to these disparities. Understanding the evidence on sleep in incarceration is essential for informing care, policy, and research.
METHODS: We conducted a scoping review of peer-reviewed literature on sleep in incarcerated populations, following the Joanna Briggs Institute Manual and PRISMA-S…
