Epstein-Barr virus-associated epithelial and mesenchymal neoplasms.

Epstein-Barr virus (EBV) is a ubiquitous human pathogen that usually maintains a harmonious relationship with its host. Rarely, this host-virus balance is perturbed, causing a diverse group of malignancies in both immunocompetent and immunosuppressed patients. In addition to its role in hematologic malignancies (Burkitt lymphoma, subsets of Hodgkin and T-cell lymphomas, posttransplant lymphomas), EBV has been implicated in both epithelial (undifferentiated nasopharyngeal carcinoma, a subset of gastric adenocarcinomas) and mesenchymal (EBV-associated smooth muscle tumor, inflammatory pseudotumo…

Gastric leiomyoblastoma: report of three cases.

Leiomyoblastoma is a rare smooth muscle tumor characterized by epithelioid cells with clear cytoplasms and an unknown biological behaviour. Since pre-operative diagnosis is difficult, the optimum strategy during the operation could be determined only by having a thorough knowledge about it beforehand. Leiomyoblastoma can be exogastric, intramural or endogastric. In the mostly benign exogastric leiomyoblastomas, total excision with resection of full thickness of gastric wall around the tumor is appropriate. Partial or total gastrectomy should be performed for intramural or endogastric tumors. W…

Ectomesenchymal hamartoma (benign "ectomesenchymoma") of the VIIIth nerve: case report.

We report a previously undescribed hamartoma of the VIIIth nerve, consisting of adipose tissue, Schwann cells associated with myelinated nerve fibers, well-differentiated smooth and striated muscle fibers, and rare ganglion cells. The tumor was found in a 35-year-old Caucasian female who presented with right-sided hearing loss. The mass, which we designate an "ectomesenchymal" hamartoma, most likely developed from pluripotent neural crest cells ("ectomesenchyme"), which are capable of differentiating into a variety of neuroectodermal and mesenchymal cell types. The development of the neural cr

[A case of esophageal cyst resected by thoracoscopic surgery].

A 59-year-old woman was admitted with back pain and an abnormal chest shadow in the left hilar region. Her chest CT scan showed a posterior mediastinal mass surrounding the descending aorta. By transesophageal ultrasonography, a diagnosis of mediastinal cyst was made. She underwent thoracoscopic resection under general anesthesia with new endoscopic devices including endo-scissors, endoscopic hemoclips and electrocautery. Pathologically, the tumor was an esophageal cyst lined by ciliated columnar and squamous epithelium overlying smooth muscle layers. She made an uneventful recovery. Her back

[A case of inflammatory myofibroblastic tumor with rapidly progressive growth arising from the retroperitoneum].

A woman in her 70s was admitted to our institution with complaints of right hypochondrium pain. Abdominal computed tomography revealed a 13-mm retroperitoneal tumor between the liver and right kidney. The tumor rapidly increased to 82mm within 2 months, a necrotic change was inside the tumor, and the inflammation spread to the surrounding diaphragm and the peritoneum. The patient underwent surgical resection including the affected diaphragm and the peritoneum. Histopathological examination revealed a myofibroblastic spindle-cell proliferation with prominent infiltration of inflammatory cells,

Epithelioid Glomus Tumor of the Uterine Cervix: A Case Report and Review.

Herein, we report a case of epithelioid glomus tumor involving the uterine cervix. A 67-yr-old woman with a long-standing history of cervical dysplasia underwent cervical conization. In addition to the patient's high-grade squamous intraepithelial lesion, histologic examination demonstrated an incidental, 0.2-cm glomus tumor in the cervical submucosa. The tumor was composed of bland epithelioid cells in scattered nests closely associated with small-caliber blood vessels. Immunohistochemically, the tumor cells were diffusely positive for smooth muscle actin and caldesmon and only focally positi

Primary Leiomyosarcoma of the Breast: A Successful Surgical Treatment in a 45-Year-Old Woman.

BACKGROUND Spindle cell carcinomas and sarcomas are 2 uncommon primary spindle cell malignant tumors of the breast. Histopathology and immunohistochemistry for cell phenotypic markers can identify the type of tumor. This report is of a 45-year-old woman with a primary leiomyosarcoma of the breast, successfully treated by surgical excision. CASE REPORT A 45-year-old woman presented to our clinic in February 2018 with a 10.5-cm tumor in the right breast with lateral invasion of the pectoralis minor and pectoralis major. When the tumor first appeared in 2016, it was 5 cm in size and was diagnosed

Leiomyoma of the renal vein: a rare tumor presenting as a renal mass.

Leiomyomas are benign mesenchymal tumors that rarely occur in the kidney. Renal leiomyomas usually occur in the renal cortex or capsule. They are less commonly found in the muscularis propria of the renal pelvis and cortical vascular smooth muscle. In this case report, we present a 41-year-old woman who had right flank pain and detected a mass in the right kidney hilum.

Cystically degenerated leiomyoma of the rectosigmoid managed laparoscopically at 13 weeks of gestation.

The safety of laparoscopic management of adnexal masses in pregnancy has been documented. Herein we report laparoscopic removal during pregnancy of a cystically degenerated leiomyoma of the sigmoid colon, which had been mistaken for an adnexal mass. When smooth muscle gastrointestinal tumors are observed, it is important that they be characterized with appropriate markers so that postoperative treatment can be individualized to the patient.

Leiomyosarcoma of the transverse colon.

A case report of leiomyosarcoma of the transverse colon was presented in view of its rarity of the occurrence. An extra-colonic, mobile abdominal mass showing a rapid growth with mild tenderness was the principal preoperative findings in the patient. The occurrence of malignant tumors in smooth muscles of the colon, not including the rectum, is extremely rare. In review of the references, about thirty cases have been previously reported.

Nodular hidradenoma of the caruncle.

An 84-year-old man presented with a tumor of the right caruncle causing epiphora due to insufficient eyelid closure. The tumor was cystic and mucinous in appearance. MRI showed a solitary tumor of the caruncle with no adhesions to neighboring tissue. The mass was removed by carunculectomy. Histopathologic examination revealed a tumor composed of epithelial cells with intervening strands of hyalinized stroma. Solid areas were predominant but a cribriform area could also be distinguished. Periodic acid-Schiff positive, diastase-labile material (glycogen) was detected in many tumor cells. Immunoh

Myoid gonadal stromal tumor: A rare testicular sex cord stromal neoplasm - a case report and review of the literature.

Myoid gonadal stromal tumor (MGST) is an exceptionally rare benign testicular sex cord-stromal neoplasm with limited published experience. We report a 30-year-old man presenting with a painless right testicular mass and normal serum tumor markers. Ultrasonography and magnetic resonance imaging demonstrated a small enhancing intratesticular lesion suspicious for malignancy. Radical inguinal orchiectomy was performed. Histopathological and immunohistochemical evaluation was consistent with MGST, showing spindle-cell proliferation with diffuse smooth muscle actin, S100, and vimentin positivity an

Pulmonary Leiomyosarcoma: A Diagnostic Challenge.

Primary pulmonary leiomyosarcoma (PPL) is an extremely rare form of lung cancer, accounting for only a small fraction of all lung cancer cases. We describe a middle-aged woman who came to the hospital with cough and breathlessness, and imaging revealed multiple lung masses. A biopsy showed spindle-shaped tumor cells positive for smooth muscle markers, confirming leiomyosarcoma. She was treated with combination chemotherapy followed by oral pazopanib, showing good clinical and radiological improvement. This case highlights how rare and difficult to diagnose this tumor can be and the importance

Intranodal palisaded myofibroblastoma: a review of the literature.

Intranodal palisaded myofibroblastoma is a rare benign primary mesenchymal neoplasm originating from differentiated smooth muscle cells and myofibroblasts. The precise etiology and pathogenesis has not been adequately explained as yet. Very few series and cases have been reported in the literature. Though inguinal region is the commonest site of this rare tumor, but the tumor at other diverse sites have been reported. Because of its rarity, it can be often misdiagnosed and confused with other disorders and more commonly with metastasis. We report an extensive review of literature about intrano

F-18 FDG PET/CT findings of primary sinonasal hemangiopericytoma: rare location in a young adult patient.

Hemangiopericytoma is an unusual tumor that arises from vascular smooth muscle cells. It is mainly experienced in 4th to 5th decades of life and soft-tissue masses are frequently located in lower extremity, pelvis, and retroperitoneum. A 29-year-old man presented with a mass lesion in the right nasal cavity associated with shortness of breath as a result of nasal obstruction. An incisional biopsy was reported as sinonasal type hemangiopericytoma. The initial staging F-18 flourodeoxyglucose PET/CT successfully demonstrated severe hypermetabolism in the primary tumor location and mild-to-moderat

Leiomyoma of Meckel's diverticulum associated with adenocarcinoma of the descending colon.

A case of leiomyoma of Meckel's diverticulum associated with a carcinoma of the descending colon, presenting with intestinal obstruction, is described. Only two cases of smooth muscle tumors of the small intestine associated with carcinoma of the large bowel have been described so far, and ours is probably the first case of its kind to be reported. The importance of angiography and a through intraoperative exploration is stressed.

[Operation for Intracardiac Angiomyolipoma;Report of a Case].

Primary tumors of the heart are rare. In the literature operations for heart tumors account for 0.3% of all cardiac surgical procedures. We report a very rare case of primary cardiac angiomyolipoma. A 39-years-old man was admitted with chest discomfort. A mass was detected in the left ventricular myocardium by enhanced chest computed tomography. We performed resection of the intramyocardial tumor under cardiopulmonary bypass. Pathologic examination revealed a 4×3×3 cm, dome-shaped, solid tumor composed of a mixture of blood vessels, smooth muscle, and fat tissue. Histopathological findings sho

Left Atrial Primary Leiomyosarcoma Presenting With Complete Right Pulmonary Vein Thrombosis in a Young Male Patient: A Case Report and Literature Review.

Primary cardiac leiomyosarcomas are extremely uncommon, constituting only a very small fraction of all primary cardiac tumors and a minor subset of cardiac sarcomas. Nearly half occur in the left atrium due to smooth muscle elements in the pulmonary venous junction. Their nonspecific presentation, often mimicking atrial myxoma or thrombus, delays diagnosis and treatment. A 19-year-old male patient presented with recurrent hemoptysis and exertional dyspnea. Initial workup suggested pneumonia; however, contrast-enhanced computed tomography (CT) and echocardiography revealed a large, heterogeneou
