Disruption of hippocampal synaptic plasticity by chronic ethanol exposure: A narrative review of neurotoxic adverse outcome pathways.

Chronic is a major neurotoxicant that disrupts hippocampal synaptic plasticity, leading to persistent cognitive deficits. This narrative review maps the adverse outcome pathways (AOPs) through which ethanol impairs synaptic function, primarily via interconnected cascades: TLR4/NF-κB-mediated neuroinflammation (triggering microglial activation and pro-inflammatory cytokines TNF-α, IL-1β), CYP2E1-driven oxidative stress (generating ROS/RNS, 4-HNE, causing protein carbonylation and mitochondrial dysfunction), and glutamate excitotoxicity (mediated by GluN2B-NMDAR subunit shifts, Ca²⁺ overload, an

AI-driven CRISPR strategies in breast cancer: Organoid modeling, adaptive editing, and precision delivery.

Triple-negative breast cancer (TNBC) is defined by profound heterogeneity, dormant metastatic reservoirs, and rapid therapy resistance. Building on our AI-Driven CRISPR Strategies in Breast Cancer framework, CRISPR-Cas9 is emerging as more than a gene-editing tool, capable of restoring circadian integrity, eliminating dormant clones, and re-programming immune surveillance. A structured PubMed, Scopus, and ClinicalTrials.gov review through 2025 integrated mechanistic, preclinical, and early clinical evidence. Beyond standard knockout, base, and prime editing, we highlight chrono-genomic repair

Instrumental variable approach to estimating individual causal effects in N-of-1 trials: application to ISTOP study.

An N-of-1 trial is a multiple crossover trial conducted in a single individual to provide evidence to directly inform personalized treatment decisions. Advances in wearable devices greatly improved the feasibility of adopting these trials to identify optimal individual treatment plans, particularly when treatments differ among individuals and responses are highly heterogeneous. Our work was motivated by the I-STOP-AFib Study, which examined the impact of different triggers on atrial fibrillation (AF) occurrence. We described a causal framework for "N-of-1" trial using potential treatment selec

Role of upper airway evaluation in the multidisciplinary management of obstructive sleep apnea in children below two years of age.

BACKGROUND: Diagnosis and treatment of obstructive sleep apnea (OSA) in infants and young children is challenging because of its clinical heterogeneity and lack of age-specific guidelines. AIM: We report the management and treatment outcome of OSA in children below 2 years of age. Treatment decisions were based upon the pattern of upper airway (UA) obstruction, clinical presentation and OSA severity. METHODS: Retrospective, non-randomized observational cohort study at a tertiary center. Children with OSA who underwent an UA evaluation (drug-induced sleep endoscopy or direct laryngoscopy) wer

Effect of Ester Moiety on Structural Properties of Binary Mixed Monolayers of Alpha-Tocopherol Derivatives with DPPC.

Phospholipid membranes are ubiquitous components of cells involved in physiological processes; thus, knowledge regarding their interactions with other molecules, including tocopherol ester derivatives, is of great importance. The surface pressure-area isotherms of pure α-tocopherol (Toc) and its derivatives (oxalate (OT), malonate (MT), succinate (ST), and carbo analog (CT)) were studied in Langmuir monolayers in order to evaluate phase formation, compressibility, packing, and ordering. The isotherms and compressibility results indicate that, under pressure, the ester derivatives and CT are ab

Scaphoid waist nonunion with humpback deformity treated without structural bone graft.

PURPOSE: Conventional treatment of scaphoid nonunion with collapse deformity and bone loss involves the use of a structural corticocancellous bone graft. With internal screw fixation, we propose that length and alignment can be restored and maintained using only cancellous interposition graft. METHODS: We describe 12 patients with established scaphoid waist nonunions. Average patient age was 22 years, all were male, and 8 cases involved the dominant wrist. Time from injury to treatment averaged 11 months. We performed open reduction through an anterior approach with correction of the deformit

Late diagnosis of pedicle screw malplacement with perforation of the thoracic aorta after posterior stabilization in a patient with multiple myeloma: case report.

STUDY DESIGN: A case report. OBJECTIVE: To demonstrate delayed diagnosis of screw malpositioning with perforation of the thoracic aorta after posterior stabilization of a Th7-vertebral collapse due to multiple myeloma. Relevant diagnostic and therapeutic strategies are outlined in the context of a rather unfortunate series of interventional events. SUMMARY OF BACKGROUND DATA: Pedicle screw instrumentation has become a well-established standard in the surgical treatment of various disorders of the spinal column. Particularly at the upper-thoracic level, the close anatomic relationship of the

Cross-domain microbiome legacy of a natural Phaeocystis globosa bloom in eutrophic coastal waters: a framework for monitoring bloom onset, collapse, and post-bloom recovery.

When a harmful algal bloom collapses, does the coastal water quality recover? We addressed this question using the natural bloom of the haptophyte Phaeocystis globosa in coastal waters around Xiamen Island, China. Bacterial and microeukaryotic communities in the water samples (n = 105) monthly collected from five sites from September 2023 to March 2024 were analyzed using 16S and 18S rRNA amplicon sequencing, genotype-specific qPCR, iCAMP-based assembly inference, partial least squares path modeling, bloom stage-specific co-occurrence networks, and amplicon-based functional prediction. Deep learning in the diagnosis, staging, and prognosis of osteonecrosis of the femoral head: a systematic review.

BACKGROUND: Osteonecrosis of the femoral head (ONFH) is a progressive ischemic bone disorder that often leads to structural collapse and early arthroplasty in adults. Early detection and accurate staging are essential for joint-preserving management. Deep learning (DL) has shown high promise in musculoskeletal imaging; however, the methodological quality and How well these ONFH-related DL models translate to clinical practice remain unclear. This systematic review synthesized current evidence on DL applications for ONFH diagnosis, staging, segmentation, and prognosis across imaging modalities,

Magnetically assisted trapping of passive colloids by active dipolar chain.

We investigate a trapping mechanism for passive Brownian particles based on mixtures with self-propelled dipolar colloids. Active dipoles, whose magnetic moment is oriented perpendicularly to their propulsion direction, spontaneously form dynamic chains that collapse into clusters through dipole-dipole interactions. These transient structures efficiently capture nearby passive particles, forming dense phases at relatively low global packing fractions. Using Brownian dynamics simulations, we analyze how the capture efficiency depends on the Péclet number (Pe) and dipolar interaction strength (λ

Stem cell therapies for biological repair in osteonecrosis: advances, challenges, and clinical insights.

Osteonecrosis is a bone condition that begins as an ischemic process and can progress to subchondral collapse and premature joint degeneration in untreated cases. Conventional treatments offer physical or symptomatic relief but do not address the underlying physiological deficiencies. Regenerative therapeutic techniques based on stem cells have been developed to stimulate osteogenesis, vascular therapeutic, and immunomodulation in bone defects. Existing clinical applications are based on mesenchymal stem cells (MSCs) derived from the bone marrow, peripheral blood, and adipose tissue, as well a

Giant Mediastinal Teratoma in a Child: A Case Report With a Brief Review of the Literature.

Giant mediastinal teratomas in the pediatric population are uncommon entities that may grow insidiously and cause significant compression of intrathoracic structures. We report the case of a 10-year-old girl with a two-year history of progressive exertional dyspnea, chest pain, orthopnea, and increasing thoracic enlargement. Chest radiography demonstrated marked mediastinal widening, and contrast-enhanced chest CT revealed a predominantly cystic mediastinal mass measuring approximately 15 × 14 cm, with mediastinal shift and ipsilateral lung collapse. Routine laboratory tests were within normal

Tensioning diaphragmoplasty for treating bilateral phrenic nerve paralysis in a dog.

A spayed female Miniature dachshund presented with a large mediastinal mass. During thymectomy, the left phrenic nerve, which was encircled by the neoplastic tissue, was resected en bloc with the tumour and the right phrenic nerve was damaged by thermal coagulation from electrosurgery. Postoperatively, the dog exhibited significant respiratory distress and paradoxical respiratory movement. Bilateral diaphragmatic paralysis was confirmed on inspiratory radiographs, and lack of structural abnormalities on thoracic computed tomography was noted. The dog collapsed 5 days postoperatively from respi

Cholera in North Kivu: impacts of armed conflict on the resurgence of the epidemic, a narrative review.

Cholera remains a major public health threat in the Democratic Republic of Congo, particularly in North Kivu, a province marked by chronic armed conflict and humanitarian crises. This review explores how insecurity, mass displacement, and the collapse of health and sanitation systems have contributed to the resurgence of cholera epidemics. Recurrent armed clashes have not only weakened local health structures but also created living conditions that facilitate the spread of waterborne diseases. Efforts by health authorities and humanitarian organizations have included emergency case management,

iNOS is necessary for GBP-mediated T. gondii clearance in murine macrophages via vacuole nitration and intravacuolar network collapse.

Toxoplasma gondii is an obligate intracellular parasite of rodents and humans. Interferon-inducible guanylate binding proteins (GBPs) are mediators of T. gondii clearance, however, this mechanism is incomplete. Here, using automated spatially targeted optical micro proteomics we demonstrate that inducible nitric oxide synthetase (iNOS) is highly enriched at GBP2+ parasitophorous vacuoles (PV) in murine macrophages. iNOS expression in macrophages is necessary to limit T. gondii load in vivo and in vitro. Although iNOS activity is dispensable for GBP2 recruitment and PV membrane ruffling; parasi

Acquired Pes Planovalgus : Current Concepts - "From Adult Acquired Pes Planovalgus to Progressive Collapsing Foot Deformity".

The clinical disorder traditionally known as pes planovalgus due to posterior tibial tendon insufficiency or adult-acquired pes planovalgus has been the subject of several publications over the past two decades. Now, it is understood that the problem does not lie in the posterior tibial tendon per se and may even occur without tendon injury. Studies have brought new concepts and understanding that question the views on this subject, culminating in the replacement of existing classifications with one that is more assertive and discriminative of the potential presentation patterns of the deformi

Digital technology-assisted minimally invasive removal of odontoma with high impacted maxillary canine by double-fenestration technique: a case report.

Odontoma is a hamartoma or developmental malformation of dental tissues and often causes delayed or impacted permanent teeth. This paper reports a case of surgical removal of an odontoma that is closely attached to the labial bone plait and examines the extraction of accompanying high-impacted maxillary canine beside the nose and maxillary sinus. Digital technology was used in reconstructing a part of a patient's maxilla, odontoma, related teeth, and other adjacent anatomical structures, accurately determine the lesion boundary, and design an optimal surgical plan. The minimally invasive extra

CCDC183 is essential for cytoplasmic invagination around the flagellum during spermiogenesis and male fertility.

Sperm flagellum plays a crucial role in male fertility. Here, we generated Ccdc183 knockout mice using the CRISPR/Cas9 system to reveal the protein function of the testis-specific protein CCDC183 in spermiogenesis. We demonstrated that the absence of CCDC183 causes male infertility with morphological and motility defects in spermatozoa. Owing to the lack of CCDC183, centrioles after elongation of axonemal microtubules do not connect the cell surface and nucleus during spermiogenesis, which causes subsequent loss of cytoplasmic invagination around the flagellum. As a result, the flagellar compa

Hip Preservation With Autologous Osteoblast Cell-Based Treatment in Osteonecrosis of the Femoral Head.

Osteonecrosis of the femoral head is a progressive disease that leads to femoral head collapse and secondary osteoarthritis if left untreated. Head preservation surgeries are notable for their inefficiency in providing a pain-free hip joint in cases with extensive involvement of the femoral head. This single-center study evaluated the effectiveness of autologous cultured osteoblast cells implanted after core decompression and debridement in 15 patients diagnosed with early osteonecrosis of the femoral head from 2010 to 2012. Overall mean follow-up was 51 months; the longest follow-up was 7 yea

Giant Mediastinal Thymic Cyst.

The authors present a rare case of giant mediastinal cyst which arises from the thymus gland, and goes down in both pleural spaces, especially in the right chest cavity where a dominant part of the cyst was present. The cyst was full with 2.5 liters of transparent fluid, and compressed surrounding structures - heart and both lungs, especially the right one which was partially collapsed. The patient was a 52 years old woman, without any clinical symptoms. Accidentally, on the screened chest X-ray a shading in the distal third of the right chest was detected. The case was well documented with a
