Abuse and misuse of antidepressants.

BACKGROUND: Rates of prescription drug abuse have reached epidemic proportions. Large-scale epidemiologic surveys of this under-recognized clinical problem have not included antidepressants despite their contribution to morbidity and mortality. The purpose of this review is to look specifically at the misuse of antidepressants and how this behavior may fit into the growing crisis of nonmedical use of prescription drugs. METHODS: We conducted a comprehensive search on PubMed, Medline, and PsycINFO using the search terms "antidepressant", "abuse", "misuse", "nonmedical use", "dependence", and "

Integrating forensic pathology and toxicology for in-depth analysis of performance-enhancing substance misuse: Insights into injury, death, and broader implications.

The death of a non-professional bodybuilder with a history of performance-enhancing drug (PED) use was thoroughly investigated. At the decedent's residence, investigators found four boxes of methylprednisolone tablets, one galenic preparation of sibutramine, and two unlabelled galenic compounds. Blood and hair samples were collected during autopsy. Toxicological analysis revealed the presence of methylprednisolone (19 ng/mL), methandrostenolone (27 ng/mL), and sibutramine (29 ng/mL) in blood. Hair analysis confirmed sibutramine (80 pg/mg) and methandrostenolone (1.28 ng/mg). GC-MS analysis of

Prevalence of drugs of abuse found in testing of illicit drug seizures and urinalysis of selected population in Hong Kong.

The study investigates the prevalence of drugs of abuse detected from 2011 to 2015 through (i) forensic drug testing of illicit drug seizures from law enforcement agencies; and (ii) analysis of common drugs of abuse in urine samples obtained from offenders/probationers under mandatory drug-use surveillance programmes. Under the selected drug testing groups, there were an average of 5334 cases/year of illicit drug seizures examined and 28,438 samples/year requiring drugs of abuse analysis in urine, from 2011 to 2015. The drug positive rates for urinalysis in the selected population group (i.e.,

Alcohol abuse, other drug abuse, and mental disorders in medical practice. Prevalence, costs, recognition, and treatment.

Millions of Americans suffer and die of alcohol abuse, other drug abuse, and mental disorders that go undiagnosed and untreated. Studies showing that up to 19% of the US adult population have these disorders also have found that only one fifth of those affected have sought help for them in the previous six months. Many of these persons have disorders that are treatable with modern medications and therapy. Because more than half of all persons with these disorders obtain all of their care from the general medical sector, a great potential exists in primary care for prevention, detection, treatm

Validity, reliability and clinical utility of ASSIST-Y in assessing risk of substance-related harm and dependence in Spanish male adolescents.

BACKGROUND: Substance use among adolescents is strongly associated with adverse physical, mental health, and social outcomes. Prevention and early intervention can reduce the likelihood of future problems, but requires valid and reliable screening tools capable of assessing risk across a range of substances. This study assessed the validity, reliability, and clinical utility of the Alcohol, Smoking and Substance Involvement Screening Test (ASSIST-Y) for adolescents aged 15-17 years. METHODS: A sample of adolescent males (N = 101), aged 15-17 years, held in a juvenile detention facility on sub

Deaths from acute drug reactions in Galician (Spain) Prisons (2001-2010).

INTRODUCTION AND OBJECTIVES: drug use is associated with multiple complications with an increase in morbidity, with death by acute drugs reactions (ADR) being the most serious. A large percentage of the prison population has problems associated with drug additions, and substance abuse is also a common internal problem of penal institutions, despite their control measures. The goal of this study is to analyse the prevalence of ADR in penitentiaries, deceased sociodemographic characteristics as well as the circumstances in which they are produced. MATERIAL AND METHODS: All deaths by ADR between

Substance use assessment: comparing self-reports with objective data in a research setting.

Accurate assessment of substance use is essential in public health, clinical, and research settings. While self-reports are widely used, they are prone to biases such as social desirability and recall errors. Objective biological measures, such as hair toxicology, offer a longer detection window and may improve data validity. This study examined the concordance between self-reported substance use and hair toxicology results, with a particular focus on cannabis. It also explored the prevalence and predictors of underreporting, and the relationship between cannabis use patterns and THC detection

What is the drug of choice of young festivalgoers?

BACKGROUND: Drug and alcohol consumption are commonplace at festivals including those aimed at younger attendees. However, there is little quantitative information about the extent of this consumption. This work investigates drug use at a school-leaver festival and how it compares to non-festival weeks. METHODS: Influent wastewater was collected over three consecutive weeks from a location where a school-leaver festival occurs. Multiple liquid chromatography-mass spectrometry methods were used to analyse the use of illicit drugs, pharmaceuticals with abuse potential, new psychoactive substanc

Annual banned-substance review - Analytical approaches in human sports drug testing.

Within the complex construct of today's antidoping work, continuously updated routine doping controls, as well as advancements in sampling and analysis have been of particular relevance and importance. New analytes of existing classes of prohibited substances are frequently included into sports drug testing assays, analytical approaches are optimized to allow for better sensitivities, selectivity, and/or faster turnaround times, and research dedicated to addressing analytical issues concerning scenarios of both (potentially) inadvertent doping and new emerging doping agents is constantly condu

Multiple fatalities in the North of England associated with synthetic fentanyl analogue exposure: Detection and quantitation a case series from early 2017.

BACKGROUND: Synthetic fentanyl analogues are highly potent opioid drugs which have no pharmaceutical use in humans. We detected the synthetic fentanyl analogues; carfentanil, butyryl fentanyl, fluorobutyrylfentanyl, furanylfentanyl, and alfentanil as well as fentanyl itself in 25 cases in early 2017. There have been no previous reports of synthetic fentanyl deaths in the United Kingdom (UK). METHODS: Cases in which the history clearly stated drug use but where a post mortem blood morphine concentration was lower than would be expected to explain the sudden death, were referred for further ana
