Urinary incontinence, the leakage of urine, is a common condition, which can have a significant impact on a patient's quality-of-life. Incontinence may arise as a consequence of a weakness of the urinary sphincter or bladder dysfunction, usually over-activity. Incontinence therapies occupy a large proportion of the healthcare budget. As no single device to manage incontinence is appropriate for all situations, a diverse range of products are available on the market and the development of improved products based on fundamental designs has been slow. This review highlights some of the key issues

OBJECTIVE: Sub-urethral slings are the "gold standard" when treating female urinary incontinence, but placing the prosthetic material has its own specific complication, exposure.
STUDY DESIGN: A retrospective study was performed on all patients who had a sub-urethral sling (TVT-O or TVT Secur) in Lille University Hospital Centre from 2006 to 2008. Follow-up was undertaken to look for re-intervention and to try to determine the risk factors.
RESULTS: A total of 386 patients were studied with a median follow-up of 4 years. The rate of exposure was 4.32%. TVT Secur gave rise to more exposures (

OBJECTIVE: This study evaluated the efficacy of intraoperative extrinsic manual compression on the bladder, or Crede maneuver (CM) for tape adjustment during transobturator tape (TOT) sling procedure versus the traditional method where tension-free tape is adjusted the same for all patients.
METHODS: All patients undergoing TOT sling procedure for stress urinary incontinence (SUI) between May 2008 and June 2011 by the first author were assessed. Tape adjustment was either performed in a traditional manner, leaving a tonsil clamp-size space between the sling and posterior urethra, or by using

INTRODUCTION: We set out to determine if insertion of a retropubic tension-free vaginal tape (TVT) sling at the time of pelvic organ prolapse surgery improves continence outcomes in women with pre-operative occult stress incontinence (OSI) or asymptomatic urodynamic stress incontinence (USI).
METHODS: We conducted a randomised controlled study of prolapse surgery with or without a TVT midurethral sling. The pre- and post-operative assessment at 6 months included history, physical examination and urodynamic testing. Quality of life (QOL) and treatment success was assessed with the UDI-6 SF, II

One of the rarely cited complications of the use of alloplastic materials in pelvic floor surgery is dyspareunia of the partner caused by prominence or vaginal exposure of the sling material. Even though this entity is real and an important problem, it is rarely reported in literature. We report six cases of partner dyspareunia, in which there were two incidences of partner penile injury, occurring as a complication of synthetic midurethral sling surgery. There were three tension-free vaginal tapes, two transobturator tapes and one anterior intravaginal sling in the series. Time interval betwe

OBJECTIVE: To identify urodynamic changes that correlate with successful outcomes after stress urinary incontinence (SUI) surgery.
METHODS: Six-hundred fifty-five women were randomized to Burch colposuspension or autologous fascial sling as part of the multicenter Stress Incontinence Surgical Treatment Efficacy Trial. Preoperatively and 24 months after surgery, participants underwent standardized urodynamic testing that included noninvasive uroflowmetry, cystometrogram, and pressure flow studies. Changes in urodynamic parameters were correlated to a successful outcome, defined a priori as (1)

PURPOSE: We compared continence results of the bladder neck sling vs the Leadbetter-Mitchell bladder neck procedure plus fascial sling in children with neurogenic urinary incontinence.
MATERIALS AND METHODS: We compared consecutive patients who received a 360-degree tight bladder neck sling to subsequent, similar patients who underwent a Leadbetter-Mitchell bladder neck procedure plus fascial sling involving a 50% reduction in bladder neck and proximal urethral diameter before a 360-degree tight sling. All patients underwent simultaneous appendicovesicostomy and none had undergone prior or si

Tension-free vaginal tape (TVT) and transobturator tape (TOT) are relatively new, minimally invasive procedures for the management of stress incontinence. All patients with urodynamic stress incontinence who underwent a TVT procedure from April 2003 to March 2005 (n = 76) were compared with all patients who had TOT from September 2004 to August 2006 (n = 73). The overall success rate for TVT was 97.4% and for TOT was 94.5%. Success was defined as no leakage, as reported by the patient at follow-up. Voiding difficulty in the TVT group was 9.2% for <1 week's duration and 2.6% for >1 week's durat

The introduction of tension-free vaginal tapes has led to a complete change of surgical therapy in female urinary stress incontinence. Different materials are used. In many departments, the classic TVT procedure with retropubic placement of the tape has been replaced by transobturator procedures. The current discussion involving transobturator tapes focuses on the question of whether the inside-out technique or the outside-in technique leads to significantly different results. Current state of the art is that all techniques are almost comparable concerning treatment success and complications.

A woman who underwent transobturator sling surgery for urinary incontinence experienced early vaginal mesh erosion, and underwent a partial sling removal. Several months later, she developed recurrent right thigh and groin abscesses and necrotizing fasciitis. The source of the infection, a retained segment of mesh in the obturator space, was identified only after several operative procedures and referrals. This case illustrates several of the areas of concern with the introduction of new surgical materials and techniques.

PURPOSE: We analyzed the effect of concomitant prolapse surgery performed at the time of sling surgery on short-term postoperative outcomes in women with urinary incontinence.
MATERIALS AND METHODS: We analyzed 1999 to 2001 Medicare claims data on a 5% national random sample of female beneficiaries who underwent sling procedures. Subjects were tracked for 12 months after surgery to assess short-term complications. Concomitant prolapse repairs and prolapse repairs performed in the first 12 months after sling surgery were identified by CPT-4 procedure codes. Postoperative complications and trea

Tension-free vaginal tape (TVT) procedure has become one of the most common treatments of female urinary stress incontinence. Success rates as high as 81.3% were reported over a follow-up period of 7 years. Erosion of the synthetic mesh is a well-described complication. The mean time for the onset of erosion after sling insertion was 11.2 months. These case reports describe an erosion of a mid-urethral tape after 18 and 28 months, which is uncommon. There is a need for long-term follow-up of patients with TVT.

BACKGROUND: The TVT-Secur (Ethicon, Somerville, NJ, USA) is a minimally invasive suburethral synthetic sling used in the treatment of female stress urinary incontinence. It claims to cause less postoperative pain and to enable performing in an office setting. However, this may be at the expense of a significant learning curve and a higher early failure rate.
AIMS: To assess objectively the success rate of the TVT-Secur procedure in the 'U' configuration at six months. Secondary outcomes focussed on subjective success rates, complications, patient satisfaction and quality-of-life (QOL).
