Rocuronium ≤1.5 mg/kg versus >1.5 mg/kg and inadequate paralysis in prehospital and retrieval intubation: A retrospective study.

OBJECTIVE: Greater Sydney Area Helicopter Emergency Medical Service encouraged an increased dose of rocuronium for rapid sequence intubations (RSIs) from 1.5 mg/kg to 2.0 mg/kg from the end of 2017 in response to a number of inadequately paralysed patients identified by our airway audit processes. Subsequent protocol change incorporated 2.0 mg/kg rocuronium. This retrospective observational study was undertaken to see if doses over 1.5 mg/kg rocuronium were associated with a reduction in the number of inadequately paralysed patients. METHODS: Retrospective review of patient prehospital notes

[Fatal outcome caused by lysthenon in a child at the early stage of primary myopathy].

A child aged 3 years 10 months with latent early stage of primary myopathy died during surgery for left-side cryptorchidism after injection of lysthenon. Asystole, the immediate cause of death, was caused by release of potassium ions from skeletal muscles into the blood. Myopathy was diagnosed at autopsy by histological examination of the diaphragm and mm. pectoralis major and gastrocnemius.

Suxamethonium and cardiac arrest.

We report a case of cardiac arrest due to hyperkalaemia following administration of suxamethonium during a procedure to facilitate a change of endotracheal tube in a septic patient. The cause of this rare but fatal complication is briefly described and discussed. In view of this, suxamethonium should be used with great caution in patients with burns and other forms of physical injury, in a number of nervous system disorders, and in critically ill patients requiring prolonged ITU care.

Differential effects of right unilateral and bilateral ECT on heart rate.

In a series of 11 melancholic patients who received both right unilateral and bilateral ECT during a single course of treatment, mean and minimum postictal heart rates were greater after bilateral ECT. Greater catecholamine release during bilateral ECT is the most likely explanation for these effects. These differences complement other evidence that bilateral ECT has a more pronounced physiological impact than unilateral ECT.

Temperature dependence of muscle function in malignant hyperpyrexia-susceptible swine.

Contractures induced by halothane, caffeine, suxamethonium chloride and potassium in malignant hyperpyrexia-susceptible (MHS) and control swine skeletal muscle preparations were temperature-dependent, but differed in their temperature response profiles. Increasing the temperatures from 20 degrees C to 37 degrees C increased drug-induced contractures in MHS muscle. At 5 degrees C drug-induced contractures were not inhibited by dantrolene sodium, deuterium oxide or glycerination in either MHS or control muscle. These observations give further support to the view that the abnormality in MHS skele

Suxamethonium fasciculations: a topographical study.

It has been found in a group of 455 gynaecological patients that not all the voluntary muscles of the body respond to suxamethonium in the same way. One group of muscles (e.g. biceps) which is sensitive to suxamethonium, responds by fasciculating in almost every individual. A second, resistant, group of muscles (e.g. pectoralis major) is almost totally unresponsive to suxamethonium. A third muscle group (e.g. triceps) shows fasciculations in about 60% of cases and appears intimately associated with the development of suxamethonium pains, for if fasciculations do occur then suxamethonium pains

The prophylaxis of malignant hyperthermia by oral dantrolene sodium in swine.

The oral administration of dantrolene for the prophylaxis of malignant hyperthermia (MH) in susceptible swine has been investigated. Complete protection from MH in pigs exposed to the known MH initiating agents, halothane and suxamethonium, was demonstrated.

The strychnine-like action of curare and related compounds on the somatosensory evoked response of the rat cortex.

1. Drugs were applied to the cerebral cortex of rats anaesthetized with pentobarbitone and changes measured in the somatosensory primary evo … i-allylnortoxiferine and atropine also produced this effect but were less potent than strychnine.6. Succinylcholine, dihydro-beta-erythroidine and gallamine triethiodide did not produce this effect (in concentrations up to 10(-3)M).7. The observations are consistent with an action of curare and strychnine on an intracortical cholinergic inhibitory system, but oth …

Stable Gastric Pentadecapeptide BPC 157 as a Therapy of Severe Electrolyte Disturbances in Rats.

This review explores the therapeutic potential of the stable gastric pentadecapeptide BPC 157 in addressing electrolyte imbalances, specifically hyperkalemia, hypokalemia, hypermagnesemia, and hyperlithiemia. In hyperkalemia, BPC 157 demonstrated a comprehensive counteractive effect against KCl overdose (intraperitoneally, intragastrically, and in vitro), effectively mitigating symptoms such as muscular weakness, hypertension, sphincter dysfunction, arrhythmias, and lethality. It also counteracted the adverse effects of succinylcholine and magnesium overdose, including systemic muscle paralysi…

Anesthetic management of a patient with limb-girdle muscular dystrophy 2B:CARE-compliant case report and literature review.

BACKGROUND: Limb-girdle muscular dystrophies (LGMDs) belong to few neuromuscular disorders mainly involving pelvic and shoulder girdle muscles. Also, cardiac or pulmonary complications, increased rhabdomyolysis risk when exposed to volatile anesthetics and succinylcholine may increase anesthesia related risks. However, current reports about the anesthesia management of these patients are limited. CASE PRESENTATION: We described our anesthetic management of a 36 years old woman with LGMD 2B receiving arthroscopic knee surgery. In consideration of the high risk of rhabdomyolysis, total intraven…
