QT dispersion in infants with apparent life-threatening events syndrome.

Apparent life-threatening event (ALTE) is a term used to define an event of unknown cause after an infant is found limp, cyanotic, bradycardic, and/or requires resuscitation. Eight to 15% of children with ALTE die of sudden infant death syndrome. Obstructive sleep apnea, bradycardia, gastroesophageal reflux, and laryngotracheal abnormalities are frequently associated with ALTE. Wide QT dispersion is associated with sudden death in heart failure and increased risk of ventricular fibrillation in acute myocardial infarction. Here, we assess QT dispersion in infants with ALTE and its correlation t…

Breast feeding and toxigenic intestinal infections: missing links in crib death?

Infant botulism results when Clostridium botulinum spores germinate, colonize the gut, and there produce botulinal toxin, which after absorption causes flaccid muscle paralysis. The observed variation in the severity of the disease was linked to the infant's milk source, in that all sudden death cases indistinguishable from typical crib death occurred in infants who had been formula-fed, whereas the more gradual onset, hospitalized cases occurred in infants who were predominantly breast-fed. Secretory IgA antibody against C. botulinum vegetative cell antigens was found in human milk from mothe

Regional Infant and Child Mortality Review Committee--2011 final report.

The 2011 annual report of the Regional Infant and Child Mortality Review Committee (RICMRC) is presented. Since 1997, the committee has revi … sult of progressive neurological diseases or conditions. In 2011 there were no deaths attributed to Sudden Infant Death Syndrome (SIDS), though there were two deaths of infants during sleep. One of these infants was ruled accidental as the baby died of aspiration and the other death occurred in an unsafe environment with its manner determined to be undecided. Six dea …

Sudden unexplained death in individuals with and without epilepsy.

PURPOSE: We aimed to compare demographics, circumstances, comorbidities, autopsy results, postmortem toxicology, and prescription claims between sudden unexplained deaths (SUD) without and with epilepsy, also known as sudden unexpected death in epilepsy (SUDEP). METHODS: SUD cases were identified from forensic autopsy and toxicology reports on a previously identified nationwide population of sudden deaths and sudden cardiac deaths in Denmark. SUD was defined as sudden cardiac death with no/subdiagnostic cardiac pathology and nonlethal toxicology. We included cases between 2000-2019 and 2007-2

[Assymetric skull? Early correct diagnosis is a must! Positional skull deformities can be avoided].

During the past few years the supine sleeping position has successfully been reintroduced for infants in order to prevent sudden infant death syndrome. However, the change in sleeping position has not been accompanied by a soft pillow under the infants head, which has probably caused a higher incidence of acquired skull deformities. These positional skull deformities need to be prevented or treated.

Bradycardia preceding apneic attacks in low-birthweight infants. The relationship and management.

Two premature infants had frequent episodes of prolonged apnea. The apneic spells were not due to the more commonly known causes of apnea in infancy, but were consistently preceded by severe bradycardia. Atropine or ephedrine produced favorable therapeutic results. Since severe bradycardia may be a cause of sudden death in infants, its recognition and treatment is important in the management of apneic infants.

Why is a prone sleeping position dangerous for certain infants?

The prone (face down) sleeping position is known to be associated with a significantly increased risk of sudden and unexpected death in infancy (sudden infant death syndrome or SIDS), however, the reasons for this are unclear. Suggested mechanisms have involved suffocation from occlusion of the external airways by soft bedding/pillows or from flattening of the nose with backward displacement of the tongue, rebreathing of carbon dioxide, blunting of arousal responses with decreased cardiac responses to auditory stimulation, diaphragmatic splinting or fatigue, lowering of vasomotor tone with tac…

Does particulate air pollution contribute to infant death? A systematic review.

There is now substantial evidence that both short- and long-term increases in ambient air pollution are associated with increased mortality … f infant mortality. It was more consistent for postneonatal mortality due to respiratory causes and sudden infant death syndrome. Differential findings for various mortality subgroups within studies suggest a stronger association of particulate air pollution with some causes of infant death. Research is needed to confirm and clarify these links, using the most approp …

[Clinical conditions associated with abnormal QT interval: clinical implications].

Impressive progress has been made in the last 40 years in the understanding of the role of QT interval and its genetic basis in sudden cardiac death risk. The present review will provide a first practical part on QT measurement and its correction for heart rate. Subsequently, the long QT syndrome and short QT syndrome will be described, as the two main arrhythmogenic congenital heart diseases characterized by abnormal QT length. Furthermore, we will discuss about prolonged QT in the pathogenesis of sudden infant death syndrome and the preventive role of neonatal ECG screening. The prognostic r…

Evaluation and management of apparent life-threatening events in infants.

Apparent life-threatening events (ALTEs) are frightening to caregivers and clinicians alike. This article provides a comprehensive review of the causes, management, and consequences of ALTEs. The information provided was collected from an extensive literature review using the search terms ALTE, sudden infant death syndrome, and apnea. There is a wide array of contributing factors to ALTE syndrome and sequalae for both infants and caregivers of infants experiencing an ALTE.

Cardiovascular abnormalities in children: a ten-year forensic pathologic study.

In order to obtain information on types, incidence, and significance of cardiovascular abnormalities in children, a total of 104 consecutive medicolegal autopsies of children aged 8 days to 16 years during a 10-year period from May 1974 to April 1984 were studied. Extensive histological examination of the hearts was performed in 92 out of 104 cases and complemented with chemical and microbiological analyses. In the natural death group consisting of 53 children, 26 (49%) showed abnormalities: 7 (13%) malformations, 11 (21%) cardiomyopathies, 5 (9%) idiopathic subaortic hypertrophy, and 3 (7%) a

[Sudden death due to undiagnosed congenital laryngeal cyst].

Indicating signs of congenital cysts of the larynx are inspiratory stridor, dyspnea, weak cry, feeding difficulties. Complete or partial absence of such symptoms does not refer to a true congenital cyst. A false diagnosis could occur, due to small directive clinical signs. A relevant observation is communicated.

Coenzyme Q deficiency may predispose to sudden unexplained death via an increased risk of cardiac arrhythmia.

Cardiac arrhythmia is currently considered to be the direct cause of death in a majority of sudden unexplained death (SUD) cases, yet the genetic predisposition and corresponding endophenotypes contributing to SUD remain incompletely understood. In this study, we aimed to investigate the involvement of Coenzyme Q (CoQ) deficiency in SUD. First, we re-analyzed the exome sequencing data of 45 SUD and 151 sudden infant death syndrome (SIDS) cases from our previous studies, focusing on previously overlooked genetic variants in 44 human CoQ deficiency-related genes. A considerable proportion of the…

[Parasomnias in infants below one-year old of age].

Certain sleep disorders which occur in children and adults show the same phenomena, although presentation, significance, cause and treatment are all different. However, there are some disorders which are only found in infancy, as in the four conditions which we review below. In spite of great variation between individuals, the general way in which the wakefulness-sleep states mature during the first months of life is well known. In general the physiological patterns of REM and NREM are in general the same in children as in adults. However, the pathological patterns are similar, but there are m

Isolated myocarditis in the first year.

Over a period of nearly 40 years, 20 cases of isolated myocarditis were traced from 3086 necropsies. Seventeen occurred in infants less than 12 months of age, often with no antecedent clinical signs (sudden deaths) or with a short clinical history of less than 24 hours' duration.

Fatty Liver and the Forensic Pathologist.

Fatty liver is a common finding in clinical practice and at autopsy. It is most commonly seen associated with alcohol abuse and in non-alcoholic fatty liver disease (NAFLD). It may also be seen in many other conditions in both adults and children. It is now recognized that NAFLD, like alcoholic liver disease, may lead to end stage liver disease. Nonalcoholic fatty liver disease is associated with increased mortality from other disorders, particularly cardiovascular diseases. Fatty liver may be seen in many conditions that concern autopsy pathologists, including drug toxicity, anorexia, hepatic

The impact of changes in preterm birth among twins on stillbirth and infant mortality in the United States.

OBJECTIVE: To examine trends for preterm births, stillbirths, neonatal and infant deaths in twin births by gestational age and birth weight … during the study period. Reductions in post-neonatal infant mortality were mainly in RDS (88%) and sudden infant death syndrome (26%). Mortality rates among infants born by either induction of labor or cesarean delivery fell during the study period and remained much lower than the overall infant mortality rate. CONCLUSION: The findings of this study suggest that du …

Early sudden unexpected death in infancy (ESUDI)--three case reports and review of the literature.

AIM: Early sudden unexpected death in infancy (ESUDI) is a rare cause of death occurring in newborns during the first hours of life. Our aim was to find additional data for the identification of risk factors and establishment of prevention strategies. METHODS: We describe three cases of ESUDI and give an overview of the literature. RESULTS: ESUDI was observed in term babies with normal birth weight after good postnatal adaptation within the first four hours of life. Maternal age was between 29 and 36 years, mothers were primiparous and of normal weight. All three events occurred during unobs
