Swallowing performance after radiotherapy for squamous cell carcinoma oesophagus.

OBJECTIVE: To consider assessment of swallowing performance status as an alternative to survival in reporting results of treatment in patients with carcinoma oesophagus. METHODS: Twenty five patients of squamous cell carcinoma of oesophagus treated with radiotherapy were evaluated clinically, radiologically and endoscopically, to assess their response to treatment. RESULTS: Two months after radiotherapy, 10 of 25 patients had swallowing score-1. During radiotherapy, oesophagitis involving normal oesophagus and diseased segment was seen in 64% of cases. Fifty six percent of patients developed

[Swallowing of fixed denture following general anesthesia--a case report].

Dental injury is a common complication associated with endotracheal general anesthesia. According to a study in Japan, the incidence of dental injury caused by endotracheal intubation is 0.69%. In one study in our country, evaluation of the oral condition was performed before, during and after anesthesia, and the frequency of dental trauma is as high as 12.1%. Fragment(s) of tooth or denture may cause serious, or even fatal complications. We report a case of swallowing of fixed denture following endotracheal general anesthesia. Our accident involved a 33-yr-old male who had experienced bilater

A comparison of swallowing during apnea and periodic breathing in premature infants.

Periodic breathing and apnea are two forms of ventilatory instability which are commonly observed in premature infants. This study was undertaken to characterize the pattern of swallowing during apnea and compare this pattern to that present during breathing. We assessed the frequency and distribution of swallows during the respiratory pauses of apnea and periodic breathing in 22 premature infants birth weight 1.2 +/- 0.2 kg, postconceptional age 34 +/- 2 wk. Twelve infants had apnea and 10 exhibited periodic breathing. During sleep the pharyngeal phase of swallowing was detected by a catheter…

Modification in activity of medullary respiratory-related neurons for vocalization and swallowing.

1. The medullary ventral respiratory group (VRG) in and near the nucleus ambiguus contains neurons related to respiration. Also found here are neurons related to vocalization and swallowing as well as motoneurons of laryngeal, pharyngeal, palatal, and esophageal muscles. Previous reports in anesthetized animals have characterized discharge properties of neurons as they relate to a single behavior, e.g., respiration. Relatively few studies have documented discharge properties during more than one behavior, e.g., respiration and swallowing. Neurons were recorded extracellularly from awake Macaca

Effects of maneuvers on swallowing function in a dysphagic oral cancer patient.

This study examined the effects of three swallow maneuvers: (1) the supraglottic swallow (voluntary closure of the vocal folds prior to the … (VFG) swallow study conducted 6 months after the patient's surgery. Biomechanical analysis of 3-mL swallows defined the extent and timing of tongue base retraction to the posterior pharyngeal wall, laryngeal elevation, laryngeal closure and cricopharyngeal opening during swallows with and without maneuvers. Airway closure duration was prolonged during sup …

Swallowing, speech, and brainstem auditory-evoked potentials in spasmodic torticollis.

To explore the controversial "brainstem theory" of spasmodic torticollis, eight consecutively referred patients were examined. Three independent examinations were conducted on the same day: a videofluoroscopic barium swallowing examination, an instrumental speech examination, and a brainstem auditory-evoked potential (BAEP) analysis. Swallowing was normal in two patients; speech physiology, in five; and BAEPs, in all. Normal BAEPs refute the brainstem theory, while abnormalities of speech and swallowing temper this conclusion. Several alternative explanations are proposed.

[Digital cineradiography (DCR) of the act of swallowing. The initial clinical experiences].

Twenty-seven patients with different swallowing disorders were studied by employing digital cineradiography (DCR) for rapid imaging of the pharynx with a temporal resolution of up to 50 frames per second and a maximum spatial resolution of .8 per/mm. To facilitate therapy planning, the DCR study can be transferred to the otorhinolaryngologist via videotape. DCR is capable to substitute analog cine or video techniques in the pre- and postoperative examination of patients with swallowing disorders.

Swallowing recovery following anterior tongue and floor of mouth surgery.

Videofluoroscopic and clinical data are presented for postsurgical recovery of tongue and hyoid activity during swallowing. Comparison is made to normal controls. Postsurgery, but prior to radiotherapy, both the lingual propulsive gesture and hyoid velocity were slower than normal. These features of the swallow had returned to the normal range following radiotherapy. The general sequencing of swallowing events, and the relationship of the end of the tongue propulsion to anterior hyoid motion were normal in those patients able to swallow. In this patient group, postsurgical radiotherapy did not

Management of pediatric patients who have swallowed foreign objects.

The clinical management of children who have swallowed foreign bodies presents a great challenge for both pediatricians and pediatric surgeons. Our 7-year experience of treating 141 patients with a history of foreign body ingestion is reported. In 114 patients, there were 122 foreign bodies present in areas from the esophagus to the rectum, as located by plain chest and abdominal x-ray examination. In two patients, a plastic toy and the plastic cap of a razor were passed with stool a few days later. In the remaining 25 patients, nothing was found. In babies under one year of age, button batter

Analysis of pressure generation and bolus transit during pharyngeal swallowing.

Current pharyngeal deglutition theory has stressed the role of the pharyngeal constrictors as producing a peristaltic wave responsible for bolus propulsion through the pharynx. This thesis presents data obtained using manofluorography which supports the significance of tongue and laryngeal motion in swallowing. The usage of the term peristalsis to describe the constrictor contraction is challenged. The results of this quantitative study of swallowing in normal subjects, laryngectomized patients, and patients with restricted tongue motion show that tongue driving pressure and the negative press

Supraventricular tachycardia induced by swallowing: a case report and review of the literature.

A 64-year-old man who complained of palpitations brought on by swallowing was found to have short runs of paroxysmal supraventricular tachycardia (SVT) induced by swallowing. Electrophysiology studies suggested that the SVT was an automatic atrial tachycardia. An esophageal manometric study demonstrated that the tachycardia was coincident with relaxation of the upper esophageal spincter and preceded peristaltic activity in the esophageal body. Atropine and bethanechol did not affect the swallow-induced tachycardia. The patient's symptoms were controlled by verapamil and quinidine. After five m

[Removal of swallowed metallic foreign bodies by orogastric magnetic intubation].

Foreign bodies with metal content ingested by 16 patients were removed from the upper gastrointestinal tract by means of a magnet inserted into the end of orogastric tube. In only one case did the foreign body uncouple from the orogastric tube-magnet arrangement during retrieval. The procedure is an alternative to endoscopic or surgical removal in some cases of metal foreign body ingestion.

These swept over the oesophagus at about 200 mm X s-1 and occurred without associated swallows. The variability of reactions of the oesophagus in eructation and differences in reactions of its different regions are discussed as arising from different degrees of sensitivity to, and stimulation by, gaseous distension of the oesophagus and stoma …

Diagnostic value of "bread-barium" swallow in patients with esophageal symptoms.

The value of a "bread-barium" swallow for diagnosis of disordered esophageal motility was examined in 53 patients, 20 of whom complained of dysphagia, 20 retrosternal chest pain, and 13 heartburn; a further 19 subjects served as controls. Esophageal manometry in the patients with dysphagia and chest pain revealed major abnormalities in 14 compared with the bread-barium swallow which showed changes consistent with esophageal spasm in 20. The standard barium swallow examination revealed major abnormalities in only five of these patients. The bread-barium examination is simple, complements the ro

Recognition and management of patients who repeatedly swallow foreign bodies.

Repeated swallowing of foreign bodies, in adults, is part of a syndrome comprising personality disorder, drug overdose, and other self-inflicted injuries. The management should be conservative, as in many cases the foreign body will pass naturally or can be left in the bowel, if necessary for several years. Surgical intervention should be reserved for the complications of perforation, severe haemorrhage and persistent complete bowel obstruction, or for the removal of a toxic substance from the bowel. Five cases are reported which illustrate the natural history and management of the problem.

Extended hemilaryngectomy for T3 glottic carcinoma with preservation of speech and swallowing.

Total laryngectomy is often applied in the treatment of invasive squamous cell carcinomas that fix one side of the larynx. The major drawback, of course, is loss of the voice. In many instances, however, preservation of the uninvolved portion of the larynx is compatible with adequate tumor margins, and the preserved laryngeal remnant, although it cannot be reconstituted to allow breathing, can readily be used for voice. The principle involved is the creation of a valved tracheopharyngeal shunt, which functions as a neoglottis during expiration but constricts to close during swallowing. To acco

Swallowing disorders in three types of head and neck surgical patients.

This study examined swallowing transit times and motility problems in three groups of patients following ablative surgery for oropharyngeal carcinoma and in a control group of 10 normal subjects. A total of 30 patients was studied: 10 after anterior floor of mouth resection, 12 after tonsil/base of tongue resection, and 8 after supraglottic laryngectomy. Videofluoroscopic studies of liquid, thin paste, thick paste, and thick paste plus liquid swallows were completed 1 week post-initiation of oral feeding following surgery. From the videotapes, oral and pharyngeal transit times were measured, a…

[Respiratory and swallowing difficulties after oesophageal atresia (author's transl)].

The main reason for these problems is a tracheo-oesophageal fistula, either recurrence of the T.O. fistula, either persistance of a fistula which has been neglected during surgery. It has been observed in 7 infants from 19 operated atresias with such problems. These functionnal troubles may be produced by different other anomalies: oesophageal stenosis and or dyskinesia often observed, gastro-oesophageal reflux, associated anomalies of the larynx or trachea; laryngeal paralysis, tracheomalacia, tracheal epithelium metaplasia, tracheal compression by abnormal vessel, neurological dysmaturity, l
