Multifocal pyomyositis. Diagnosis on technetium-99m MDP bone scan.

Pyomyositis is a bacterial infection of skeletal muscle occurring in the absence of trauma or a primary site of infection. The condition is rare in temperate climates. The diagnosis is often difficult to make and frequently delayed since the suppurative process is confined by the tense overlying muscle and fascia. The bone scan is very useful in differentiating underlying osteomyelitis and septic arthritis. An 11-year-old boy is described who presented with multifocal pyomyositis in the elbow region and thigh. Lateral blood pool images suggested infection with liquefaction within the muscles w

[When mosquitoes no longer fear the cold: arboviruses in Europe].

Infectious diseases result from complex interactions between a host, a pathogen and a vector within a given environment. Over recent decades, climate change, population movements and international trade have contributed to the emergence of arboviruses in temperate regions by facilitating the establishment of their vectors and pathogens. The presence of Aedes and Culex mosquitoes in Europe enables local transmission of viruses such as dengue (DENV), chikungunya (CHIKV) and West Nile virus (WNV). Clinicians should therefore consider these infections in the differential diagnosis of febrile illne

What is the role of arthroscopy in hand and wrist trauma?

The use of arthroscopy of the hand and wrist has recently increased sharply in the elective setting and this, not surprisingly, has been followed by an increasing use in the trauma setting. Advocates for the use of arthroscopy in these patients cite the improved assessment of the displacement and reduction of fractures and the early diagnosis of associated injuries, while others temper this with concerns about increased operating time and possible complications. The data relating to patient-reported benefits and costs are limited. The aim of this review was to discuss the current available lit

Pyomyositis After a Group C Streptococcal Toxic Shock Syndrome.

Pyomyositis is a bacterial infection deep within the muscles, often leading to multiple intramuscular abscesses. While historically linked with tropical regions, its incidence in temperate zones has been increasing, primarily due to factors such as immunosuppression. Typically, it manifests as a subacute infection, although when caused by Group C Streptococcus and resulting in toxic shock syndrome, it can lead to poorer outcomes. Here, we report a rare case of extensive multifocal bilateral pyomyositis in an immunocompetent young woman, preceded by toxic shock syndrome.

A case of triple infection including strongyloides stercoralis in a microscopic polyangiitis patient.

We present the case of a microscopic polyangiitis (MPA) patient who developed strongyloidiasis, nocardia and citrobacter freundii (CF) infection after corticosteroides and immunosuppressant therapy. When digestive, respiratory or other system symptoms consecutively occur in a immunocompromised host who lives in tropical or temperate zone and have close contact with soil, we should take strongyloidiasis into consideration despite absence of eosinophilia. Mixed infection with nocardia cannot be easily excluded. It is essential to search for the etiology proof with multiple approaches positively

Current progress toward vaccine and passive immunization approaches for Strongyloides spp.

Strongyloides stercoralis is a helminth parasite that can infect millions of people worldwide, particularly in tropical, subtropical and temperate regions with poor sanitation. Several aspects of epidemiology, biology and host-parasite interactions of S. stercoralis have been studied, and substantial knowledge has been acquired; however, very few studies on immunotherapeutic control strategies to prevent infection and disease in humans have been conducted. Therefore, this article reviews the current progress and targets toward vaccine and passive immunization approaches for Strongyloides spp.

Pyomyositis of the leg with early neurologic compromise.

Pyomyositis, although uncommon, is being reported with greater frequency in temperate climates. The presentation is similar to a number of infectious processes, and when associated with a traumatic event, the clinical picture may be confused with that of a musculoskeletal injury. This, coupled with an unfamiliarity of the disease, may result in a delay in diagnosis. Early antibiotic therapy may obviate surgery. Progression to the suppurative stage requires surgical drainage along with antibiotics. CT guided drainage may be accomplished in certain cases. In immunocompromised patients, progressi

Designing blood-stage vaccines against Babesia bovis and B. bigemina.

The tick-transmitted apicomplexan parasites Babesia bovis and B. bigemina cause significant disease in cattle in many tropical and temperate areas of the world. These parasites present a challenge for vaccine development, and yet provide a system for studying the pathogenesis, mechanisms of protective immunity and regulation of host immune responses associated with intraerythrocytic protozoan parasites in a non-rodent species. In this article, Wendy Brown and Guy Palmer review strategies for identifying candidate vaccine antigens of B. bovis and B. bigemina and for priming immune responses to

Selected gastrointestinal pathologies in tropical sub-Saharan Africa.

Doctors need to be well informed about differences in the presentation of certain diseases in tropical and temperate climates. In this article the characteristics of some gastrointestinal diseases, as they recur in sub-Saharan Africa, are briefly reviewed. Diseases of the stomach--including ulcertaion and cancer--are uncommon in Africa, although duodenal ulcer is common all over the tropics. In contrast, colorectal cancer is an extremely rare illness in sub-Saharan Africa, while hepatocellular carcinoma is much commoner than in Europe or North America and the very high incidence of this tumour

Chronic cavitary pulmonary sporotrichosis: efficacy of oral itraconazole.

The small number of patients suffering from pulmonary involvement with Sporothrix schenckii has prevented prospective controlled studies that could determine the optimal therapy for this chronic infection. The clinician's ability to determine the best medical treatment for chronic cavitary pulmonary sporotrichosis is also tempered by the limited use of newer azole antifungal agents in this disorder as well as the relative lack of efficacy reported with older therapies. We present a 50-year-old male with primary pulmonary sporotrichosis whose chronic cavitary disease responded to oral itraconaz

[Tropical pyomyositis. A case report].

Tropical pyomyositis is an infection of large muscle groups that can lead to sepsis and death. The most common etiologic agent is Staphylococcus aureus. It usually occurs in patients living in the tropics but is seen with increasing frequency in temperate climates, particularly in immunosuppressed patients, where it may be misdiagnosed and may cause severe morbidity and mortality. Diagnosis is based on the examination of pus from a muscle aspirate and treatment consists of surgical incision, drainage and appropriate antibiotic therapy. It is stressed to take into account pyomyositis in the dif

A case of the frontal lobe syndrome following head injury in Harare, Zimbabwe.

A 51 year old male, presented with recurrent outbursts of aggression, hypersexuality, sexual indiscretion and hoarding litter following a head injury sustained in a road traffic accident. His premorbid personality was described as energetic but quick tempered. He had no delusions or perceptual disturbances and cognitive functions were intact. Physical and detailed neurological examination were essentially normal, with the exception of disuse atrophy of the lower limb muscles. He lacked insight into his problems. This case illustrates the need for a multidisciplinary approach to assessment, man

Melatonin in the control of the estrous cycle of the Indian desert gerbil (Meriones hurrianae: Jerdon).

Daily administration of melatonin for 8 weeks, either in the morning (9 a.m.) or late in the afternoon (5 p.m.), resulted in significant prolongation of the estrous cycle in the Indian desert gerbil Meriones hurrianae. The gonadosomatic index increased significantly (p < 0.01) in the melatonin-treated gerbils compared to the control animals. Histological study of the ovaries revealed enlargement of follicles and corpora lutea with hypertrophied granulosa cells in the melatonin-treated gerbils in comparison to their controls. The data suggest that the mechanism involved in the gonadal response

Clinical experience with the Xomed Audiant osteointegrated bone conducting hearing device: a preliminary report of seven cases.

An osteointegrated bone conducting hearing device has recently become available in North America. As with any new medical treatment, initial enthusiasm is soon tempered by its inherent limitations. Critical and objective analysis of results and candid reporting of clinical experiences will lead to the establishment of realistic indications for any new treatment. We present the current indications for insertion of the Xomed Audiant Bone Conductor, our early clinical experience and our thoughts on its future role in the management of patients with conductive hearing loss.

Endemic institutional strongyloidiasis in British Columbia.

The incidental finding in 1984 of Strongyloides stercoralis larvae in a resident of a chronic care institution who had a vague clinical illness prompted a review of the other residents. Five other cases were identified after exhaustive laboratory investigations. Fecal-oral spread was considered the most likely manner of the spread of infection. The possibility of endemic strongyloidiasis in institutions should be considered, even in temperate climates, when there is unexplained persistent illness or high eosinophil counts. Serologic testing is a useful adjunct to fecal examination in such situ

Absorption of nutrient energy in southern Indian control subjects and patients with tropical sprue.

Fecal energy excretion was measured by bomb calorimetry, in a group of 30 healthy volunteers and 15 patients with tropical sprue, to determine the absorption of nutrient energy. The mean energy absorption in the healthy volunteers (91.6%) was less than in similar subjects in temperate climates. The reduction of energy absorption in the volunteers, who all have tropical enteropathy, suggests that this condition leads to wastage of 5% of energy intake. In patients with tropical sprue the energy absorption was significantly lower. A strict metabolic balance study was not essential to detect energ

Arteriomegaly: classification and morbid implications of diffuse aneurysmal disease.

Ninety-one patients with arteriomegaly and diffuse aneurysmal disease below the level of the renal arteries have been classified according to the extent and location of aneurysmal change. There exists a high incidence of thrombotic and embolic complications, and treatment entails increased rates of morbidity and mortality when compared to surgical treatment of simple abdominal aneurysms of peripheral artery aneurysms. Complete revascularization at the initial operation would appear to give the best result, but this approach must be tempered by the individual patient risk factors and the urgenc

[Lysogeny by phages specific to Yersinia pestis in bacteria of the microbiocenoses of wild plague-carrying rodents].

For the first time lysogenic bacteria of the genus Escherichia, spontaneously producing bacteriophages specific to the infective agent of plague, have been detected in the microbiocenoses of midday gerbils, plague carrier in the Volga-Urals natural focus. The morphological, cultural and biochemical properties of 22 lysogenic Escherichia strains isolated from wild rodents, as well as the spectra of the lytic activity of temperate bacteriophages produced by the above-mentioned microorganisms have been studied. Proceeding from the findings obtained in this research, all the strains have been divi

Maintenance of function in relocation of community-dwelling older adults.

Relocation is often seen as a high-risk stress for the elderly. The present study compared relatively healthy older people moving voluntarily with nonmovers. An examination of behavioral test performance in a pre-post design revealed no decrement in function among those who changed residence. The proscription against relocation of older adults would appear to best be tempered with a careful examination of the conditions surrounding the move and the particular population involved. If there is indeed risk in the relocation of the institutionalized aged, there appears to be no necessary comparabl
