Biomimetic Constructs for Achilles Tendon Regeneration and their Translation to Human Medicine.

BACKGROUND: Achilles tendon injuries are among the most common lower-body tendon injuries, often resulting fromoveruse and repetitive motion. Current treatments, ranging from conservative therapies to biological grafts, have drawbacks, including limited regenerative capacity and the risk of graft rejection. To overcome these challenges, tissue engineering shows growing promise for Achilles tendon regeneration, with ongoing research focused on developing biomimetic constructs that utilize various materials and biologics. Since every tendon has unique biomechanical and physiological characterist…

Postoperative Rehabilitation Following Subscapularis Repair and Biceps Tenodesis in an Adolescent Overhead Athlete: A Resident's Case Report.

BACKGROUND: The incidence of rotator cuff pathology in adolescent athletes is incredibly rare, consisting of only 1% of upper extremity injuries in those under the age of 20. Most rotator cuff injuries in this population are of the supraspinatus with few being of the subscapularis. The subscapularis aids with internal rotation and stabilization of the glenohumeral joint. Surgical management of these injuries in an adolescent athlete presents unique challenges due to periods of rapid growth in adolescence. Additional challenges exist in guiding post-operative physical therapy due to limited inf

Combined reconstruction of the anterior cruciate ligament and posterolateral corner with a single femoral tunnel.

Combined injuries involving the anterior cruciate ligament (ACL) and posterolateral corner (PLC) occur in approximately 10% of complex knee injuries. The current tendency is to reconstruct both the ACL and the structures of the PLC. In injuries involving multiple ligaments, a potential problem in the reconstruction is the convergence of tunnels in the lateral walls of the femur. As a solution to this problem, we propose a combined technique for reconstruction of the ACL and PLC with a single tunnel in the lateral femoral wall. Combined ACL/PLC reconstruction is performed with 2 semitendinosus

Functional restoration of elbow flexion in brachial plexus injuries: results in 167 patients (excluding obstetric brachial plexus injury).

From 1985 to 1990, 167 patients were treated for impaired elbow flexion caused by brachial plexus injury. Surgical procedures were divided into two categories: nerve reconstruction (128 patients) and muscle or tendon transfers (39 patients). Surgery in the nerve reconstruction group included direct suturing, nerve grafting of portions of the brachial plexus responsible for elbow flexion, or nerve transfer (intercostal, phrenic, or spinal accessory nerve) to the musculocutaneous nerve. The second category included tendon or muscle transfer or a functioning free muscle transplantation for biceps

Avulsion injuries of the pelvis.

Pelvic avulsion injuries occur in both the skeletally immature and adult patient populations. Avulsion injuries are most common in the adolescent age group and usually present as an avulsion of the unfused apophysis at the level of tendon attachment resulting from violent muscular contraction during an athletic endeavor. Acute apophyseal avulsion injuries are usually easily detected and adequately imaged with radiographs, but occasionally advanced imaging such as magnetic resonance imaging or computed tomography is required to detect and fully delineate the extent of injury. Chronic injuries c

Anatomic Multiple Ligament Reconstructions of the Knee.

BACKGROUND: Multiple ligament injuries of the knee occur in a variety of settings, often from athletic activities. Multiple cruciate and collateral ligament injuries may be associated with hamstring tendon rupture, common peroneal nerve (CPN) injury, meniscus, bone, and cartilage damage. INDICATIONS: After evaluation for concomitant life-threatening and vascular injuries (especially of the popliteal artery), the knee is assessed through a thorough physical examination and imaging series, including varus, valgus, and posterior stress radiography, and magnetic resonance imaging (MRI). Repair of a Primary Patellar Tendon Midsubstance Rupture With Hamstring Autograft Augmentation.

Patellar tendon ruptures are traumatic injuries disrupting the extensor mechanism. Common presentations include patellar alta, knee extension deficits, and a tendon gap. Although nonoperative treatment can be considered for partial tears or microtearing, full patellar tendon ruptures should be treated surgically. Acute treatment is also critical to prevent the patella from scarring in a proximal position. This technique describes an augmented repair of an acute patellar tendon midsubstance tear. Hamstring autografts, routed through the tibial tubercle and patella, are used to augment a repair

Open Achilles tendon lacerations.

In contrast to closed Achilles tendon ruptures, open injuries are rarely reported in the literature. This paper provides information about open Achilles tendon wounds that are eventually seen in the Middle East. The reporting unit, Hamad Medical Corporation, is one of the biggest trauma centers in the Gulf area and the major health provider in Qatar. This is a retrospective study including patients admitted and operated for open Achilles tendon injuries between January 2011 and December 2013. Two hundred and five cases of open Achilles tendon lacerations were operated in Hamad General Hospital…

Primary repair of complete Achilles tear augmented with amnion allograft wrap in college basketball player with a history of contralateral Achilles rupture: a case report.

BACKGROUND: The Achilles tendon is the thickest tendon in the human body and is responsible for plantar flexion with muscle contraction. It is able to withstand tensile loads as large as ten times the body's weight or greater at times of peak stress on the tendon. Due to the repetitive and massive stress inflicted on the Achilles tendon, it is prone to injuries, especially in running and jumping athletes. Ruptures typically present after unsustainable stress placed on the tendon from rapid contraction with classic presentation of a sudden "pop". These injuries tend to occur in middle-aged men

Inveterate Section of Deep Flexor in Pediatric Hand, Periosteum Clues.

Diagnostic delay in flexor tendon injuries is common in pediatric patients, and age influences both recovery and postoperative management. We report the case of a three-year-old girl who presented with the inability to flex the distal interphalangeal joint of the left third finger, two months after the digit became entrapped in a chair. Ultrasonography revealed cortical ossification of the middle phalanx compressing the flexor tendon, without clear evidence of discontinuity. Surgical exploration was undertaken due to a strong clinical suspicion of flexor digitorum profundus rupture, revealing …

Absence of flexor digitorum profundus muscle and variation of flexor digitorum superficialis muscle in a little finger: two case reports.

PURPOSE: Knowledge of rare variants of the FDP is of high clinical importance for physicians examining patients for tendon lacerations and especially for hand surgeons operating tendon injuries. METHODS: During routine dissection at our Department of Anatomy both cases were observed. RESULTS: Variations of flexor digitorum superficialis and flexor digitorum profundus muscles of the little finger were observed in two cadavers. In both cases, the flexor digitorum profundus muscle for the little finger was absent. Moreover, in the first case, the flexor digitorum superficialis muscle for the li…

[Modified collateral ligament reconstruction with the tendon of the abductor pollicis tendon in treatment of chronic radial instability of the basal thumb joint].

Injuries to the radial collateral ligament of the MP joint of the thumb occur much less frequently than injuries to the ulnar collateral ligament. Ligament injury is usually a result of direct trauma. The resultant instability to the thumb usually impairs hand function. In acute lesions to the radial collateral ligament, direct repair is often possible. In cases of chronic radial joint instability, secondary ligament reconstruction with a free tendon graft, most commonly the palmaris longus tendon, is usually recommended. We describe another technique using the tendon insertion of the abductor

Closed crush injury of the metacarpophalangeal joint.

Closed crush injuries from direct blows to the dorsum of the hand may produce significant metacarpophalangeal (MCP) joint symptoms or dysfunction. We have treated 11 patients who had chronic pain and swelling, but without extensor tendon subluxation, for an average of 7.4 months. Only two responded to nonoperative treatment. The nine who had exploratory surgery had a consistent anatomic lesion that consisted of a partial, arcuate tear of the sagittal fibers. All the patients improved postoperatively. Cadaver dissections of the dorsal MCP tendon mechanism in 11 fresh specimens (44 MCP joints) d

Extensive Loss of Tibialis Anterior Tendon: Surgical Repair With Split Tendon Transfer of Tibialis Posterior Tendon: A Case Report.

Extensive damage of the tibialis anterior tendon is rare and mainly caused by trauma. Surgical treatment of these injuries can become challenging owing to the limited availability of autogenous graft resources for reconstruction of the defect. In the present case report, we describe a large defect in the midfoot soft tissue after a traffic injury, which included complete loss of the tibialis anterior tendon. The tendon was reconstructed by split tendon transfer of the tibialis posterior tendon without sacrificing function, which was confirmed by the follow-up examination at 6 years after injur

Monitoring Muscle-Tendon Adaptation Over Several Years of Athletic Training and Competition in Elite Track and Field Jumpers.

Differences in muscle and tendon responsiveness to mechanical stimuli and time courses of adaptive changes may disrupt the interaction of the musculotendinous unit (MTU), increasing the risk for overuse injuries. We monitored training-induced alterations in muscle and tendon biomechanical properties in elite jumpers over 4 years of athletic training to detect potential non-synchronized adaptations within the triceps surae MTU. A combined cross-sectional and longitudinal investigation over 4 years was conducted by analyzing triceps surae MTU mechanical properties in both legs via dynamometry an

Ex vivo study correlating the stiffness of the ovine patellar tendon to age and weight.

Tendons play a crucial role in the musculoskeletal system. In humans, tendon injuries, especially chronic tendinopathy, are very common and the patellar tendon is a frequent location for tendinopathy or injuries. The biomechanical characteristics of the patellar tendon, such as elasticity and stiffness, are of paramount importance and constitute major outcome measures in research studies. We aimed to assess whether the stiffness of the healthy ovine patellar tendon changes with age and weight in a population of normal animals. Sixty-eight 'patella-patellar tendon-tibial tuberosity' units from …

The "lateral gutter drive-through" sign: an arthroscopic indicator of acute femoral avulsion of the popliteus tendon in knee joints.

Acute avulsions of the popliteus tendon and the lateral collateral ligament of the femur (peel-off lesion) are common and can both be missed during preoperative assessment and later overlooked during surgery if not specifically sought out. Arthroscopic observation of the direct avulsion sign of the torn end of the popliteus tendon occurs less frequently in patients with acute and subacute injuries. This study describes an arthroscopic sign that detects these tears when they either involve only the popliteal tendon or include the lateral collateral ligament assessed from the lateral gutter of t
