Deactivation of cardiac implantable electronic devices in palliative patients: When and how.

Cardiac implantable electronic devices have transformed medicine as they improve quality of life and prevent premature death. In palliative care settings, deactivation of these devices must be discussed, particularly at end-of-life. In terminally ill patients it is consensual to recommend implantable cardioverter defibrillator deactivation once shocks are frequent and painful. Concerning pacemakers, the decision to deactivate is controversial and it usually is not an option at patients' end-of-life, since in pacing-dependent patients, such low heart rates might induce symptoms of bradycardia,

Delivery of End-of-Life Care in Patients Requesting Withdrawal of a Left Ventricular Assist Device Using Intranasal Opioids and Benzodiazepines.

With the increasing prevalence of the left ventricular assist device (LVAD) in patients with end-stage cardiomyopathies, an increasing number of these patients are dying of noncardiac conditions. It is likely that the palliative care clinician will have an ever-increasing role in managing end of life for patients with LVADs, including discontinuation of LVAD support. There exists a paucity of literature describing strategies for effective delivery of palliative care in patients requesting discontinuation of LVAD therapy. Here, we present a case of a patient with metastatic cancer who requested

Current and emerging practice of end-of-life care in British prisons: findings from an online survey of prison nurses.

BACKGROUND: There are concerns about prisoners and detainees not having equal access to end-of-life care while in prison. There is a lack of existing literature about the standards of end-of-life care in UK prisons. OBJECTIVES: The aim of this study was to investigate the views of current and former prison nurses with regard to the end-of-life care being provided in UK prisons. METHODS: Prison nurses were invited to participate in an online survey and asked about their role in the prison, prisoners' access to healthcare facilities, and any barriers and good practices to end-of-life care. The

Towards a basic drug kit for the dying patient.

PURPOSE OF REVIEW: Awareness of palliative care, including knowledge and skills regarding symptom alleviation via pharmacological and non-pharmacological interventions, is limited in many settings. Lists have been published of recommended drugs for palliative care that include medications for different disease stages, using different modes of administration. RECENT FINDINGS: Recent studies confirm that many of the most common symptoms in dying patients, for example pain, dyspnoea, anxiety, nausea, and delirium, are similar regardless of diagnosis. The classes of drugs most commonly recommende

[Palliative care].

WHO describes palliative care as the approach to patients with incurable illnesses. It covers identification and treatment of pain and other physical symptoms, psychological, social, and spiritual difficulties. A tight cooperation between the family doctor, the hospital (medical clinic with the subspecialists, geriatric and palliative care centre), the Spitex, the social, psychological, and the pastoral workers is needed. The family doctor needs to know much about medication and specific interventions in order to alleviate the patients' symptoms such as pain, breathlessness, cough, difficultie

Cancer pain control by infusion techniques in the home situation in the Northern Netherlands: an innovative project on the use of medical technology in the home situation.

Since 1982, 229 terminally ill cancer pain patients were treated by the administration of epidural opioids. An adequate level of aid was provided by the coordinated efforts of the patients' general practitioner, the district nurse, and the anesthesiologist. Based on 9 yr of experience with this model, a project on quality improvement and organization of cancer pain control by infusion techniques in the home situation was proposed. The purpose of the study is to create a nationwide organization for cancer pain control for terminal patients on a home-care basis. Specially trained ambulatory fiel

Patterns of dignity-related distress at the end of life: a cross-sectional study of patients with advanced cancer and care home residents.

BACKGROUND: To provide effective palliative care in different settings, it is important to understand and identify the sources of dignity-related distress experienced by people nearing the end of life. AIM: To describe and compare the sources of dignity-related distress reported by cancer patients and care home residents. DESIGN: Secondary analysis of merged data. Participants completed the Patient Dignity Inventory (assessing 25 sources of dignity-related distress) and measures of quality of life and depression. SETTING/PARTICIPANTS: A total of 45 adult patients with advanced cancer referr

Impact of Guided Interventions on Terminal Extubation: A Pilot Project.

BACKGROUND: After terminal extubation, more than 30% of patients experience a period of persistent dyspnea. Practices regarding terminal extubation at the end of life vary widely, and evidence for practice superiority is lacking. Critical care nurses are often tasked with making intervention decisions. LOCAL PROBLEM: At a 368-bed acute care hospital, nurses reported a lack of training on managing end-of-life symptoms associated with terminal extubation and the need for a standardized protocol to guide assessment and interventions. The aim of this study was to examine how use of an evidence-ba

Enacting compassionate care at the end-of-life in long-term care: a qualitative interpretive study.

BACKGROUND: Approximately one-quarter of the Canadian population will die in long-term care (LTC) homes, highlighting the importance of improving end-of-life (EOL) care in these settings. There is a growing recognition of the need to integrate palliative approach to care in these settings. Central to this approach is compassionate care, which supports the emotional, psychological, and spiritual needs of residents and their families at EOL. Compassion is fundamental to person-centered care and plays a critical role in shaping how staff and families experience dying, death and bereavement. Compa

Students' perceptions of preparedness for delivering palliative and end-of-life care as they transition to registered nurse.

BACKGROUND: Ensuring that nurses are prepared for the delivery of palliative care is essential to the provision of high-quality palliative and end-of-life care. Education plays a key role in the preparation but may encompass more than skills competence. AIM: To understand student nurses' perceptions of their preparation and experiences of delivering palliative and end-of-life care as they complete their BSc Adult Nursing pre-registration programme. METHODS: Interpretative phenomenological analysis was used to explore student nurses' perceptions of delivering palliative and end-of-life care.

Assisted dying as an ageing issue: centring older people in the debate.

Assisted dying is predominantly a later-life phenomenon, yet debate and policy design rarely centre older adults' perspectives. This commentary reframes assisted dying as an ageing issue, synthesising existing research on how older people understand autonomy, suffering, dementia, dependency and end-of-life choice. While acknowledging safeguarding concerns, including those raised by the British Geriatrics Society, it argues that prevailing legal models focused on terminal prognosis or narrowly defined suffering exclude many older people experiencing cumulative decline or anticipating future los

Antibiotics at life's end: key role in treating end-of-life pneumonia?

INTRODUCTION: Pneumonia is a common occurrence at the end of life (EOL). However, clear definitions and consensual guidelines for managing this condition are lacking. Diagnosing EOL pneumonia and deciding whether to treat it with antibiotics can be challenging. AREA COVERED: This special report provides a narrative review of epidemiological data, diagnostic tools for EOL pneumonia, guidance on antibiotic use, and ethical considerations in this context. Literature from 2000 to 2024 was analyzed using PubMed and Cochrane databases. EXPERT OPINION: At the EOL, respiratory symptoms must be manag

Palliative Care Consultation and End-of-Life Care Among Patients With Esophageal Cancer and Inpatient Mortality.

PURPOSE: Palliative care (PC) is an important facet of treatment for patients with advanced esophageal cancer because of symptom burden, low overall 5-year survival rate, and significant impact on quality of life. This patient population experiences high hospitalization burden. The purpose of this study was to analyze the effect of PC on end-of-life (EoL) hospitalizations and evaluate racial differences in EoL care. METHODS: The National Inpatient Sample (NIS) was queried between 2016 and 2020 for hospitalizations with esophageal cancer that ended with inpatient death. The primary EoL outcome

'It showed me that he sensed I was there. . .', The 'relational' use of an electrophysiological tool for evaluating the level of discomfort of people at the end of life: A reflexive thematic analysis.

BACKGROUND: The Analgesia Nociception Index device may serve as a guide to a person's parasympathetic activity via the heart rate variability. It is used to assess the level of discomfort of non-communicative patients at the end of life. Although the index's acceptability in palliative care have been demonstrated, the healthcare professionals and the patient's relatives did not have direct, continuous access to the monitor screen. AIM: To evaluate how healthcare professionals and patients' relatives in a palliative care unit perceived the use of the Analgesia Nociception Index. DESIGN: A qua

Development and Validation of a Ready-to-Talk Measure for Use in Adolescents and Young Adults Living With Advanced Cancer.

BACKGROUND: In the era of evolving and emerging therapies, adolescents and young adults (AYAs) living with advanced cancer experience a high degree of uncertainty, making palliative care and end-of-life (PCEOL) discussions difficult. Clinical conversations determine values/preferences that guide shared decision-making and goals of treatment, including end-of-life care when cancer progresses. Initiating PCEOL conversations is challenging for clinicians. OBJECTIVE: This study describes the development and validation of an instrument that measures AYA readiness to engage in PCEOL clinical conver

Subcutaneous Tranexamic Acid at the End of Life in a Patient With Hereditary Hemorrhagic Telangiectasia.

Hereditary hemorrhagic telangiectasia (HHT) is an autosomal dominant vascular disorder characterized by vascular malformations, which cause frequent bleeding events including epistaxis and gastrointestinal bleeding.1 We describe the management of an 84-year-old woman with HHT who received end-of-life care on a palliative care ward. When the patient was unable to swallow her tranexamic acid tablets, this was converted to a continuous subcutaneous infusion, which was continued for a week until death. She had no significant bleeding events or site reactions. To our knowledge, this case report is
