Techniques for Extraction Socket Regeneration for Alveolar Ridge Preservation.

BACKGROUND: Alveolar bone undergoes volumetric changes after extraction due to physiologic bone remodeling. The amount of alveolar bone available during prosthodontic treatment can affect the esthetic outcome of the treatment and make implant placement challenging. Socket preservation techniques are advocated postextraction to maintain the bone's vertical and horizontal alveolar bone dimensions and prevent its atrophy. AIM: This review is oriented toward a clinician, describing the different materials and techniques in practice today for socket preservation. REVIEW RESULTS: A variety of meth

Histologic and Clinical Evaluation of Ridge Augmentation of Extraction Sockets with Severe Bone Defects: A Clinical Prospective Study.

PURPOSE: To primarily evaluate the dimensional changes of bone and soft tissue following ridge augmentation in compromised molar regions. The secondary objective was to evaluate the histologic composition of augmented sites. MATERIALS AND METHODS: The study included 27 patients who underwent augmentation of extraction sites with grafts covered by a collagen membrane. CBCT was taken immediately after augmentation (T2) and after 8 months of healing, before implant placement (T3). The width and height of the extraction sites were recorded at extraction (T1) and reentry surgery (T4). A histomorph

A Clinical Classification System for the Treatment of Postextraction Sites.

A postextraction socket is always open to different treatment possibilities. A straightforward clinical classification may help evaluate which surgical approach is best suited for the case being treated. Four different classes are defined on the basis of the local anatomy of the site, available bone volume, and soft tissue level. For every clinical situation, either immediate placement, early placement, alveolar ridge preservation, or staged approach can be selected as a treatment modality according to the classifications listed.

[Recent research advances on alveolar ridge preservation after tooth extraction].

Resorption of alveolar bone that occurs following tooth extraction is irreversible, it may compromise the restoration of implants or conventional prostheses. Ridge preservation can minimize ridge resorption after tooth extraction. In this article, healing features of socket after tooth extraction, factors influencing ridge remodeling, and the use of bioma-terials were reviewed.

What is the Optimum for Alveolar Ridge Preservation?

In the dental implant era, there has been growing interest in exploring the most effective methods to minimize morphologic alteration in the postextraction site. Despite modern methods of ridge preservation, resorption of the ridge is inevitable. The optimal approach to minimizing the rate and amount of these changes is still a subject of controversy. This article provides a contemporary review of the different approaches to preserve alveolar ridge dimensions. A suggested classification for single extraction sockets is presented along with multiple treatment options for each class.

Dimensional changes of the post extraction alveolar ridge, preserved with Leukocyte- and Platelet Rich Fibrin: A clinical pilot study.

OBJECTIVES: This clinical trial explored the clinical and radiographic dimensional changes of the alveolar ridge in the first 4 months after tooth extraction in combination with the application of Leukocyte- and Platelet Rich Fibrin (L-PRF). METHODS: Eighteen single rooted maxillary and mandibular sockets were filled with L-PRF without soft tissue closure. Clinical measurements (bone sounding) were performed using a customized acrylic stent and radiographic measurements were accomplished using Cone Beam Computed Tomography (CBCT), immediately after tooth extraction and after 4 months. Alveolar ridge sockets preservation with bone grafting--review.

INTRODUCTION: Alveolar bone seems to play a key role in providing support to the teeth, which are anchored to the bone by desmodontal fibers. The progressive alveolar bone resorption process occurs due to a loss of anatomic, biologic and mechanical factors. Mechanical stimulation of alveolar bone during mastication is crucial in keeping the teeth and underlying bone healthy. Tooth extraction leads to typical bone deficiency of ridge width and height of alveolar crest and reduces the possibility of placing screw titanium implants. When tooth extraction is necessary, trauma should be minimized d

The role of functional occlusal relationships in temporomandibular disorders: a review.

This review highlights the consensus existing in past research on the role of functional occlusal factors in the pathophysiology of temporomandibular disorders (TMD). The functional occlusal relationships considered are balancing and working occlusal contacts, length and symmetry of retruded contact position-intercuspal position (RCP-ICP) slides, occlusal guidance patterns, parafunction, and dental attrition. Controlled studies fail to demonstrate any association between occlusal interferences and TMD signs or symptoms. Temporomandibular joint condylar autorepositioning secondary to intracapsu

Prosthetic management of implants placed with the socket-shield technique.

Partial extraction therapies, such as the socket-shield technique, use the patient's tooth tissues and periodontium to preserve the alveolar ridge and limit postextraction resorption. Internal exposure through the overlying peri-implant mucosa has been reported as the most common complication, suggesting that the preparation technique requires modification. This technique report describes the prosthetic management of the socket-shield technique, emphasizing preparation of the socket-shield to the bone crest, and the creation of an S-shape prosthetic emergence profile to support maximal soft ti

Clinical evaluation of a modified marginal sulcular incision technique in endodontic surgery.

The authors evaluated periodontal parameters following apical surgery using a new marginal sulcular incision. In 65 cases, surgical access was achieved by means of a sulcular incision technique without the involvement of the adjacent periodontia and the interproximal papillae and in 33 cases by means of a submarginal trapezoidal technique. Periodontal parameters (probing pocket depth, gingival recession, clinical attachment loss, and tooth mobility [periotest]) were recorded at baseline and after 6 months. Scar formation and the loss of papilla height were evaluated photographically. No signif

Assessment of Nanoscale Configuration of Dentin in Non-carious Cervical Lesions.

Introduction Non-carious cervical lesions (NCCLs) are structural deformities or tissue loss in the cervical region of teeth, which are closer to the gum line that do not result from caries (tooth decay). On the tooth enamel at the gingival margin, these lesions are frequently seen as V- or U-shaped notches. The causes of NCCLs are multifaceted and involve intricate relationships between chemical, mechanical, and biological elements. The slow loss of enamel and dentin in the cervical area occurs due to mechanical causes like abrasive toothbrushing and occlusal tension, as well as chemical varia

Treatment of ectopically erupting maxillary permanent first molars with a distal extended stainless steel crown.

The exaggerated mesial eruption of the permanent first molar can result in the tooth becoming impacted, or the premature atypical resorption and exfoliation of the primary molar, with a resultant loss of space. A stainless steel crown with a distal guide plane was placed on a primary molar to correct the aberrant eruption angle of the permanent tooth. This procedure can be used successfully for both partially and totally tissue-impacted teeth, and is described in this report.
