A 14-year-old girl with focal dermal hypoplasia (Goltz-Gorlin-syndrome) presented with dysphagia, hoarseness, inspiratory stridor, intermittent dry cough and a 10% weight loss. Endoscopy showed that these symptoms were caused by papillomatosis of the hypopharynx and the larynx. The papillomatous masses were resected subtotally by endoscopic laser treatment. We report four cases of massive tongue swelling after spinal surgery. Swelling resulted from the position of the spine during surgery (flexed thoracic-cervical position), fixation of the endotracheal tube, additional surgical gauzes packed around the endotracheal tube, and compression of the base of the tongue. Of 27 children treated for acute epiglottitis, 26 survived. Eighteen patients treated by nasotracheal intubation were devoid of sequelae, whereas of six patients in whom tracheotomy was performed, one developed an ugly scar and two had slight tracheal stenosis at the tracheostomy site. HISTORY: A 21-year-old female was injured by accidental strangulation. Dyspnea and stridor occurred with delay, and led to emergency intubation.
FINDINGS: Physical examination showed strangulation marks and neck emphysema. Computed tomography confirmed laryngotracheal separation and revealed misplacement of the ventilation tube.
