Tranexamic acid and convulsive seizures after isolated coronary artery bypass surgery: the role of cardiopulmonary bypass and renal function.

In cardiac surgical patients, tranexamic acid (TXA) has been associated with an increased risk of convulsive seizure (CS). We aimed to investigate whether in patients undergoing isolated coronary artery bypass grafting (CABG) surgery the use of cardiopulmonary bypass (CPB) impacts the risk of CS. We studied 4198 propensity score matched patients. Patients who underwent CABG surgery without CPB, received a single bolus of 1 g TXA. Patients who underwent CABG with CPB, additionally received a TXA dose of 0.5 g in the CPB prime and an infusion of 0.2 g/h until the end of CPB. The risk of CS in th…

Laser-assisted delivery of tranexamic acid for melasma: Pilot study using a novel 1927 nm fractional thulium fiber laser.

BACKGROUND: Melasma can be associated with immense psychosocial stress, which can impact quality of life. One theory suggests that ultraviolet light can increase plasmin activity in keratinocytes, which has led to the investigation of tranexamic acid for treating melasma, since it possesses anti-plasmin properties. The use of laser-assisted drug delivery can also increase the uptake of topical medications. AIMS: A prospective pilot study was performed to assess the utility of combination treatment with low-energy, low-density 1927 nm fractional thulium fiber laser and topical tranexamic acid …

Machine learning for prehospital prediction of trauma-induced coagulopathy: a national evaluation of the potential impact on tranexamic acid administration.

BACKGROUND: Trauma-induced coagulopathy (TIC) is characterized in part by hyperfibrinolysis. Inhibiting fibrinolysis with tranexamic acid (TXA) decreases mortality in trauma patients, with greater benefit the earlier it is given. However, identifying which patients could benefit has been the subject of controversy. Machine learning models that predict TIC early after injury may aid clinician decision-making. The aim of this study was to identify the proportion of patients with different risks of developing TIC based on TIC-Bayesian network (BN) who received TXA within 1 hour of injury. METHOD…

Tranexamic acid and beyond: A systematic review of pediatric post-tonsillectomy hemorrhage protocols and introduction of a universal management guideline.

INTRODUCTION: Tonsillectomy is one of the most common procedures performed in children. A well-known complication of tonsillectomy is post-tonsillectomy hemorrhage (PTH). PTH is a very serious complication due to the potential for airway compromise and severe bleeding. Most hospital protocols call for observation in scenarios where there is no active bleeding and return to operating room in situations where bleeding is not controlled. In recent years, the use of tranexamic acid (TXA), an antifibrinolytic agent, has been incorporated into the management of post-tonsillectomy bleeds, through neb…

Tranexamic acid as an adjunct to resuscitative endovascular balloon occlusion of the aorta does not worsen outcomes in a porcine model of hemorrhage.

BACKGROUND: Non-compressible torso hemorrhage (NCTH) represents a leading cause of preventable mortality in trauma. Resuscitative endovascular balloon occlusion of the aorta (REBOA) stabilizes NCTH but may predispose patients to thrombus generation. REBOA must therefore be prospectively evaluated for coagulation risks with concomitant usage of anti-fibrinolytic tranexamic acid (TXA). Using a porcine model of hemorrhage, it was hypothesized that TXA with REBOA would worsen coagulation outcomes and organ damage. MATERIALS AND METHODS: Thirty-two male Yorkshire swine underwent 30% blood volume h…

Role of Tranexamic Acid in the Management of Chronic Subdural Hematoma: A Systematic Review and Meta-analysis of Randomized Controlled Trials.

Chronic subdural hematoma (cSDH) is a prevalent neurosurgical disorder with increasing incidence, especially among the geriatric population. Although surgical management has been the conventional approach, the optimal strategy remains debated. Tranexamic acid (TXA), an antifibrinolytic agent, has emerged as a potential alternative, but its efficacy and safety require thorough investigation. This systematic review and meta-analysis aimed to evaluate the role of TXA in the management of cSDH, focusing on its impact on recurrence rates, clinical outcomes, and adverse events. Randomized controlled…

Tranexamic Acid in Postpartum Hemorrhage Management: A Multinational Systematic Review of Efficacy and Safety in Both Vaginal and Cesarean Births.

Postpartum hemorrhage (PPH) is a major cause of maternal mortality worldwide, and there is an urgent need for adjuncts to uterotonic therapy. Tranexamic acid (TXA), an agent that inhibits fibrinolysis, has shown promise in surgical and trauma settings, but its role in postpartum hemorrhage prevention and treatment remains unclear. We systematically reviewed six randomized, placebo-controlled trials (total of 54934 participants) in both vaginal and cesarean delivery. Among women with postpartum hemorrhage, tranexamic acid was observed to lower the risk of bleeding-related mortality and reduce t…

Tranexamic Acid Use in Neck of Femur Fractures: Impact on Blood Loss and Clinical Outcomes at a UK Teaching Hospital.

Background Patients with neck of femur (NOF) fractures experience high morbidity and transfusion rates. Tranexamic acid (TXA) routinely reduces perioperative blood loss in elective lower limb arthroplasties; however, its use in fragility fracture management remains variable, and there is no nationally agreed guideline for its use in hip fracture surgery. This study aims to evaluate the impact of TXA administration on transfusion rates, postoperative hemoglobin drop, in-hospital complications, and 30-day mortality in patients with NOF fractures at a UK-based teaching hospital. Methods A retrosp…
