The ResidentialCare Transition Module: a single-blinded randomized controlled evaluation of a telehealth support intervention for family caregivers of persons with dementia living in residential long-term care.

BACKGROUND: Families do not fully disengage from care responsibilities following relatives' admissions to residential long-term (RLTC) care settings such as nursing homes. Caregiver stress, depression, or other key outcomes remain stable or sometimes increase following a relative's RLTC entry. Some interventions have attempted to increase family involvement after institutionalization, but few rigorous studies have demonstrated whether these interventions are effective in helping families navigate the potential emotional and psychological upheaval presented by relatives' transitions to RLTC env

Canadian Cardiovascular Society Consensus Conference guidelines on heart failure--2008 update: best practices for the transition of care of heart failure patients, and the recognition, investigation and treatment of cardiomyopathies.

Heart failure is a clinical syndrome that normally requires health care to be provided by both specialists and nonspecialists. This is advantageous because patients benefit from complementary skill sets and experience, but can present challenges in the development of a common, shared treatment plan. The Canadian Cardiovascular Society published a comprehensive set of recommendations on the diagnosis and management of heart failure in January 2006, and on the prevention, management during intercurrent illness or acute decompensation, and use of biomarkers in January 2007. The present update bui

Maximal Exercise Capacity in Chronic Kidney Disease Stage 4-5 Patients Transitioning to Renal Replacement Therapy or Continuing Conservative Care: A Longitudinal Follow-Up Study.

INTRODUCTION: Chronic kidney disease (CKD) is associated with impaired maximal exercise capacity (MEC). However, data are scarce on the development of MEC in CKD stage 4-5 patients transitioning to renal replacement therapy (RRT). METHODS: We explored the change in MEC measured in watts (Wlast4) with 2 consecutive maximal bicycle stress ergometry tests in 122 CKD stage 4-5 patients transitioning to dialysis and transplantation in an observational follow-up study. RESULTS: Mean age was 58.9 ± 13.9 years and 43 (35.2%) were female. Mean time between the baseline and follow-up ergometry tests w

Can primary care team-based transition to insulin improve outcomes in adults with type 2 diabetes: the stepping up to insulin cluster randomized controlled trial protocol.

BACKGROUND: Type 2 diabetes (T2D) brings significant human and healthcare costs. Its progressive nature means achieving normoglycaemia is increasingly difficult, yet critical to avoiding long term vascular complications. Nearly one-half of people with T2D have glycaemic levels out of target. Insulin is effective in achieving glycaemic targets, yet initiation of insulin is often delayed, particularly in primary care. Given limited access to specialist resources and the size of the diabetes epidemic, primary care is where insulin initiation must become part of routine practice. This would also s

[A Young Male Case of Rectosigmoid Adenocarcinoma with Diffuse Peritoneal Metastasis Managed with Palliative Care and Cell-Free and Concentrated Ascites Reinfusion Therapy to Initiate and Transit to Outpatient Systemic Chemotherapy].

A 30s-year-old male complaining of abdominal distension and pain was referred to our hospital. The support by palliative care team was initiated soon after emergency admission. Colonoscopic examination revealed a circumferential ulcerated tumor of rectosigmoid and the tumor was diagnosed as an adenocarcinoma. Contrast-enhanced CT revealed multiple enlarged lymph nodes, diffuse peritoneal nodules and massive ascites, which led us to the diagnosis of advanced rectosigmoid cancer. Because bowel obstruction was not observed, early initiation of systemic therapy was planned. Although abdominal pain

Essential components of an effective transition from paediatric to adult neurologist care for adolescents with Duchenne muscular dystrophy; a consensus derived using the Delphi methodology in Eastern Europe, Greece and Israel.

PURPOSE: An increasing number of patients with Duchenne muscular dystrophy (DMD) now have access to improved standard of care and disease modifying treatments, which improve the clinical course of DMD and extend life expectancy beyond 30 years of age. A key issue for adolescent DMD patients is the transition from paediatric- to adult-oriented healthcare. Adolescents and adults with DMD have unique but highly complex healthcare needs associated with long-term steroid use, orthopaedic, respiratory, cardiac, psychological, and gastrointestinal problems meaning that a comprehensive transition proc

A pilot study on the effects of the transition of paediatric to adult health care in patients with haemophilia and in their parents: patient and parent worries, parental illness-related distress and health-related Quality of Life.

The aim of this pilot study was to investigate the effects of the transition from paediatric to adult health care services in haemophilia patients and their parents. We compared pretransition children (n = 9) and their parents (n = 18) to posttransition patients (n = 8) and their parents (n = 21). Pre- and posttransition patients did not differ in self-rated health-related quality of life (QoL) or worries about the transition. Fathers of posttransition patients rated their son's QoL as poorer than those of pretransition patients (P = 0.034) and indicated higher levels of illness-related distre

'You've Just Got to Keep Pestering': Barriers and Enablers of Attaining Continuity of Hepatitis C Care for People Transitioning Between Prison and Community Health Services in South-East Queensland, Australia.

Highly effective direct-acting antiviral (DAA) therapies for hepatitis C (HCV) have been available in Australian prisons since 2016. To address treatment interruption following release from prisons, the Queensland Injector's Health Network (QuIHN) launched a Prison Transition Service (PTS) in south-east Queensland, Australia. Presently, the factors associated with continuity of post-release HCV care are poorly understood. The objective of this qualitative study was to explore the barriers and facilitators to HCV treatment among people recently released from prisons among PTS clients and stakeh

Transition from childhood to adolescence: steps to be taken.

PURPOSE OF REVIEW: The aim of this article is to give an overview of recent literature on transitional care of children with complex urological disease. RECENT FINDINGS: Most studies on transitional care concerned spina bifida patients. Assessment of current practices revealed that formalized clinics for transitional care may improve healthcare. However, there is still little consensus among healthcare providers on how to organize transitional care. A widely recognized problem is a lack of interested, dedicated, and well-trained staff for transitional care and lifelong follow-up of complex ur…

Bridging the gap: a qualitative process evaluation from the perspectives of healthcare professionals of an audit-and-feedback-based intervention to improve transition to adult care for young people living with type 1 diabetes.

BACKGROUND: The transition from pediatric to adult care is a vulnerable time for young people living with type 1 diabetes (T1D). Bridging the Gap (BTG) is an audit-and-feedback (AF) intervention aimed at improving both transitions-in-care processes and diabetes management in the year following transition. As part of BTG, we conducted a qualitative process evaluation to understand: (a) what was implemented and how; and (b) the contextual factors (micro-, meso- and macro-) that affected implementation, outcomes and study processes. METHODS: Using qualitative descriptive methodology, interviews

Transition to Practice: Implementing Healthy Work Environment Standards through Nurse Resident-Led Evidence-based Practice Change in Transcatheter Aortic Valve Replacement Care on a Cardiac Telemetry Unit.

Implementing healthy work environment standards helps to improve the care environment for nurses and patients. These standards were used as a framework during a nurse resident-led evidence-based practice project. The transcatheter aortic valve replacement team collaborated with the nurse residents throughout the evidence-based practice project to design a fast-track patient selection checklist and give input into a care protocol for their cardiac telemetry unit.

Numbers, characteristics, and medical complexity of children with life-limiting conditions reaching age of transition to adult care in England: a repeated cross-sectional study [version 1; peer review: 2 approved].

BackgroundThe number of children with life-limiting conditions in England is known to be increasing, which has been attributed in part to increased survival times. Consequently, more of these young people will reach ages at which they start transitioning to adult healthcare (14-19 years). However, no research exists that quantifies the number of young people with life-limiting conditions in England reaching transition ages or their medical complexity, both essential data for good service planning.MethodsNational hospital data in England (Hospital Episode Statistics) from NHS Digital were used

Transition services in 22q11 deletion syndrome: Hit or miss.

INTRODUCTION: 22q11 deletion syndrome (22q11DS) is a rare genetic condition and the most common microdeletion in humans. The symptomatology is broad and variable between individuals. It is usually diagnosed in childhood and requires a multidisciplinary lifelong approach including a structured transitional care process. This study therefore seeks to enrich our understanding of transitional care for individuals with 22q11DS by exploring the insights from healthcare professionals (HCPs) from Europe and Canada with multidisciplinary backgrounds. METHODS: This interview study was part of a larger …

Transition from pediatric to adult healthcare for patients with chronic functional constipation: a scoping review and provider insight.

Chronic functional constipation is complex and often requires a multifaceted treatment approach. Patients are treated by medical and surgical units in outpatient and inpatient settings. Some adolescents will require transition to adult services for ongoing management, but little is understood about the transitional care needs of this cohort. A scoping review of PubMed, MEDLINE and Embase examined transitional care for patients with chronic functional constipation aged 10 to 30 years. Additionally, correspondence was sought from hospitals with specialized Colorectal Centers. Three studies, all …
