Power Doppler ultrasound examination of the intracerebral and extracerebral vasculature.

Power Doppler is a new ultrasound technique based on the visualization of the Doppler power spectrum. In 22 patients with different lesions of brain-supplying vessels the results of power Doppler ultrasound were compared with those of digital subtraction angiography and conventional color Doppler ultrasound. In patients with intracranial high-degree stenoses, power Doppler was largely free of those artifacts that occur with conventional color Doppler (e.g., echoshadowing and aliasing). The vessel lumen was seen in all patients with more than 80% stenoses. Power Doppler was superior to conventi

Transcranial color-coded duplex sonography in cerebral arteriovenous malformations.

BACKGROUND AND PURPOSE: It is well known that significant changes in cerebral hemodynamics may occur during the treatment of cerebral arteriovenous malformations with the complication of intracerebral hemorrhage and parenchymal edema. We used transcranial color-coded duplex sonography to study alterations in blood flow velocities during staged embolization. METHODS: Forty-one patients aged 40 +/- 13 years (mean +/- SD) with angiographically proven cerebral arteriovenous malformations were studied. The blood flow velocities of the anterior, middle, and posterior cerebral arteries were measured

Radial Artery Pseudoaneurysm With Cutaneous Manifestations in a Patient With Neurofibromatosis Type 1.

Neurofibromatosis type 1 (NF1) is an autosomal dominant disorder with a broad spectrum of clinical features, including cutaneous manifestations. Although vascular complications of NF1 are less frequently recognized, they are significant. This report describes a rare case of a radial artery pseudoaneurysm that manifested as a rapidly enlarging erythematous cutaneous nodule in a 43-year-old man with NF1 one year after transradial cardiac catheterization. A clinical examination revealed a pulsatile lesion, and color Doppler ultrasonography and computed tomography angiography confirmed the diagnos

Thoracic outlet syndrome: a rare case with bilateral cervical ribs and bilateral anterior scalene hypertrophy.

Thoracic outlet syndrome (TOS) is a rare neurovascular disorder generally caused by the presence of a cervical rib or hypertrophic scalene anterior muscle that can compress the brachial plexus and/or subclavian vessels. In the vascular form, the symptoms are caused by the compression of the artery and/or the subclavian vein. In the first case, the compression is caused by the cervical rib and leads to hypo-perfusion with cooling and cyanosis of the upper limb, while in the second case, the compression is caused by the anterior scalene muscle and leads to congestion, cyanosis, swelling and pain

Cavernous hemangioma of the tongue: A rare case report.

Hemangiomas are developmental vascular abnormalities and more than 50% of these lesions occur in the head and neck region, with the lips, tongue, buccal mucosa, and palate most commonly involved. They are considered as hamartomas rather than true neoplasms. Here we report a case of hemangioma of the body of the tongue, discussing the diagnostic aspects and treatment modalities of such lesion and emphasizing the role of the color Doppler ultrasonography, especially in the diagnosis and treatment. Factors such as patient's age, size and site of lesion and the proximity of lesion to vital structu

Molar ectopic pregnancy in the uterine cornus.

Cornual pregnancy is a rare form of ectopic pregnancy. The incidence of hydatiform molar pregnancy is 1 in 1000 to 2000 pregnancies. Molar cornual ectopic pregnancy is extremely rare. A 41-year-old nulliparous woman was admitted via the emergency department because of vaginal bleeding for 2 weeks. Transvaginal sonography exhibited a heterogeneous hypoechoic shadow in the endometrium that suggested a hematometra including blood clots and tissue, and a multicystic echogenic mass, with flow at color Doppler ultrasonography, in the lateral wall of the uterus. A laparoscopic cornuostomy was perform

Sirenomelia with oligodactylia: early ultrasonographic and hysteroscopic embryoscopic diagnosis during the first trimester of gestation.

The case we describe reports the early sonographic findings of sirenomelia with oligodactylia at 9 weeks of gestation by transvaginal two-dimensional color Doppler ultrasonography imaging and its confirmation by hysteroscopic embryoscopy at 12 weeks to further characterize the findings. The embryo showed increased nuchal translucency and fused lower limbs with a large intra-abdominal vessel and two-vessel umbilical cords. The pregnancy was terminated by medical abortion induction after the patient viewed the embryoscopic images. Diagnosis is commonly made later in the second trimester of pregn

Magnetic resonance angiography findings of penile Mondor's disease.

A 53-year-old male was admitted to our Emergency Department affected by a contemporary high-flow priapism and induration on the dorsal penile surface, in consequence of a prior transrectal prostate biopsy performed 2 weeks earlier on the basis of a suspicion of prostate cancer. We describe a penile Mondor's disease (penile superficial dorsal vein thrombosis) of uncertain pathogenesis involving the penile superficial vein, and employing a careful diagnostic pathway by using magnetic resonance angiography (MRA). In the literature many reports described pulsed- and color-Doppler ultrasonography c

Role of US in testicular and scrotal trauma.

High-frequency ultrasonography (US) with a linear-array transducer is the modality of choice for the initial evaluation of patients with acute scrotal pain after trauma. Testicular trauma is the third most common cause of acute scrotal pain. US is useful in the triage of patients for medical or surgical management because it reliably depicts tunica albuginea rupture, intra- and extratesticular hematomas, and testicular contusions. Color Doppler US allows direct evaluation of testicular perfusion and detection of uncommon conditions, such as testicular torsion, that may be associated with scrot

Cavernous hemangioma: diffuse enlarged venous spaces within the myometrium in pregnancy.

BACKGROUND: Diffuse enlarged vessels throughout the myometrium are very rare. This case illustrates the diagnosis and clinical management of a pregnancy complicated by large vessels diffusely distributed throughout the myometrium. CASE: A primigravida measured large for dates at 27 weeks gestation. Ultrasonography demonstrated tubular echolucent spaces throughout the myometrium. No flow could be detected within them by color or spectral Doppler. During cesarean delivery blood loss was 1,700 mL, but the uterus was successfully closed with 3 suture layers. A biopsy specimen showed myometrium co

Bleeding gastric varices as a result of splenic vein compression by a celiac arterial aneurysm.

Celiac arterial aneurysms are very unusual and often lack clinical manifestations. According to our review of the literature, this is the first report of a patient with a large celiac arterial aneurysm who exhibited hematemesis from gastric varices. Arteriography, as well as color Doppler ultrasonography and enhanced computed tomography, contributed to the diagnosis of this aneurysm, which was best exposed by a left thoracoabdominal approach. In this patient the lesion was a false aneurysm with perforation of the celiac artery, so simple closure of the orifice was carried out and the revascula

Thrombosed persistent median artery in a medical sonographer.

The use of gray-scale and color Doppler ultrasound as a first-line imaging modality for wrist pain can be advantageous, especially in cases of clinically suspected carpal tunnel syndrome. This report presents the case of a medical sonographer who exhibited acute wrist pain secondary to a thrombosed persistent median artery after excessive stress loading on her scanningwrist. Ultrasound is able to easily diagnose this rare condition, which may at times be associated with carpal tunnel symptoms due to mass effect from the expanded aberrant artery.

Missing decidual Doppler signals as a new diagnostic criterion for placenta accreta spectrum: A case described using superb microvascular imaging.

We present a case of a 34-year-old pregnant woman with a prior cesarean delivery presenting with placenta previa. Placenta previa accreta was diagnosed from missing decidual flow signals using superb microvascular imaging (SMI). At 31 weeks' gestation, B-mode ultrasonography showed that the placenta was attached to the anterior uterine segment, extending over the internal cervical os. In normally appearing myometrium, SMI demonstrated double layers of flow signals underneath the placental basal plate, corresponding to myometrial and decidual flows. The thin myometrium located on the bladder wh

Intrauterine diagnosis and management of fetal goiter: a case report.

Fetal goiter can be the result of maternal hyperthyroidism treated with antithyroid drugs. Polyhydramnios may be the presenting symptom and can be diagnosed prenatally by sonography. We report a case of fetal goiter diagnosed at 30 weeks of gestation and fetal hypothyroidism confirmed by cordocentesis. Intra-amniotic levothyroxine was administered. Onset of preterm labor could not be prevented. The risks and benefits of intrauterine treatment of fetal goitrous hypothyroidism are discussed.

Use of colour duplex imaging to diagnose and guide angioplasty of lower limb arterial lesions.

BACKGROUND: The purpose of this study was to investigate whether colour duplex imaging alone could safely and effectively be used to diagnose lower limb arterial lesions and guide subsequent percutaneous transluminal angioplasty (PTA). METHODS: Patients with discrete lower limb arterial lesions, preferably stenoses, which could be visualized clearly by colour duplex imaging were selected for duplex-guided PTA. Duplex-guided PTA was performed in an operating theatre using conventional balloon catheters. RESULTS: Duplex imaging was used to diagnose and guide PTA of 55 arterial lesions in 50 le

[Transesophageal, anatomic and Doppler echocardiography. Technic, indications, interpretation].

Cardiac ultrasonography completed by the Doppler technique has demonstrated his ability to visualize almost all of the anatomical pathology including description and semi-quantification of pathological blood flows within the heart. However, the physical properties of the ultrasound beam frequently limit the imaging process and Doppler flow studies. Absorption of ultrasound by the chest wall is a major determinant of quality and hence interpretation, together with the depth of the examined zone of interest. It's a common finding that echo-Doppler images are better in children than in obese adul

A novel non-invasive diagnostic marker for arterial erectile dysfunction: cavernosal artery ondulation index measured in the flaccid phase.

BACKGROUND: Arteriogenic insufficiency (AI) is the most common vascular cause of erectile dysfunction (ED) and is traditionally diagnosed using semi-invasive papaverine-induced penile Doppler ultrasonography (P-PDUS). AIM: To evaluate the feasibility and diagnostic accuracy of the cavernosal artery ondulation index (CA-OI) measured during the flaccid penile phase for identifying AI in men with ED. METHODS: This prospective study included 90 men with clinically confirmed ED (International Index of Erectile Function-5 ≤ 25) and suspected vascular etiology. Gray-scale and color Doppler ultrason
