Preliminary evaluation of vitiligo using in vivo reflectance confocal microscopy.

BACKGROUND: Vitiligo is the most common pigmentary disorder with a global incidence from 0.1% to 2% in different geographical areas. Histopathology and histochemistry have shown the reduction of melanocytes in achromic patches, but microscopic changes of lesional and non-lesional skin are still not completely understood. Reflectance confocal microscopy (RCM), based on the different light reflectance index of cutaneous structures, allowed in vivo, en face microscopic evaluation of superficial skin layers with a resolution similar to skin histology. AIM: The purpose of this study was to evaluat

Recent advances in pathophysiology and current management of itch.

The neurophysiology of itch, the dominant symptom of skin disease, has previously received scant attention. Recent advances in the neurophysiology and molecular basis of itch include the use of microneurography to demonstrate the existence of a subset of itch-dedicated afferent C neurons distinct from neurons which transmit pain; use of functional positron emission tomography (PET) and magnetic resonance imaging (MRI) of the brain to reveal an itch-specific activation matrix, and new evidence of a functional "dialogue" between C neuron terminals and dermal mast cells in which recently describe

[Effect of ultraviolet radiation on the immune system and the effect of exogenous photochemoprotecive agents on the ultraviolet radiation induced immunosuppression].

Ultraviolet radiation (UVR) of the UVB- and UVA-region suppresses immunity and in the dose relation manner it induces cutaneous or systemic state of immunotolerance. It was proved by the successful experimental transplantation of a tumor graft to the skin of UV-radiated animals and on the other side by unsuccessful attempts at skin contact-sensitization. UVR induces this condition either by direct inhibition of antigen-presentating function of the Langerhans cells, or indirectly by the stimulation of secretion of immunomodulatory cytokines in keratinocytes. UVR inhibits also functions of the n

Broad-spectrum sunscreens provide better protection from the suppression of the elicitation phase of delayed-type hypersensitivity response in humans.

It is well established that ultraviolet radiation has immunomodulatory effects that may be involved in skin cancer. Recent studies have shown that ultraviolet A radiation (320-400 nm) as well as ultraviolet B (290-320 nm) is immunosuppressive. This means sunscreens that mainly absorb ultraviolet B (protection against erythema) may be less effective in preventing ultraviolet radiation-induced immunosuppression than broad-spectrum products. We have studied the effects of ultraviolet A exposure on the human delayed-type hypersensitivity response and compared the efficacy of sunscreens having diff

Treatment of generalized vitiligo in children with narrow-band (TL-01) UVB radiation therapy.

BACKGROUND: Only a few clinical trials have been performed on the treatment of generalized vitiligo in children. Recently, narrow-band UVB therapy has been reported to be an effective and safe therapeutic option in adult patients with vitiligo. OBJECTIVE: We studied the efficacy and safety of UVB (311 nm) therapy in children with generalized vitiligo and evaluated the effect of the therapy on the quality of life in these children. METHODS: In an open trial, 51 children (20 males, 31 females) with generalized vitiligo were treated twice weekly with narrow-band UVB radiation therapy for the ma

Actinic cheilitis.

Ultraviolet (UV) radiation of the vermilion border of the lips can precipitate recurrent herpetic infections, varying degrees of dysplasia, and squamous cell carcinoma. UV light also can aggravate certain systemic conditions, such as lupus erythematosus, and produce solar urticaria, porphyrias, and drug photosensitivity reactions.

Involvement of membrane attack complex of complement in UV-B-induced acantholysis in pemphigus.

The initial acantholytic process induced by irradiation of UV-B on uninvolved skin of patients with pemphigus was immunohistologically studied using monoclonal antibodies that bind to antigenic determinants present in the C5b9 complex. Membrane attack complex of complement-related antigens in suprabasal intercellular sites could be detected after 5 hours of irradiation, at which time there was no acantholysis. At 24 hours, the staining intensity markedly increased and suprabasal clefts with acantholytic epidermal cells could be seen. In contrast, acantholysis did not develop in the absence of

Alopecia areata including the cilia and brows of two sisters.

A pair of sisters developed total loss of eyelashes and eyebrows as part of the condition known as alopecia areata. The genetic and emotional aspects of this disorder of partial and complete hair loss are discussed.

[Preoperative preparation of patients with thyrotoxicosis].

Transfusion of UV-irradiated autoblood in the complex preoperative preparation of patients with different degrees of thyrotoxicosis facilitates normalization of the protein-forming function of the liver, decreases concentration of thyroid hormones, stimulates erythropoiesis and is an effective means of preoperative preparation and complex treatment of patients with thyrotoxicosis.

An evaluation of fluorescent light sources for use in phototherapy of neonatal jaundice.

An evaluation of fluorescent lamps for use in the phototherapy of neonatal jaundice was undertaken by making spectral irradiance measurements of each lamp. Conclusions are drawn on the usefulness of current methods of measuring exposure dose to radiation from the lamps. UV hazards from the lamps are evaluated.

Narrow-band UV-B phototherapy: an effective and reliable treatment alternative for extensive and recurrent pityriasis versicolor.

PURPOSE: In the previous studies, positive effect of sunlight on seborrheic dermatitis may well be explained by the direct influence of UV-light on the yeasts. Narrow-band ultraviolet-B phototherapy appears to be a very effective and safe treatment option for patients with severe seborrheic dermatitis. We aimed to investigate the efficacy of narrow-band ultraviolet-B phototherapy as an alternative treatment for pityriasis versicolor. MATERIAL AND METHODS: A total of 38 patients diagnosed with disseminated pityriasis versicolor having more than four relapsing episodes within 12 months were tre

No additional effect of topical calcipotriol on narrow-band UVB phototherapy in patients with generalized vitiligo.

BACKGROUND/PURPOSE: There is no definite cure for vitiligo; however, treatment responses with photobiological modalities are quite acceptable. Of all these, narrow-band UVB phototherapy was proposed rather recently. Calcipotriol has been shown to have stimulating activity on melanogenesis besides immunomodulatory and anti-inflammatory effects. This study was performed to determine whether adding topical calcipotriol to narrow-band UVB phototherapy enhances the efficacy of treatment. METHODS: In this prospective, single-blinded (investigator), right-left comparison clinical study, 20 patients

Effects of topical preparations on the erythemogenicity of UVB: implications for psoriasis phototherapy.

BACKGROUND: Topical preparations are sometimes applied before phototherapy without consideration of their potential to block UVB. OBJECTIVE: Our purpose was to examine the ability of topical preparations to block UVB. METHODS: Volunteers pretreated with mineral oil, a clear liquid emollient, 5% crude coal tar, 6% salicylic acid ointment, emollient creams, and petrolatum underwent minimal erythema dose testing. Transmission of UVB through a clear film coated with the preparations was measured. RESULTS: Tars and salicylic acid blocked UVB. Thick application of petrolatum and emollient creams
