The tourniquet revisited as an adjunct to lower limb revascularization.

PURPOSE: The purpose of this study was to evaluate the role and efficacy of the tourniquet in lower limb revascularization. METHODS: During a 3-year period, 195 patients underwent 205 infrainguinal reconstruction operations in the lower extremity. These patients underwent bypass with a tourniquet and inflow occlusion (group 1) or bypass without a tourniquet (group 2). The type of infrainguinal reconstruction, tourniquet ischemia time, blood loss, and complications related to tourniquet use were recorded. A subset of patients underwent serial muscle biopsies. Specimens from calf muscle were ta

Occupational and Patient Radiation Dose and Quality Implications of Femoral Access Imaging During Coronary Angiography.

PURPOSE: Cardiologists often perform angiography of the common femoral artery (CFA) access site to evaluate whether the anatomy is suitable for deployment of a vascular closure device or to assess whether iatrogenic vessel damage has occurred. The choice of acquisition mode has radiation dose implications. The objective of this study was to investigate the influence of the selected type of CFA x-ray imaging mode (fluoro save, cine acquisition and digital subtraction angiography (DSA)) and tube angle on patient and staff dose during coronary angiography. MATERIALS AND METHODS: Assessment of im

Identification of hospital outliers in bleeding complications after percutaneous coronary intervention.

BACKGROUND: Post-percutaneous coronary intervention (PCI) bleeding complications are an important quality metric. We sought to characterize site-level variation in post-PCI bleeding and explore the influence of patient and procedural factors on hospital bleeding performance. METHODS AND RESULTS: Hospital-level bleeding performance was compared pre- and postadjustment using the newly revised CathPCI Registry(®) bleeding risk model (c-index, 0.77) among 1292 National Cardiovascular Data Registry(®) hospitals performing >50 PCIs from 7/2009 to 9/2012 (n=1,984,998 procedures). Effect of a transpositional muscle flap on VEGF mRNA expression in a canine fracture model.

The effect of sepsis on neovascularization in fractures that follows open fractures is important to the understanding of bone and soft-tissue healing. An animal model was designed that mimics the open fracture and the clinical repair of the human, high-energy open fracture. Vascular endothelial growth factor (VEGF) mRNA levels in canine bone samples were determined in samples from days 0 and 7. Canine right tibiae were fractured with a penetrating, captive-bolt device and then repaired in a standard clinical fashion using an interlocking intramedullary nail. Animals were subject to one of the

The Extended Utility of CHA2DS2VASc and HAS-BLED Scores in the Selection for Transcutaneous Left Atrial Appendage Closure.

BACKGROUND AND PURPOSE: Left atrial appendage closure (LAAC) is an option for stroke prevention in atrial fibrillation patients. Randomized studies have demonstrated the effectiveness and safety of LAAC but included patients with an average risk of stroke and bleeding complications. The current study aimed to assess the extended utility of CHA2DS2VASc (congestive heart failure; hypertension; age ≥75 years [doubled]; type 2 diabetes; previous stroke, transient ischemic attack, or thromboembolism [doubled]; vascular disease; age 65 to 75 years; and sex category) and HAS-BLED (hypertension; abnor

Clinical and angiographic results of transluminal extraction coronary atherectomy in saphenous vein bypass grafts.

BACKGROUND: Transluminal extraction coronary (TEC) atherectomy is a relatively new device that has recently been approved by the Food and Drug Administration. Because of its ability to aspirate clot and atheromatous material, TEC atherectomy may be useful in patients with stenoses in saphenous vein bypass grafts. METHODS AND RESULTS: TEC atherectomy was performed on 158 saphenous vein graft lesions in 146 consecutive patients with a mean age of 65 +/- 8 years (78% men). Clinical indications for atherectomy included stable angina (37%), unstable angina (54%), and postinfarction angina after re

Results of the self-expandable BA9 stent for treatment of large angle coronary bifurcation.

Since the advent of coronary angioplasty the treatment of bifurcation lesions has always proved a complex issue resulting in lower angiographic success rates, increased risk of restenosis, higher rates of dissection, myocardial infarction, and acute vessel closure. The advent of coronary stenting reduced the risks, but in-stent restenosis was noted to be frequent at the ostium of the side branch; for this reasons two-stent techniques were developed to try to combat this phenomenon. Novel dedicated stents have recently been developed to provide easier access to the SB and to scaffold more effec

Incidence, Predictors, and Impact of Vascular Complications After Transfemoral Transcatheter Aortic Valve Implantation With the SAPIEN 3 Prosthesis.

Vascular complications (VCs) after transfemoral transcatheter aortic valve implantation (TAVI) have always been reported to occur frequently. Studies addressing VCs have been conducted with older-generation prostheses. We aimed to evaluate the incidence, predictors, and impact of VCs after transfemoral TAVI with the balloon-expandable SAPIEN 3. We report a single-center retrospective analysis of 400 consecutive patients of a prospectively acquired cohort. All patients underwent transfemoral TAVI with SAPIEN 3 between January 2014 and December 2016. VC was defined according to the Valve Academi

Zero-Contrast Percutaneous Left Atrial Appendage Closure: A Large-Scale, Single-Center, Real-World Study.

BACKGROUND: Left atrial appendage closure (LAAC) is a proven stroke prevention method for patients with non-valvular atrial fibrillation (AF) who are not suitable for long-term anticoagulation. However, the use of iodinated contrast agents during the procedure can increase the risk of acute kidney injury (AKI), especially in patients with chronic kidney disease (CKD). This study evaluates the safety and effectiveness of a zero-contrast LAAC technique using transesophageal echocardiography (TEE) and fluoroscopy. METHODS: In this retrospective study, 324 consecutive patients underwent zero-cont

Quality Improvement in the Practice of Blood Transfusions in Transcatheter Aortic Valve Implantation Procedures.

Background Transcatheter aortic valve implantation (TAVI) is a standard procedure for symptomatic severe aortic stenosis and is especially favored in patients with a high surgical risk. Although TAVI is minimally invasive, blood transfusion remains a relevant postoperative concern. Glenfield Hospital routinely cross-matched four units of packed red cells (PRCs) for every TAVI patient. This practice put significant strain on transfusion services and hospital resources with the increased volume of TAVIs performed, from 182 cases in 2021 to 543 in 2024. Changes in data regarding blood transfusion

The Management of Abdominal Evisceration in Tactical Combat Casualty Care: TCCC Guideline Change 20-02.

Historically, about 20% of hospitalized combat injured patients have an abdominal injury. Abdominal evisceration may be expected to complicate as many as one-third of battle-related abdominal wounds. The outcomes for casualties with eviscerating injuries may be significantly improved with appropriate prehospital management. While not as extensively studied as other forms of combat injury, abdominal evisceration management recommendations extend back to at least World War I, when it was recognized as a significant cause of morbidity and was especially associated with bayonet injury. More recent

A novel allograft combined with modified purse-string suture fixation for annulus fibrosus repair: biomechanical evaluation in a porcine lumbar disc model.

BACKGROUND: Annulus fibrosus (AF) defects may permit recurrent nucleus pulposus extrusion and are difficult to repair because of limited vascularity and persistent mechanical loading. Commercial bone-anchored annular closure devices are intended to occlude large post-discectomy defects, whereas tissue-derived grafts may provide an alternative defect-filling strategy. However, the immediate sealing performance of allograft-based AF repair remains incompletely characterized. PURPOSE: This study developed an allograft designed to stably repair AF defects and evaluated its sealing performance in

Left Atrial Appendage Closure With Catheter Ablation vs. Ablation Alone on Outcomes of Atrial Fibrillation in Heart Failure With Reduced Ejection Fraction: A Propensity Score-Matched Analysis.

Background Combining left atrial appendage closure with catheter ablation (LAACCA) has been proposed as a potential approach to improving outcomes by simultaneously addressing arrhythmia and reducing stroke risk. This study compares the in-hospital outcomes of LAACCA vs. catheter ablation (CA) alone for atrial fibrillation (AFib) in patients with heart failure with reduced ejection fraction (HFrEF). Methods We analyzed adult hospitalizations with HFrEF and AFib who underwent LAACCA or CA alone from the 2016-2020 nationwide inpatient sample using validated ICD-10 codes. Propensity score matchin
