Deep left hilar pedicle isolation technique with in situ umbilical portion dissection for recipient hepatectomy in adult living donor liver transplantation.

BACKGROUND: To overcome technical challenges in vascular reconstruction during adult living donor liver transplantation, we routinely dissect the umbilical portion of the portal vein in situ, enabling extensive dissection of the left and middle hepatic arteries. The umbilical portion with the round ligament can also be used as an autologous graft for venous reconstruction. Herein, we present our surgical technique and assess its clinical impact, which has not been systematically evaluated. METHODS: Adult recipients who underwent living donor liver transplantation at our institution from 2018

Interventional treatment for true aneurysm of superior mesenteric artery: A systematic review of the literature on open surgical strategies.

BackgroundTrue aneurysms of the superior mesenteric artery (SMAA) are rare but clinically significant due to the risk of severe complications such as mesenteric ischemia and fatal hemorrhage upon rupture. While the European Society for Vascular Surgery (ESVS) recommends endovascular repair as the first-line treatment, open surgery remains a valuable option when endovascular repair is not feasible due to clinical or technical constraints.MethodA systematic literature review was conducted covering the period from 2010 to 2024. Twenty-five surgically treated SMAA cases were identified. Data were

Revascularization of popliteal and below-knee arteries with polytetrafluoroethylene.

The expanded polytetrafluoroethylene (PTFE) graft (Gore-tex) is the most frequently used synthetic graft when an alternative to autologous saphenous vein is required. Early results have been encouraging. In the present paper we report on 6 years of results from 153 above-knee (AK) femoropopliteal bypass grafts, 74 below-knee (BK) femoropopliteal bypass grafts, and 54 femorotibial/peroneal bypass grafts. The main indication for the vascular reconstruction was severe ischemia. Preventive antibiotics were given to 95% of the patients. Minimum observation time was 1 year after implantation. There

Accurate and continuous ultrasonography evaluation of small diameter vascular prostheses in vivo.

There is a large clinical requirement for novel vascular grafts; however, the development of novel vascular grafts has not been extremely successful to date. The most successful method for the continuous evaluation of vascular grafts in vivo remains unclear. Therefore, an optimal successive, non-invasive imaging modality is necessary for the study of vascular transplantation. In the present study, a common rabbit model of carotid artery defect was utilized. The patency and hemodynamic characteristics of implanted grafts was examined following surgery by color Doppler ultrasound in three modes,

Prospective Evaluation of Radiocephalic Arteriovenous Fistula Outcomes in a High-Volume Centre: Patency, Complications, and Influencing Factors.

Introduction Despite strong recommendations by the National Kidney Foundation (NKF) for the creation of radiocephalic arteriovenous fistula (RC AVF), most volume centres are reluctant due to contradictory complication rates and sustained patency. Factors influencing outcomes warrant attention, emphasising the importance of tailored approaches. Multicentre studies are recommended for broader validation of these findings. Therefore, the current study aimed to evaluate the outcome of RC AVF in our setup and to identify factors that influence its outcome. Method The prospective observational study

The RADAR technique in reconstruction of failed autologous arteriovenous fistulas due to juxta-anastomotic stenosis is equivalent to that with traditional surgery in maintenance hemodialysis patients.

OBJECTIVE: Autologous arteriovenous fistula (AVF) is recommended as superior vascular access for hemodialysis but has a high rate of failure, and juxta-anastomotic stenosis (JAS) is one of the predominant causes of fistula failure. The aim of this study was to compare the primary patency in reconstruction of failed AVFs due to JAS between the radial artery deviation and reimplantation (RADAR) technique and traditional surgery (end-vein to side-artery neo-anastomosis) in maintenance hemodialysis (MHD) patients. METHODS: A total of 1215 MHD patients with failed AVF were enrolled in this retrosp

Treatment of Aorto-iliac and Infrainguinal Vascular Infections with a Prefabricated Bovine Pericardial Graft.

BACKGROUND: The use of biological grafts provides acceptable mid- and long-term results in native or prosthetic vascular infections. Several reports describe the successful use of bovine pericardium in case of vascular infections, mainly as a large patch to be sutured as a tubular graft. Recently, a novel prefabricated bovine pericardium graft (Biointegral Surgical No-React® Inc, Mississauga, ON, Canada) has been introduced in clinical practice with promising results. In this study, we report our preliminary experience utilizing Biointegral Surgical graft in case of native and or prosthetic ao

Vasoregression linked to neuronal damage in the rat with defect of polycystin-2.

BACKGROUND: Neuronal damage is correlated with vascular dysfunction in the diseased retina, but the underlying mechanisms remain controversial because of the lack of suitable models in which vasoregression related to neuronal damage initiates in the mature retinal vasculature. The aim of this study was to assess the temporal link between neuronal damage and vascular patency in a transgenic rat (TGR) with overexpression of a mutant cilia gene polycystin-2. METHODS: Vasoregression, neuroglial changes and expression of neurotrophic factors were assessed in TGR and control rats in a time course. …

Interleukin-1 alpha as a factor in occlusive vascular disease.

The purpose of this study was to examine the recognized ability of interleukin-1 alpha (IL-1 alpha) to alter the functional properties of endothelial cells and to induce replication of smooth muscle and fibroblasts. Such changes could potentially link IL-1 alpha pathogenetically to the myointimal proliferation of vascular sclerosis. Using a peroxidase-immunoperoxidase immunohistochemical method, saphenous veins and internal mammary arteries were examined for the presence of IL-1 alpha before their implantation as aortocoronary bypass grafts. Occluded saphenous vein grafts requiring replacement

Successful prosthetic graft replacement for middle aortic syndrome in a 9-year-old child.

Middle aortic syndrome is a rare vascular disorder characterised by segmental narrowing of the distal thoracic aorta and/or proximal abdominal aorta, often accompanied by renal artery involvement. The condition may arise from genetic abnormalities, acquired factors, or idiopathic causes. In this study, we report the case of a 9-year-old boy who was found to have hypertension during a routine school health examination. Computed tomography angiography confirmed the diagnosis of middle aortic syndrome, revealing long-segment stenosis of the distal thoracic aorta. Following a comprehensive assessm

Tissue-engineered blood vessels for clinical translation: Design logic, representative advances and persistent barriers.

Tissue-engineered blood vessels are promising alternatives for small-diameter vascular reconstruction, but clinical translation remains limited by thrombosis, compliance mismatch, delayed endothelialization and unstable host remodeling. This review synthesizes representative advances in scaffold design, seed-cell selection, bioactive regulation and biofabrication, emphasizing how these components interact under hemodynamic load. Rather than cataloguing technologies, we argue that translational success depends on matching material architecture, cellular phenotype, immune remodeling and manufact

Successful Treatment of a Subclavian Artery Stenosis With a Coronary Bioresorbable Vascular Scaffold.

PURPOSE: To report the use of a coronary bioresorbable vascular scaffold to treat subclavian artery disease. CASE REPORT: A 58-year-old man was admitted for left subclavian steal syndrome. Angiography showed significant left subclavian stenosis in proximity to the ostium of the left vertebral artery. To maximize radial support and minimize restenosis risk while avoiding the chance of vertebral compromise, a coronary bioresorbable vascular scaffold (3.5×28 mm) was implanted after predilation. The device was postdilated with a 1.0-mm oversized balloon, with immediate improvement in hemodynamics

Management of vascular access inflow-outflow imbalance: A bimodal approach.

A more accurate descriptive and clinically useful diagnosis based upon pathophysiology for what is commonly referred to as venous outflow stenosis is inflow-outflow imbalance. In these cases, the total outflow capacity of the AV access is inadequate to handle the inflow volume (Qa) without an increase in pressure. The relative inadequacy of the access outflow capacity in comparison to Qa results in increased outflow resistance and a proportional increase in intraluminal pressure. The clinical indicators associated with venous stenosis are the resulting manifestations of this imbalance. The poi
