2021 American College of Rheumatology/Vasculitis Foundation Guideline for the Management of Antineutrophil Cytoplasmic Antibody-Associated Vasculitis.

OBJECTIVE: To provide evidence-based recommendations and expert guidance for the management of antineutrophil cytoplasmic antibody-associated vasculitis (AAV), including granulomatosis with polyangiitis (GPA), microscopic polyangiitis (MPA), and eosinophilic granulomatosis with polyangiitis (EGPA). METHODS: Clinical questions regarding the treatment and management of AAV were developed in the population, intervention, comparator, and outcome (PICO) format (47 for GPA/MPA, 34 for EGPA). Systematic literature reviews were conducted for each PICO question. The Grading of Recommendations Assessme…

[Vasculitis-related acute abdomen in systemic lupus erythematosus--ultrasound appearances in lupus patients with intra-abdominal vasculitis].

Although an acute abdomen often occurs in lupus patients, definitive treatment is commonly delayed due to the difficulty in diagnosis. We performed an abdominal ultrasound (US) in two patients with severe abdominal pain and recognized mural thickenings of intestinal walls. In the first case high dose prednisone was given for severe abdominal pain, with subsequent perforation of sigmoid colon. Pathological specimens showed a thrombotic vasculitis in the mesenteric artery and arteriole of intestinal walls. In the second case physical examination elicited a rebound tenderness. An abdominal X-ray …

Illustrated histopathologic classification criteria for selected vasculitis syndromes. American College of Rheumatology Subcommittee on Classification of Vasculitis.

We describe the histopathologic criteria for the diagnosis of 7 selected vasculitis syndromes: polyarteritis nodosa, Churg-Strauss syndrome, Wegener's granulomatosis, hypersensitivity vasculitis, Henoch-Schönlein purpura, giant cell (temporal) arteritis, and Takayasu arteritis. The criteria apply to the stereotypical cases in each category; they were formulated after a review of submitted biopsy material from 278 of the 1,000 patients entered into the American College of Rheumatology Vasculitis Study Registry, and from prior experience with the pathologic diagnosis of vasculitis in 1,079 nonre…

From Pyelonephritis to Vasculitis: A Challenging Diagnosis of Renal-Limited Vasculitis Presenting As Acute-on-Chronic Renal Failure.

Renal-limited vasculitis (RLV) is a rare form of anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis. The kidneys are the primary, and often the only, organs affected by this disease. A 66-year-old white male presented with hematuria and acute-on-chronic renal failure. Computed tomography (CT) of the abdomen and pelvis revealed left-sided pyelonephritis/cystitis. Urinalysis showed elevated white blood cell (WBC) and red blood cell (RBC) counts, with 1+ bacteria and 3+ proteinuria. Serum creatinine was 5.6 mmol/L, and estimated glomerular filtration rate (eGFR) was 10 mL/min/1.73 …

Red blood cell distribution width as a useful indicator to predict systemic vasculitis in patients with cutaneous vasculitis.

Cutaneous vasculitis can be limited to skin or a manifestation of primary systemic vasculitis. However, there are no definite markers to predict systemic involvements. Recent studies have shown that a higher red blood cell distribution width (RDW) is associated with disease activity in various disorders. We evaluated whether RDW can be used as an indicator for predicting systemic disease in patients with initial cutaneous involvements. We reviewed clinical and laboratory information of 143 patients with cutaneous vasculitis and 15 pigmented purpuric dermatosis patients seen at single academic …

Effect of anti-interleukin-5 antibody on development of vasculitis in an ovalbumin-induced eosinophilic vasculitis mouse model.

BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare and intractable chronic disease. Glucocorticoids are the mainstay of treatment for EGPA. The drugs that target interleukin (IL)-5 signaling have also been marketed or developed in recent years. While no animal model can completely recapitulate EGPA, the ovalbumin (OVA)-induced eosinophilic vasculitis mouse model exhibits pathological similarities in the lungs. However, the effect of an anti-IL-5 drug has not yet been investigated using this model. This study used the OVA-induced eosinophilic vasculitis model to evaluate…

Efficacy and safety of telitacicept in children with IgA vasculitis and IgA vasculitis nephritis: a single-center retrospective study.

BACKGROUND: Immunoglobulin A vasculitis (IgAV) is the most common childhood vasculitis and can lead to immunoglobulin A vasculitis nephritis (IgAVN) in severe cases, potentially progressing to kidney failure in a subset of children. Safer and more effective treatments are needed to improve outcomes in these children. This study aimed to evaluate the efficacy and safety of telitacicept in the treatment of children with IgAV and IgAVN. METHODS: This is a single-center, retrospective observational study of twenty four children with IgAV or IgAVN who received telitacicept treatment, and thirty ma…

Investigating the forensic pathological characteristics of systemic vasculitis: A retrospective study of 20 deaths caused by systemic vasculitis.

Systemic vasculitis (SV) is a condition characterized by vascular inflammatory disease that often involves the medium and small arteries of various organs throughout the body. SV is difficult to diagnose due to the diversity of clinical symptoms and manifestations, and only tissue biopsy is of great significance. Even so, complications or secondary lesions of SV can also lead to death. In forensic medicine, we can often observe multiple vasculitis in histological observations, which is easily overlooked as a primary cause of death in the final diagnosis. Twenty SV cases were registered in our …

Infliximab-Associated IgA Vasculitis Masquerading as Crohn's Disease Relapse: Leukocytoclastic Vasculitis Confirmed on Duodenal Biopsy - A Case Report.

INTRODUCTION: Tumor necrosis factor (TNF)-α inhibitors are widely used in the management of inflammatory bowel disease (IBD), yet they can rarely provoke paradoxical immune-mediated complications such as vasculitis. CASE PRESENTATION: We describe the case of a 38-year-old man with ileocolonic Crohn's disease (CD) in remission during infliximab (IFX) maintenance therapy who presented with fever and diarrhea, raising the initial concerns of disease relapse. He subsequently developed severe abdominal pain, melena, and palpable purpura on his lower legs. Laboratory tests revealed elevated serum I…

Vasculitis working group: selected unanswered questions related to giant cell arteritis and anti-neutrophil cytoplasmic antibody-associated vasculitis.

Evidence to guide assessment and management of patients with vasculitis is lacking for many important clinical questions. The evidence surrounding several common questions about management of vasculitis was reviewed. Patients with giant cell arteritis (GCA) are at risk for developing extra-cranial large vessel inflammation. Clinicians should be aware of this complication and search for large vessel involvement in patients with GCA who have ischemic symptoms. Research is needed to define optimal strategies to identify patients with such complications. Because of the hazards of chronic corticost…

An update on the pharmacotherapy of ANCA-associated vasculitis.

INTRODUCTION: Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV) are rare, life-threatening autoimmune conditions requiring complex pharmacological management. Contemporary strategies have transitioned from empirical cytotoxic therapy toward mechanistically targeted approaches, reflecting a deeper understanding of B-cell immunity and the alternative complement pathway. This review synthesizes current guidance and presents recent evidence on treatment modalities and emerging options. AREAS COVERED: This paper reviews and synthesizes current international guidance from the

Translational approaches to the study of eosinophils in vasculitis.

Eosinophil basic biology, including immunoregulatory functions, plays a complex role in a myriad of disorders where eosinophils play a role. Established in vivo and in vitro models and novel emerging techniques for studying eosinophils are integral to parsing the involvement of eosinophils in the pathogenesis of disorders including vasculitis. Knowledge of translational approaches from eosinophil-associated diseases and their application to eosinophilic vasculitis are highlighted. It is becoming increasingly evident that the roles of eosinophils in disease are complex. The use of targeted biol…

The Ulcerative Effects of Levamisole-Induced Vasculitis.

Levamisole-induced vasculitis is an autoimmune vasculitis that is most commonly associated with the use of levamisole-adulterated cocaine. We present a case of a 35-year-old undomiciled woman who was admitted to the inpatient psychiatric unit for drug detoxification and evaluation of bilateral lower extremity ulcerations. Her clinical presentation, along with a history of cocaine use, raised suspicion for levamisole-associated vasculitis. Symptomatic resolution and cutaneous healing in these cases are typically observed within 2-14 months following sustained abstinence from cocaine. This case …
