Obstructive Hypertrophic Cardiomyopathy and Takotsubo Syndrome: How to Deal With Left Ventricular Ballooning?

Currently, there are 2 proposed causes of acute left ventricular ballooning. The first is the most cited hypothesis that ballooning is caused by direct catecholamine toxicity on cardiomyocytes or by microvascular ischemia. We refer to this pathogenesis as Takotsubo syndrome. More recently, a second cause has emerged: that in some patients with underlying hypertrophic cardiomyopathy, left ventricular ballooning is caused by the sudden onset of latent left ventricular outflow tract obstruction. When it becomes severe and unrelenting, severe afterload mismatch and acute supply-demand ischemia app

Modified Lecompte procedure for the anomalies of ventriculoarterial connection.

BACKGROUND: The Lecompte procedure for correcting transposition of the great arteries has an advantage because it obviates the need for an extracardiac conduit for the reconstruction of the pulmonary outflow tract. In this study, we evaluated the effectiveness and the application of the Lecompte procedure based on our experiences. METHODS: A retrospective review was conducted of the records of 45 patients who underwent the Lecompte procedure during the past 11 years to achieve direct right ventricle to pulmonary artery continuity. Mean age at operation was 2.4+/-1.7 years (range 3.5 months to

Hypertrophic cardiomyopathy with midventricular obstruction and apical aneurysm: a case report.

A 71-year-old woman had hypertrophic cardiomyopathy associated with midventricular obstruction and an apical aneurysm in the left ventricle. She had had abnormal electrocardiograms for more than 30 years and for the past year had been suffering from occasional attacks of dizziness and low systemic blood pressure. Holter 24-h electrocardiographic monitoring revealed ventricular paroxysmal contractions (676/day) with nonsustained ventricular tachycardia. Doppler echocardiography revealed paradoxical jet flow from the apical aneurysm to the left ventricular outflow during early diastole. Magnetic

Variations in clinical presentation of Fallot's tetralogy in infancy. Angiographic and pathogenetic implications.

Striking variability has been observed in the presenting features in infancy of patients subsequently shown to have tetralogy of Fallot. Some patients presented with severe cyanosis in the neonatal period while others had a systolic murmur and cyanosis only on crying. In these latter patients cyanosis became present at rest over the subsequent months. Yet others presented with episodic attacks of unconsciousness, and a final group presented with dyspnoea and heart failure, accompanying a left-to-right shunt. Angiography demonstrates corresponding variations in the anatomy of the ventricular ou

The Konno-Rastan Procedure in the Current Era: Still A Feasible Option?

INTRODUCTION: The Konno-Rastan procedure is a commonly used surgical technique to address complicated subaortic stenosis and to enlarge the aorta via an anterior route. The objective of this report is to describe the experience of our unit with this procedure. METHODS: Between 2002 and 2022, we identified pediatric patients who underwent the Konno-Rastan procedure. Relevant information was retrospectively collected. RESULTS: There were 16 patients who underwent the Konno-Rastan procedure. The median follow-up was nine years (4 - 16 years), and there was no operative mortality. All patients h

Use of β-blockade and levosimendan for separation from extracorporeal life support in an infant with postoperative diastolic dysfunction.

Diastolic dysfunction is common in infants and neonates with left ventricular (LV) outflow tract obstruction and may lead to low-cardiac output in the postoperative period. We present a management strategy for severe postrepair diastolic dysfunction in an infant with critical congenital aortic stenosis and LV hypertrophy, employing β-blockade and levosimendan.

Chronic renal failure in an English bull terrier with polycystic kidney disease.

An entire female English bull terrier, aged five years and one month, was diagnosed with polycystic kidney disease by renal ultrasonography. It had thickening and abnormal motion of the mitral valve on 2D and M mode echocardiography, and left ventricular outflow tract obstruction, characterised by turbulence in the left ventricular outflow tract and elevated aortic blood flow velocity, detected by colour flow and spectral Doppler echocardiography, respectively. Two years later, haematology, serum biochemistry and urinalysis data suggested the presence of compensated renal failure. The dog was

Transthoracic echocardiography of ruptured chorda tendinea.

The discovery of a left ventricular mass obliges the clinician to perform a differential diagnosis including tumour or lipoma versus thrombus. The discovery of a left ventricular outflow tract obstruction by a mitral mass also obliges the clinician to perform a differential diagnosis including vegetations versus rupture of the chordae tendineae and its assessment also presents important clinical implications. Echocardiographic diagnosis of rupture of the chordae tendineae may be difficult to make. We present a case of ruptured chorda tendinea. Also this case focuses attention on differential d

Ventricular thrombus and subarachnoid bleeding during support with ventricular assist devices.

We report the case of a 23-year-old man with acute aortic valve insufficiency caused by endocarditis, who after emergency aortic valve replacement developed biventricular heart failure. The heart failure was treated with temporary assist devices. Subarachnoid bleeding and thrombus obstruction of the left ventricular outflow tract was detected. The postoperative course is presented with special emphasis on management of subarachnoid bleeding and the simultaneous use of anticoagulation necessary for ventricular assist devices.

Surgical strategies to address re-operative complex left ventricular outflow tract and thoracic aortic pathology: Cleveland Clinic children's experience.

BACKGROUND: Complex patients requiring operations on the left ventricular outflow tract, aortic valve, or thoracic aorta after previous repair of aortopathy constitute a challenging group, with limited information guiding decision-making. We aimed to use our institutional experience to highlight management challenges and describe surgical pearls to address them. METHODS: Forty-one complex patients with surgery on the left ventricular outflow tract, aortic valve, or aorta at Cleveland Clinic Children's between 2016 and 2021 following previous repair of aortic pathology were retrospectively rev

[Hypertrophic obstructive cardiomyopathy with ruptured mitral chordae tendineae: a case report].

A 61-year-old man was treated medically under a diagnosis of hypertrophic obstructive cardiomyopathy 7 years previously. He was transferred to our hospital because of sudden onset of dyspnea. Chest radiography showed marked pulmonary congestion. Echocardiography revealed hypertrophic obstructive cardiomyopathy, with a left ventricular pressure gradient of 120 mmHg, accompanied by severe mitral regurgitation due to ruptured chordae tendineae. Mitral valve replacement and the Morrow operation were performed. After the operation, no left ventricular outflow pressure gradient was detected by echoc

Management of Interrupted Aortic Arch.

The introduction of prostaglandin E1 in the late 1970s revolutionized the management of interrupted aortic arch. Complete resuscitation should proceed over several days if necessary before surgery is undertaken. One-stage primary neonatal repair with direct arch anastomosis and ventricular septal defect closure is the preferred surgical approach. Selective cerebral perfusion with near-infrared monitoring is being used with increasing frequency. Although repair of interrupted arch is physiologically corrective, it should not be viewed as fully corrective because of the high incidence of importa

Why is the patient still symptomatic after myectomy for HCM!

We describe the case of a young female patient with hypertrophic cardiomyopathy (HCM) with a dual chamber implantable cardioverter defibrillator (ICD), and complete heart block from a prior septal ablation procedure. Due to persistent left ventricular outflow tract (LVOT) obstruction and symptoms, she underwent surgical myectomy. Her exertional symptoms continued despite improved LVOT flow parameters. A comprehensive evaluation, including ICD interrogation, helped arrive at the correct diagnosis and treatment plan.

Echocardiographic assessment of takotsubo cardiomyopathy: beyond apical ballooning.

It has been >25 years since the first report of the takotsubo cardiomyopathy (TC). Although left ventriculography was originally used to depict its typical and impressive wall motion abnormality mimicking "takotsubo", or octopus pot, echocardiography plays a pivotal role in detecting not only its left ventricular (LV) wall motion abnormality, apical ballooning, but also various other findings. First of all, apical ballooning is not an essential finding for TC anymore. Mid-ventricular LV asynergy with or without apical involvement is a basic pattern of the LV wall motion abnormality. Reflections of inflections in hypertrophic cardiomyopathy.

The shape of Doppler velocity tracings in obstructive hypertrophic cardiomyopathy offers insights into its pathophysiology. Inflection points are the points on a curve where its shape changes from concave to convex, or vice versa. These dynamic systolic abnormalities are caused: 1) by the amplifying nature of the obstruction; and 2) by the adverse effect of the sudden imposition of afterload in midsystole. The midsystolic drop in left ventricular ejection velocities and the premature termination of longitudinal shortening are compelling evidence of the deleterious mechanical effect of obstruct

[Cardiac pacing in cardiomyopathy].

Pacing is useful not only for therapies of arrhythmias but also for other cardiac disorders. In this article, the pacing therapies for idiopathic cardiomyopathies were reviewed. AV sequential pacing has been reported as beneficial methods to improve symptoms of hypertrophic obstructive cardiomyopathy(HOCM) patients, but the mechanisms and expectancy of efficacy of pacemaker implantation are not clarified. Prospective double blind studies to see the efficacy of pacing on HOCM patients were done and reported in the two continents, one was PIC study in Europe and the other was M-PATHY in the nort

The Egg Muffin Technique-A Novel Bipatch Enfolding Repair for Membranous Ventricular Septal Aneurysm with Aortic Valve Replacement: A Case Report.

A 70-year-old man with severe aortic stenosis was referred to our department for surgery. Echocardiography revealed a peak/mean pressure gradient of 101/61 mm Hg and a membranous ventricular septal aneurysm protruding into the right ventricular outflow tract without shunt flow. Right heart catheterization demonstrated right ventricular outflow tract obstruction with a peak pressure gradient of 11 mm Hg. Aortic valve replacement using a bioprosthetic valve and enfolding repair of the membranous ventricular septal aneurysm were performed. The folded aneurysmal tissue was compressed and reinforce

[Hypertrophic obstructive cardiomyopathy and double-chamber pacing. Long-term results in a consecutive series of 22 patients].

The authors report their experience with dual-chamber pacing in hypertrophy obstructive cardiomyopathy. 22 patients (14 women and 8 men) mean age 60 +/- 13 years were implanted between 1992 and 1998. The criteria for pace-maker implantation were the presence of severe symptoms related with hypertrophy obstructive cardiomyopathy (dyspnea, angina, syncope) and left ventricular outflow tract gradient at mean 30 mmHg. Before pacing, all patients received a medical therapy which included beta-blockers or calcium inhibitors. This treatment was considered as ineffective or responsible of side effects
