Outcomes of Single-Sided Thoracic vs. Bilateral Thoracic and Lumbar Anterior Vertebral Body Tethering in Lenke 1 Curves with Lumbar C Modifier.

STUDY DESIGN: Retrospective review of a prospectively collected single-center adolescent idiopathic scoliosis (AIS) database. OBJECTIVE: To evaluate the outcomes of single-sided thoracic anterior vertebral body tethering (VBT) versus bilateral thoracic/thoracolumbar VBT for the treatment of Lenke 1C type curves, as well as the outcomes of Lenke 1A/B curves compared to 1C curves. BACKGROUND: Lenke 1C curves can be successfully treated with selective thoracic fusion alone. However, there are few data to support whether this classification scheme holds true for VBT in Lenke 1C curves. MATERIAL…

Strategic Use of Single-Level Vertebral Body Sliding Osteotomy Within a 360-Degree Fusion Construct for the Cervical Ossified Posterior Longitudinal Ligament.

Cervical myelopathy caused by ossification of the posterior longitudinal ligament (OPLL) presents significant surgical challenges, especially in multilevel disease with kyphosis or high canal-occupying ratios. While anterior cervical discectomy and fusion (ACDF) and anterior cervical corpectomy and fusion (ACCF) are commonly used, both carry notable risks, particularly in patients with poor bone quality. Vertebral body sliding osteotomy (VBSO) is a novel technique that enables anterior decompression by translating the vertebral body without resection, thereby minimizing dural manipulation and …

Apical stress redistribution during anterior vertebral body tethering for thoracic adolescent idiopathic scoliosis: a finite element analysis of a novel surgical technique.

PURPOSE: Apical stress redistribution (ASR) is proposed to mitigate failure risks after anterior vertebral body tethering for adolescent idiopathic scoliosis. It consists in releasing set-screws at peri-apical levels following curve tensioning to redistribute stresses within the construct. This study determines the biomechanical impact and curve correction obtained with ASR. METHODS: Finite element models of anterior vertebral body tethering were constructed for three typical scoliotic patients with Lenke 1 curves. ASR was simulated by releasing tension on the cable at the level of the three …

Vertebral Body Tethering in 49 Adolescent Patients after Peak Height Velocity for the Treatment of Idiopathic Scoliosis: 2-5 Year Follow-Up.

Vertebral Body Tethering (VBT) is a non-fusion surgical treatment for Adolescent Idiopathic Scoliosis (AIS) that elicits correction via growth modulation in skeletally immature patients. VBT after peak height velocity is controversial and is the subject of this study. A retrospective review of Risser 3−5 AIS patients treated with VBT, and min. 2-year FU was performed. Pre to post-op changes in clinical outcomes were compared using Student’s t-test or the Mann-Whitney test. A total of 49 patients met criteria, age 15.0 ± 1.9 years, FU 32.5 ± 9.1 months. For thoracic (T) major curvatures, T curv…

Children with Spinal Muscular Atrophy Have Reduced Vertebral Body Height and Depth and Pedicle Size in Comparison to Age-Matched Healthy Controls.

BACKGROUND: Most children with spinal muscular atrophy (SMA) develop spinal deformity, which may require surgical intervention. In addition to poor bone stock, vertebral body shape may hinder the placement of spinal implants resulting in complications and poor outcome. The aim of this study was to analyze whether vertebral body morphology of children and adolescents with SMA is altered in comparison to healthy age-matched controls. METHODS: In this prospective cohort study, 17 children with SMA (mean age 8.7 ±1.0 years) and 13 adolescents with SMA (mean age 13.6 ±1.4 years), all with some deg…

Cerebral autoregulation in the vertebral and middle cerebral arteries during combine head upright tilt and lower body negative pressure in healthy humans.

The majority of cerebral autoregulation research has focused on the middle cerebral artery. However, many symptoms of presyncope indicate posterior cerebral hypoperfusion. To address this issue, we measured cerebrovascular reactivity, cerebral blood flow velocity and dynamic cerebral autoregulation in the middle cerebral artery and vertebral arteries during orthostatic stress to presyncope in 9 healthy subjects. There was no significant difference in either the decline in cerebral blood flow velocity or indices of dynamic cerebral autoregulation between the middle cerebral and vertebral arteri

Effects of testosterone replacement therapy on cortical and trabecular bone mineral density, vertebral body area and paraspinal muscle area in hypogonadal men.

Loss of bone and muscle mass are major findings of male hypogonadism. In order to determine the long-term effect of testosterone replacement therapy on spinal bone and muscles, the trabecular and cortical bone mineral density, vertebral body area and paraspinal muscle area were assessed by quantitative computed tomography in 32 testosterone-substituted patients, aged 18-74 years, with idiopathic hypogonadotropic hypogonadism (n=6), pituitary insufficiency (n=5), Klinefelter syndrome (n=12) or other forms of primary hypogonadism (n=9). They were followed for a mean period of 3.2+/-1.7 years (me…

A Case of Pyogenic Vertebral Osteomyelitis and Iliopsoas Abscess Caused by Invasive Pneumococcal Disease Serotype 35F: Utility of Diffusion-Weighted Whole-Body Imaging With Background Body Signal Suppression as an Adjunctive Diagnostic Tool.

We present a case of invasive pneumococcal disease (IPD) complicated by both iliopsoas abscess and pyogenic vertebral osteomyelitis caused by serotype 35F Streptococcus pneumoniae in a 64-year-old man with a history of splenectomy who was unvaccinated. The patient experienced difficulty moving, severe back pain, vomiting, and high fever. Advanced imaging techniques, including T2-weighted lumbar MRI and diffusion-weighted whole-body imaging with background body signal suppression (DWIBS), revealed hyperintensities in the left iliopsoas muscle and L4-L5 vertebral bodies, facilitating diagnosis.
