The direction of apparent movement during transient pressure vertigo.

Pressure vertigo or alternobaric vertigo is the type of vertigo normally associated with relative overpressure in the middle ear. As with other types of vertigo, it should occur only if there is unequal stimulation of the left and right vestibular systems: there should be a lawful relation between the orientation of the head, the side of the overpressured ear, and the direction of apparent movement. Few published accounts give information on all these aspects. This paper reports some old and new cases which suggests that, when the head is upright during the ascent, overpressure in the right ea…

The Impact of Follow-Up on Etiological Classification of Pediatric Vertigo.

Background: Vertigo and dizziness in children represent diagnostically challenging conditions with heterogeneous etiologies. At initial presentation, a substantial proportion of pediatric patients remain without a definitive etiological diagnosis. Evidence on the impact of longitudinal follow-up on etiological classification in pediatric vertigo is limited. Methods: This observational cohort study uses prospectively collected clinical data. Children aged 1-17 years who presented to a tertiary ENT clinic with vertigo and/or dizziness between 2015 and 2020 were systematically enrolled and follow…

Living With the Fear of Recurrence in Benign Paroxysmal Positional Vertigo.

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) is a common vestibular disorder characterized by brief vertigo episodes. It often leads to functional limitations, psychological distress, and fear of recurrence. OBJECTIVE: The purpose of this phenomenological study was to gain a deep understanding of the lived experiences of patients with BPPV. METHODS: Participants were initially recruited through convenience sampling at a BPPV specialist clinic, with additional participants identified via snowball sampling. Data were gathered through individual, in-depth interviews. FINDINGS: A cen…

Vertigo and dizziness due to vertebrobasilar TIA: a prospective study.

PURPOSE: Prospective studies on vascular vertigo and dizziness (VVD) due to vertebrobasilar transient ischemic attack (VBTIA) have been sparse. This study aimed to characterize clinical features, response to treatments, and prognostic factors of VVD due to VBTIA using a cohort established in 2021. METHODS: We recruited 103 patients (58 female individuals, 56.3%), with a mean age of 70.9 ± 9.3 years (range = 37-85), between January 2021 and January 2024. All patients met the diagnostic criteria of "Probable transient VVD" published by the Bárány Society. The mean interval from symptom onset to…

Vertigo in the Setting of COVID-19 Infection: A Case Report.

Extensive research has been conducted on the pathophysiological sequelae of the SARS-CoV-2 virus. Newer symptoms pertaining to cardiovascular, gastrointestinal, and neurological systems, apart from the recognized respiratory system, are constantly being reported. We report a case of a 22-year-old female with a seven-day history of COVID-19 who presented to the emergency department with an acute vertigo attack associated with nausea and vomiting for three hours. Physical examination and audiometry showed no neurological and auditory deficits; she was diagnosed with vestibular neuritis. The pati…

Vertigo: An Atypical Presentation of Neurocysticercosis Successfully Treated With Albendazole.

A 20-year-old male presented to our facility with a worsening sensation of "the room spinning around" himself for the past three weeks. In the last week, he began to experience daily spells lasting for three hours each without losing consciousness. The patient had recently migrated from Central America six weeks prior to admission. On physical examination, his vital signs were within normal limits, with no focal neurological deficits. Magnetic resonance imaging (MRI) of the brain revealed a cystic-appearing lesion in the fourth ventricle with associated mass effect on the posterior aspect of t

Facts of Vertigo in Adolescents: Controversies and Challenges - A Narrative Review.

Vertigo is a common complaint in the general population affecting 5% of adults in one year. At least 29.5% of adults have referred vertigo during life. Even though the prevalence of vertigo is well known in adults the epidemiologic data in adolescents is sparse. To date, it is known that adolescent females are usually affected by vertigo and some conditions such as depression and anxiety are found in this population. However, the lack of information about the prevalence of most common types of vertigo in adolescents, predisposing factors, challenges, and controversies in clinics in the literat…

Benign Paroxysmal Positional Vertigo: Is It Really an Otolith Disease?

The current theory in physiopathology of benign paroxysmal positional vertigo is the mechanical theory, namely the cupulolithiasis-canalolithiasis theory. Repositioning maneuvers based on this theory has now taken place in therapy. However, mechanical theory is insufficient to explain some clinical situations and cannot fully enlighten the physiopathology. Mechanical theory is based on very few histological studies. Currently, these few articles are still used for reference. Anatomically, there are uncertainties that need to be explained in this theory. In this literature review, the histologi…

The Spectrum of Vestibular Disorders Presenting With Acute Continuous Vertigo.

OBJECTIVE: To explore the composition of vestibular disorders presenting with the acute vestibular syndrome (AVS). METHODS: We performed a case analysis of 209 AVS patients between January 2016 and December 2020. These patients were grouped into different disorder categories according to the relevant diagnostic criteria. RESULTS: We classified the 209 patients into 14 disorder categories, including 110 cases of vestibular neuritis, 30 of idiopathic sudden sensorineural hearing loss with vertigo, 17 of the first attack of continuous vertigo with migraine, 15 of Ramsay Hunt syndrome, 11 of acu…

Relation of infarction location and volume to vertigo in vertebrobasilar stroke.

OBJECTIVE: Vertigo is a common presentation of vertebrobasilar stroke. Anecdotal reports have shown that vertigo occurs more often in multiple than in single brainstem or cerebellar infarctions. We examined the relation between the location and volume of infarction and vertigo in patients with vertebrobasilar stroke. METHODS: Consecutive patients with vertebrobasilar stroke were prospectively recruited. The infarction location and volume were assessed in the diffusion-weighted magnetic resonance imaging. RESULTS: Fifty-nine patients were included, 32 (54.2%) with vertigo and 27 (45.8%) witho…

Hodgkin's lymphoma presenting with an unusual horizontal Nystagmus and vertigo.

Neurological presentation of paraneoplastic syndromes is rare. They are often difficult to diagnose, especially when they precede the diagnosis of cancer. Hodgkin's lymphoma is associated with several paraneoplastic neurological syndromes such as cerebellar degeneration and dermato/polymyositis. Peripheral vestibular syndrome is uncommon presentation of these paraneoplastic syndromes. We report the case of a 52-year-old man with no prior medical history who presented to the otolaryngology clinic with vertigo precipitated by nystagmus, nausea and weight loss. Diagnostic workup revealed a nodula…

Effects of Peripheral Vertigo on Inflammatory and Immunologic Laboratory Markers.

The neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and mean platelet volume (MPV) are markers of subclinical inflammation and atherothrombosis. We investigated whether these easily accessible blood count parameters are affected by peripheral vertigo (PV). We studied 142 patients who were diagnosed with PV and referred to our hospital along with 135 healthy patients who were free of inflammation as a control group. The patient group was divided into 3 study groups depending on the presence of benign paroxysmal positional vertigo (BPPV): BPPV, vestibular dysfunction (V…

Repetitive Episodic Isolated Vertigo in a Patient with Cerebellar Infarction.

Isolated vertigo is an important symptom of posterior circulation stroke. It has been reported that 11.3% of patients with isolated vertigo have a stroke and that most lesions are located in the cerebellum, particularly in the posterior inferior cerebellar artery. We report the case of a 63-year-old man with multiple atherosclerotic risk factors and atrial fibrillation who showed repeated episodes of isolated vertigo. His repeated vertigo was short-lasting and was often triggered by body position, mimicking benign paroxysmal positional vertigo. Cranial computed tomography on the third hospital…

Comparison of the effects of the temperature of intratympanic dexamethasone injections on vertigo.

OBJECTIVE: This study aimed to compare if vertigo improved in patients when the dexamethasone used in the intratympanic (IT) injection was applied at body temperature or at room temperature. METHODS: The study included 54 patients who had undergone intratympanic treatment due to sudden hearing loss and tinnitus. The IT injection was administered to all patients, 2 times with 1-day intervals. Two different IT injection techniques were used for each patient: injecting dexamethasone at room temperature and injecting it at body temperature. Patients were asked to report the vertigo they felt imme…

Barometric pressure and the incidence of benign paroxysmal positional vertigo.

OBJECTIVES: To investigate the relationship between barometric pressure and the incidence of benign paroxysmal positional vertigo (BPPV). METHODS: 181 patients diagnosed with classic BPPV seen between 2011 and 2016 were identified. Demographic information, data of onset, and date of presentation were recorded. Historical barometric data for each of the 60 months were recorded. In addition, monthly counts of other atmospheric, infectious, and allergic variables for that time period were recorded. Correlation analysis compared monthly incidence of BPPV with absolute and relative changes in atmo…
