Characteristics of traumatic brain injuries sustained among veterans seeking homeless services.

This hypothesis-generating research describes the characteristics of traumatic brain injuries (TBIs) sustained among 229 Veterans seeking homeless services. Nearly all participants (83%) had sustained at least one TBI prior to their first episode of homelessness. Among participants with a TBI, assaults, transportation-related accidents, and falls were the most common causes of these injuries. Thirty percent of individuals sustained injuries with severity levels that would be expected to be associated with ongoing TBI-related deficits. Forty-three percent of the Veterans sustained at least one …

QTc Prolongation in Veterans With Heroin Dependence on Methadone Maintenance Treatment.

BACKGROUND: QTc prolongation and Torsade de Ppointes have been reported in patients on methadone maintenance. OBJECTIVES: In this study, QTc was compared before and after the veteran (n = 49) was on a stable dosage of methadone for 8.72 ± 4.50 years to treat heroin dependence. Risk factors were correlated with the QTc once the veteran was on a stable dose of methadone. Differences in the clinical risk factors in subgroups of veterans with below and above mean QTc change was compared. PATIENTS AND METHODS: ECG data was obtained from a 12-lead electrocardiogram (pre-methadone and on methadone)…

Latent classes of PTSD symptoms in Iraq and Afghanistan female veterans.

BACKGROUND: Recent studies have used latent class analysis (LCA) to identify subgroups of individuals who share similar patterns of PTSD symptom endorsement; however, further study is needed among female veterans, whose PTSD symptom expression may vary from that of their male counterparts. The current study examined latent PTSD symptom classes in female veterans who returned from recent military service in Iraq and Afghanistan, and explored military and demographic variables associated with distinct PTSD symptom presentations. METHODS: A retrospective analysis was conducted using existing med…

Pharmacogenetics of naltrexone and disulfiram in alcohol dependent, dually diagnosed veterans.

BACKGROUND: Disulfiram and naltrexone were evaluated in treatment of individuals with co-occurring alcohol dependence and other Axis I disorders (e.g., Major Depression). We explored pharmacogenetic interactions in genotyped subjects. METHODS: Alcohol dependent (AD) subjects received naltrexone alone, placebo alone, disulfiram with placebo or disulfiram with naltrexone. They were genotyped for OPRM1 rs1799971 (Asn40Asp), and DBH rs1611115 (C-1021T). N = 107 male European-American subjects were included. RESULTS: There were no significant interactions with OPRM1. DBH interacted with naltrexon

Street outreach and other forms of engagement with literally homeless veterans.

Street outreach is one of the most direct methods of engaging homeless individuals, but the characteristics of those most likely to be engaged this way is not well-understood. Data from the Department of Veterans Affairs (VA) Homeless Operations Management and Evaluation System showed that of the 70,778 literally homeless veterans engaged in VA homeless services in 2011-2012, 12% were through street outreach while the majority was through provider referrals (41%) and self-referrals (28%). Veterans engaged through street outreach had more extensive histories of recent homelessness, were more li…

Glaucoma medication adherence in veterans and influence of coexisting chronic disease.

BACKGROUND: The aim of this study was to characterize veterans' adherence to glaucoma medications and follow-up eye care, and to examine the influence of coexisting chronic conditions such as posttraumatic stress disorder (PTSD)/anxiety. PATIENTS AND METHODS: A retrospective review of computerized medical records was performed for 161 veterans taking topical glaucoma medications. Medication possession ratio (MPR) was calculated using pharmacy data. Mean follow-up ratio (FUR) was calculated using the number of days between visits. RESULTS: Among the health conditions examined, the most preval…

SETTING: Veterans Affairs Medical Center (VAMC). PARTICIPANTS: Older adults who underwent total hip and knee replacement (THR and TKR) from 2002 to 2009. MEASUREMENTS: Using multivariate logistic regression, the independent effect of cardiopulmonary comorbidities and diabetes on VTE was analyzed. Functional status expressed in a summary physical component score (PCS) was also analyzed in a subset of individuals in …

Significant life Events and PTSD in Elderly World War II Veterans.

The authors reviewed clinic records of 20 WWII veterans referred for evaluation of posttraumatic stress disorder (PTSD). Twelve patients qualified for a diagnosis of PTSD, and eight reported several symptoms of PTSD. The most frequent symptoms were insomnia (80%), nightmares (75%), and irritability (65%). Fourteen veterans had an exacerbation of symptoms occurring with life stresses, including retirement, deteriorating health, or death of a loved one. The authors conclude that elderly WWII combat veterans should be monitored for changes in PTSD symptoms when significant stressful life events o…

Lifetime major depression and comorbid disorders among current-era women veterans.

BACKGROUND: Most research on women veterans' mental health has focused on postraumatic stress disorder (PTSD) or reactions to military sexual trauma. Although depression is also a frequent diagnosis among women veterans, little is known about its characteristics, including comorbid conditions and patterns of disorder onset. We investigated lifetime diagnoses of major depressive disorder (MDD) and comorbid conditions in a primarily treatment-seeking research sample of male and female veterans to determine frequency of lifetime MDD, comorbid disorders and their temporal onset. METHOD: The 1700 …

Hazardous drinking and its association with homelessness among veterans in care.

BACKGROUND: While scholarship on alcohol use and homelessness has focused on the impact of alcohol abuse and dependence, little is known about the effects of lower levels of misuse such as hazardous use. Veterans receiving care in the Department of Veterans Affairs Health Care System (VA) constitute a population that is vulnerable to alcohol misuse and homelessness. This research examines the effects of hazardous drinking on homelessness in the Veterans Aging Cohort Study, a sample of 2898 older veterans (mean age=50.2), receiving care in 8 VAs across the country. METHODS: Logistic regression…

Elderly homeless veterans in Los Angeles: chronicity and precipitants of homelessness.

OBJECTIVES: We compared the characteristics of chronically homeless and acutely homeless elderly veterans to better understand precipitants of homelessness. METHODS: We conducted interviews with 33 chronically and 26 acutely homeless veterans aged 65 years and older receiving transitional housing services in Los Angeles, California, between 2003 and 2005. We asked questions regarding their sociodemographic characteristics and other social status measures. Other precipitants of homelessness were acquired via observation and open-ended and structured questions. RESULTS: Both veterans groups we…

Serum urate and incidence of kidney disease among veterans with gout.

OBJECTIVE: To study the association between serum urate level (sUA) and the risk of incident kidney disease among US veterans with gouty arthritis. METHODS: From 2002 through 2011 adult male patients with gout who were free of kidney disease were identified in the data from the Veterans Administration VISN 16 database and were followed until incidence of kidney disease, death, or the last available observation. Accumulated hazard curves for time to kidney disease were estimated for patients with average sUA levels > 7 mg/dl (high) versus ≤ 7 mg/dl (low) based on Kaplan-Meier analyses; and sta…

Systematic review of women veterans' health: update on successes and gaps.

OBJECTIVE: We assessed the state of women veterans' health research by conducting a systematic review of scientific literature published from 2004 to 2008, updating a prior review spanning the history of this literature to 2004. METHODS: We identified articles by searching scientific databases and contacting experts. Relevant articles were independently evaluated by two physician reviewers. We categorized 195 articles by study design, funding source, period of military service, research topic, and health condition. RESULTS: More research was published during this 5-year review (n = 195) than…
